Liberty and the individual: the colony asylum in Scotland and England.

This paper analyses the buildings, spaces and interiors of Bangour Village public asylum for the insane, near Edinburgh, and compares these with an English asylum, Whalley, near Preston, of similar early-twentieth-century date. The village asylum, which developed from a European tradition of rendering the poor productive through 'colonisation', was more enthusiastically and completely adopted in Scotland than in England, perhaps due to differences in asylum culture within the two jurisdictions. 'Liberty' and 'individuality', in particular, were highly valued within Scottish asylum discourses,

The Role of the Built Environment: How Decentralized Nurse Stations Shape Communication, Patient Care Processes, and Patient Outcomes.

Increasingly, health communication scholars are attending to how hospital built environments shape communication, patient care processes, and patient outcomes. This multimethod study was conducted on two floors of a newly designed urban hospital. Nine focus groups interviews were conducted with 35 health care professionals from 10 provider groups. Seven of the groups were homogeneous by profession or level: nursing (three groups), nurse managers (two groups), and one group each of nurse care technicians ("techs") and physicians. Two mixed groups were comprised of staff from pharmacy, occupatio

An Improved Ward Architecture for Treatment of Patients with Ebola Virus Disease in Liberia.

During the recent outbreak of Ebola virus disease (EVD) in west Africa, we established an Ebola treatment center (ETC) with improved ward architecture. The ETC was built with movable prefabricated boards according to infectious disease unit standard requirements. The clinical staff ensured their own security while providing patients with effective treatment. Of the 180 admissions to the ETC, 10 cases were confirmed with EVD of which six patients survived. None of the clinical staff was infected. We hope that our experience will enable others to avoid unnecessary risks while delivering EVD care

Ward design and neonatal jaundice in the tropics: report of an epidemic.

Architectural modifications to an existing tropical obstetric ward involved extension of the roof overhangs to a width of several metres. These extensions excluded most of the daylight from the ward. An alarming increase in the incidence of jaundice (bilirubin concentration greater than or equal to 240 mumol/l (greater than or equal to 14 mg/100 ml] from 0.5% to 17% in newborn infants occurred after the modifications. Tropical obstetric wards and nurseries should continue to be built with windows facing north-south for coolness. They should, however, have as many windows as possible and the ro

Prevention of healthcare-associated invasive aspergillosis during hospital construction/renovation works.

The association between healthcare-associated invasive aspergillosis and hospital construction/building works is well recognized. This infection can cause significant morbidity and mortality and imposes a substantial burden on the healthcare system. The population of patients at risk for this opportunistic infection has expanded and multi-triazole drug resistance has emerged globally. Hence the need for a multi-faceted approach to prevent acquisition of invasive aspergillosis in acute care settings. This article is a summary of the Irish National Guidelines for the prevention of healthcare-ass

Literature Review: Evidence-Based Health Outcomes and Perceptions of the Built Environment in Pediatric Hospital Facilities.

PROBLEM: The current knowledge of evidence-based design for adults is not always implemented when hospital buildings are designed. Scientific data are sparse on the effects of hospital design in pediatric settings on health outcomes in children, parents, and staff. The objective of this review is to determine the evidence-based impact of the built environment in pediatric hospital facilities on health outcomes in children, parents, and staff. ELIGIBILITY CRITERIA: A systematic literature review was carried out on the electronic databases Cochrane Library, Embase, Medline and CINAHL from the p

Could we predict airborne Aspergillus contamination during construction work?

BACKGROUND: Aspergillus fumigatus is a major opportunistic pathogen causing nosocomial infection. Hospital outbreaks of invasive aspergillosis have been associated with demolition and building construction. This study was designed to examine the impact of meteorologic factors and different periods of work on outdoor fungal airborne concentrations. METHODS: The study was conducted at Necker Enfants Malades Hospital, a 650-bed teaching care hospital recently involved in a large construction program, including renovation, construction, and demolition. During the work phases, prospective external

Designing a hybrid surgical ICU to avoid intrahospital transport in selected critically Ill patients: the CUHK-SZ institutional concept.

BACKGROUND: Intrahospital transport for surgery exposes critically ill patients to equipment failure, displacement of tubes and lines, interrupted monitoring, and physiologic instability. Although transport protocols can reduce these hazards, they do not address the underlying physical decoupling of surgical infrastructure from the critical care environment. OBJECTIVE: This institutional perspective describes the rationale and design of a hybrid surgical ICU planned for the Chinese University of Hong Kong, Shenzhen (CUHK-SZ) Medical Center: a dedicated clinical space designed to function as b

Uncovering 'Weak signals' through patient journey mapping to inform the future redesign of the medical consultation room.

BACKGROUND: Designing hospital environments requires balancing functional efficiency with patient-centered care. Subtle environmental and procedural frictions, often overlooked in traditional evaluations, can significantly affect patient experiences. This study explored how Patient Journey Mapping (PJM), and Mobile Ethnography (ME) can identify weak signals (recurring yet understated signs of discomfort, confusion, or disempowerment) and translate them into actionable design improvements in healthcare settings. METHODS: A case study was conducted in a private hospital in Portugal. Patients doc

The Creation of a Biocontainment Unit at a Tertiary Care Hospital. The Johns Hopkins Medicine Experience.

In response to the 2014-2015 Ebola virus disease outbreak in West Africa, Johns Hopkins Medicine created a biocontainment unit to care for patients infected with Ebola virus and other high-consequence pathogens. The unit team examined published literature and guidelines, visited two existing U.S. biocontainment units, and contacted national and international experts to inform the design of the physical structure and patient care activities of the unit. The resulting four-bed unit allows for unidirectional flow of providers and materials and has ample space for donning and doffing personal prot
