Low use of medication in home deliveries in the Netherlands.

OBJECTIVES: In view of the growing concern for de-medicalizing childbirth, the aim of this study is to give detailed figures on the use of medication during home deliveries in the Netherlands. METHODS: A prospective study of medication use by 68 community midwives during 716 home births in the Netherlands. RESULTS: Medication was used in 58.4% of the home deliveries, with an average of 1.4 drugs per delivery. The drugs used were mostly oxytocin (in 35.6% of all deliveries) and local anesthetics (in 32.9%). When medication was used, it was administered before cutting the umbilical cord in 16.

Women's experiences of planned home birth in Sweden: a netnographic study.

BACKGROUND: Planned home birth is an option for healthy women with low-risk pregnancies and is attended by midwives in the home setting. While well established in several countries, planned home birth remains uncommon in Sweden. Available evidence suggests that, for selected women, home birth can be as safe as hospital birth and may be associated with greater autonomy, control, and more positive birth experiences. However, women's experiences of planned home birth in Sweden remain underexplored. OBJECTIVES: To explore women's experiences of planned home birth attended by midwives in Sweden.

Exploring decision making to create an active offer of planned home birth.

BACKGROUND: Despite planned home birth having the potential to provide a psychologically and physically safe birth environment, home birth is unusual within most highly-resourced countries. Women are not always provided with the opportunity or autonomy to birth at home. Additionally, amongst the small numbers of women who do birth at home, minoritised and marginalised communities are underrepresented. AIMS: To explore if an 'active offer' concept may provide a useful approach to offering planned home birth. METHODS: This was a sequential multiple methods study, to explore the use of the acti

Can Clean Delivery Kits Prevent Infections? Lessons from Traditional Birth Attendants in Nigeria.

BACKGROUND: In resource-poor settings, perinatal infections contribute significantly to maternal and neonatal deaths, and the use of clean delivery kits (CDKs) has been proposed as a tool to reduce the risk of infection-related deaths. This study aims to assess the acceptability and effectiveness of CDKs in preventing infections in deliveries attended by traditional birth attendants (TBAs) in Abeokuta, Nigeria. METHODS: The study was a cluster-randomized trial with 67 birth centres/clusters, 453 births/mothers, and 457 babies randomized to intervention or control arms; intervention involved s

Interest in Home Birth During the COVID-19 Pandemic: Analysis of Google Trends Data.

INTRODUCTION: Nearly all (94%-99%) pregnant persons in developed countries search for pregnancy-related information online. The advent of the novel coronavirus disease 2019 (COVID-19) and the associated restrictions in hospital policies may have pushed pregnant persons in the United States to consider giving birth at home to achieve their desired birth experience. METHODS: Google Trends is an open, rich source of real-time, anonymized, relative data on disease patterns and population behavior that provides data in the form of search volume index (SVI): the search volume for a queried term rel

Suspected maternal infanticide in a case of hydranencephaly.

A medico-legal autopsy case of hydranencephaly in a male infant which was first suspected of maternal infanticide is reported. The infant was 48 cm in height, weighed 2.86 kg and the circumference of the head, the chest and the abdomen was 32.2 cm, 31.0 cm and 30.4 cm, respectively, with no deformities of the head or body. Autopsy examination, however, revealed a severe defect in the central nervous system. The cranial cavity was filled with a cloudy dark red fluid (ca. 310 ml) instead of the cerebral hemispheres. The residual central nervous tissues were mostly subtentorial structures from th

Examining women's choice between home and institutional births: Insights from the Salud Mesoamérica Initiative (SMI).

This study examines women's childbirth choices between home and institutional settings in the context of the Salud Mesoamérica Initiative, a results-based financing program focused on disadvantaged populations. This study characterizes the childbirth experience using the World Health Organization's Positive Childbirth Experience framework, identifies factors influencing women's decisions for home versus institutional births, and describes reasons for transitions between these settings, especially for women with multiple pregnancies. A retrospective observational study was carried out using sec

Is fear of childbirth related to the woman's preferred location for giving birth? A Dutch low-risk cohort study.

BACKGROUND: In The Netherlands, women with low-risk pregnancy are routinely given the option of home birth, providing a unique opportunity to study the relationship between fear of childbirth (FOC) and preference for childbirth location, and whether women experience higher FOC when the actual location differs from their preference. METHODS: In this prospective cohort study, 331 nulliparous and parous women completed a questionnaire at gestational week 30 (T1) and two months postpartum (T2). FOC was assessed using versions A (T1) and B (T2) of the Wijma Delivery Expectancy/Experience Questionn

Spontaneous Perineal Trauma during Non-Operative Childbirth-Retrospective Analysis of Perineal Laceration Risk Factors.

Childbirth-related perineal trauma (CRPT) is defined as damage to the skin, muscles of the perineum, as well as to the anal sphincter complex and anal epithelium. The aim of the study was to analyze the risk factors for spontaneous injuries to the soft tissues of the birth canal during non-operative delivery. This was a single-center retrospective case-control study. The study included the analysis of two groups, the study group featured 7238 patients with spontaneous perineal laceration (any degree of perineal laceration) and the control group featured patients without perineal laceration wit

Midwifery Provision of Home Birth Services.

The number of individuals choosing to give birth at home has increased significantly in the United States over the past decade. It is essential to gain a greater understanding of the evidence regarding the provision of midwifery care to people considering this option. The safety of planned home birth continues to be debated and studied primarily through observational research both globally and within the United States. Findings from well-designed studies demonstrate that pregnant people who are essentially healthy at term with a singleton fetus and choose a planned home birth have positive out

Barriers and enablers to emergency obstetric and newborn care services use in Wolaita Zone, Southern Ethiopia: a qualitative case study.

BACKGROUND: Globally, 11.4 million untreated obstetric complications did not receive Emergency Obstetric and Newborn Care (EmONC) services yearly, with the highest burden in low and middle-income countries. Half of the Ethiopian women with obstetric complications did not receive EmONC services. However, essential aspects of the problem have not been assessed in depth. This study, therefore, explored the various aspects of barriers and enablers to women's EmONC services utilization in southern Ethiopia. METHODOLOGY: A qualitative case study research design was used in nine districts of the Wol

Trends and predictors of the use of unskilled birth attendants among Ethiopian mothers from 2000 to 2016.

INTRODUCTION: Previous studies have shown that home delivery assisted by unskilled individuals contributes to maternal and neonatal deaths in developing countries. It also increases the risks of long-term maternal morbidities such as uterine prolapse, pelvic inflammatory disease, fistula, incontinence and infertility. This study aimed to determine the trends and predictors of the use of unskilled birth attendants among Ethiopian mothers from 2000 to 2016. METHODS: This study USED used the Ethiopia Demographic and Health Survey data for the years 2000 (n = 10,873), 2005 (n = 9,861), 2011 (n =

Justified skepticism about Apgar scoring in out-of-hospital birth settings.

BACKGROUND: The Apgar score is used worldwide to assess the newborn infant shortly after birth. Apgar scores, including mean scores and those with high cut-off scores, have been used to support claims that planned home birth is as safe as hospital birth. The purpose of this study was to determine the distribution of 5 min Apgar scores among different birth settings and providers in the USA. METHODS: We obtained data from the National Center for Health Statistics of the US Centers for Disease Control birth certificate data for 2007-2010 for all singleton, term births of infants weighing ≥2500

Bilirubin encephalopathy in a term infant after planned home delivery.

A term infant was delivered at home by his father after a pregnancy which involved no organized medical care. By the third day of life the infant developed fever, poor feeding and severe jaundice. The infant was found to have a serum indirect bilirubin of 49 mg/dl secondary to isoimmune hemolytic anemia due to anti-c antibody. The infant survived but suffers from clinical manifestations of kernicterus.

Phototherapy: the hospital as risk factor.

In a retrospective study over six years the incidence of phototherapy was examined in two groups of healthy neonates who were born spontaneously and at term in hospital. They were comparable in all respects except that one group was cared for at home and the other group was cared for in hospital. It appeared that the infants in hospital received phototherapy seven times more often than those at home, and surveillance at home was not inferior to that in hospital. There is no reason to assume that neonatal jaundice occurred more often in neonates in hospital than in those at home. Thus the diffe

Home confinement: the positive results in Holland.

This study was an objective evaluation of the outcome of home confinement, which is still common in Holland, compared with delivery in hospital. To this end, 1,692 women in Groningen were interviewed three weeks after confinement to find out how their pregnancy, delivery and childbed period had progressed according to the place of confinement, and to obtain facts about the morbidity of their babies.Among women who had opted for home confinement (if allowed) significantly fewer complications occurred during pregnancy, delivery or puerperium than among those who had their babies in hospital.
