Efficacy of Home-Based Versus Hospital-Based Disimpaction with Polyethylene Glycol in Pediatric Functional Constipation: A Randomized Control Study.

BACKGROUND/AIMS: Functional constipation (FC) is the most common cause of childhood constipation. Children with FC and fecal impaction need to undergo disimpaction. Polyethylene glycol (PEG) is commonly used for disimpaction either in a hospital setting as a continuous lavage or in a home-based setting over 3-6 days. Randomized control trials comparing the two regimens are not available. The study objectives were to compare the efficacy, side effects, cost-effectiveness, and parental satisfaction of home- and hospital-based disimpaction. METHODS: Consecutive children attending our hospital (a

Exploring Stakeholder Perspectives on the Transitional Care Needs of Elderly Patients from Hospital to Home: A Phenomenological Study in Shanxi Province, China.

BACKGROUND: Elderly patients, due to their complex medical conditions and extensive care needs, are at risk of experiencing low-quality or fragmented care during transitions between different healthcare settings. After transitioning from hospital to home, inadequate self-care abilities may result in further health deterioration and increased risks of adverse outcomes. Currently, China lacks effective transitional support services from hospital to home, hindering the smooth transition for elderly patients. Therefore, understanding the specific care needs of elderly patients during this period p

[Discussion on care management operation of a visiting nurse--a case of increased ADL by the support of independent life].

In addition to the visiting nursing service conventionally provided, the Department of Long-term Care Insurance Service of this hospital inaugurated the home care supporting service in April 2000. Senior citizens rated higher in the degree of necessity of care in the initial accreditation and in the renewal accreditation of the Long-term Care Insurance tend to have fewer changes in the services for the last two years. At present, care managers of various professions are involved in the home care supporting services and have no choice but to provide care in non-specialty areas. Under the situat

Family experiences of paediatric palliative care during hospital-at-home: a systematic review.

PURPOSE: This systematic review explores the family experiences of hospital-at-home (HaH) care for children in paediatric palliative care (PPC). While the HaH model is increasingly promoted as a safe and cost-effective alternative to hospital-based care, particularly in paediatrics, little is known about how families and children in PPC actually experience this form of care. METHODS: We conducted a comprehensive search across five databases: PsycArticles, PsycInfo, PubMed, ScienceDirect, and Scopus. The search was limited to articles published between 2010 and 2023, in English or French, invo

Daily struggles: living with long-term childhood technology dependence.

With advances in technology, children with chronic illnesses are surviving longer. This improved survival rate has resulted in a growing population of families caring for their children with complex needs at home. This grounded theory study explored how caring for a child who requires home ventilation affects family members. Families moved through a process of struggling daily as they cared for their child with complex needs. This ongoing struggle was characterized by three distinct dimensions: getting over the hump, starting to breathe, and having to deal with a subsequent event. Implications

A Case Study of Comprehensive Postoperative Management for Elderly Total Hip Arthroplasty Patients Based on Peplau's Interpersonal Relationship Theory.

With a growing population of elderly patients undergoing hip arthroplasty, traditional nursing care often faces challenges dus to fragmented services and inadequate continuity. Based on Peplau's interpersonal relationship theory, we explored a comprehensive hospital-home-community management model for an 80-year-old female patient following total hip arthroplasty. This nursing model was structured into four sequential phases (orientation, identification, exploitation, and resolution) and incorporated a structured multidisciplinary team, a stepwise health education system, discharge preparation

The future of community nursing: Hospital in the Home.

With an increasing ageing population who often have multiple long-term conditions, there is a growing need to provide an alternative type of care to the traditional hospital-based model. 'Hospital in the Home' is a model that provides integrated care for patients in their home. The @home service was established in 2013 by Guy's and St Thomas' NHS Foundation Trust. The service provides health care in patients' home, supporting early discharge from hospital as well as preventing avoidable admissions and readmissions saving valuable hospital bed days and reducing length of stay. An innovative model of system-linked community care: home-based traction as an alternative to institutional treatment.

Home-based traction is an alternative treatment to conventional hospital-based traction for children with orthopedic conditions such as congenital dislocated hip and Legg-Perthes disease. The application of a proposed theoretical model is used to describe the process of the home-based traction innovation. Creativity and innovation occasions a reconceptualization of stresses. The reconceptualization of stress, plus the operational support of coordinated in-home services, can help families and nurses in the community and in the hospital develop, implement, and evaluate this kind of innovative ca

Hospital-at-home programs for patients with acute exacerbations of chronic obstructive pulmonary disease (COPD): an evidence-based analysis.

UNLABELLED: In July 2010, the Medical Advisory Secretariat (MAS) began work on a Chronic Obstructive Pulmonary Disease (COPD) evidentiary framework, an evidence-based review of the literature surrounding treatment strategies for patients with COPD. This project emerged from a request by the Health System Strategy Division of the Ministry of Health and Long-Term Care that MAS provide them with an evidentiary platform on the effectiveness and cost-effectiveness of COPD interventions. After an initial review of health technology assessments and systematic reviews of COPD literature, and consultat

Hybrid Hospital-at-Home Program in Singapore: Ethnographic Study.

BACKGROUND: Hospital-at-Home (HaH) programs deliver hospital-level care in the home setting, traditionally emphasizing high-touch, in-person clinical visits. In recent years, especially during the COVID-19 pandemic, HaH models have rapidly evolved to incorporate digital technologies, enabling remote consultations and monitoring as part of care delivery. While both in-person and remote HaH modalities have individually shown positive outcomes with patient experience and clinical safety, less is known about how hybrid models, combining both modalities, are experienced by patients and caregivers.
