Perspectives of physicians on risk factors for patient aggression and violence against physicians in Chinese hospitals: a Q-methodology study.

BACKGROUND: The prevalence of patient (and their relatives/friends) aggression and violence against healthcare professionals in general, and physicians in particular, is a recognized problem worldwide. While numerous risk factors for such aggression and violence from patients (and their relatives/friends) have been identified, little is known about which risk factors are perceived as relatively most important in a specific context and among a particular group, and about the potentially differing views on the relative importance. This lack of insight prohibits preventive measures being tailored

Do psychiatric departments in general hospitals have an impact on the physical health of mental patients?

PURPOSE OF REVIEW: Recent literature has increased awareness of the physical health problems that affect mental-health patients. The close association of psychiatric departments with other medical departments can lead to improved communication and support between all medical specialties. This review assesses the impact these developments have on the physical health of individuals with psychiatric disorders. RECENT FINDINGS: The current literature shows that mentally ill individuals have increased physical illness comorbidity. They do not regularly attend screening programmes or follow-up appo

[Mental illnesses in medical certificates in 18 century Hessen].

This study deals with the medical certificates (Attestata medica) for mentally disturbed or insane and mad individuals which were issued by physicians (Stadt-/Landphysici) between 1750 and 1807 to gain admission to two of the Hessen High Hospitals (Hessische Hohe Hospitäler), Haina and Merxhausen. They provide an approach to mental illness in the 18th century from two perspectives: a study of coping with sickness, especially in the countryside, and mirrored by the doctor's view. In undoubted cases of madness the physician appears merely as a representative of public health care (Medizinische P

How Uncertainty Is Talked About in Language Discordant and Concordant Health Consultations: Patterns of Epistemic Adverb Use and Communication Accommodation Strategies.

INTRODUCTION: In this study, we explored how uncertainty was expressed through epistemic adverbs in health consultations where patients and health practitioners (HPs) spoke the same language (language-concordant) versus when they did not (language-discordant). Further, we applied Communication Accommodation Theory (CAT) to identify, in these settings, how HPs used communication strategies and their patients' communicative responses. METHOD: Fifty-three health consultations between bilingual and monolingual patients and their HPs in a metropolitan emergency department were recorded and transcr

Experiences and perspectives related to shared decision-making among outpatients with degenerative joint disease in Taiwan: a qualitative study.

OBJECTIVES: Various treatment options are available for degenerative joint disease (DJD). SETTING: Primary care clinics of a regional teaching hospital in Taiwan, October 2021-May 2022. PARTICIPANTS: 21 outpatients with at least three visits for DJD and who were

[Appropriate use of strong opioids in medical units: Recommendations and action to be taken in daily practice].

The use of strong opioids in medical units is recurrent, mainly for analgesic purposes. The risk of occurrence of an overdose or an opioid use disorders causes very legitimate concerns for the physician, which may limit the use of opioid treatment or the adaptation of the doses necessary to relieve the patient. We provide a summary of the literature aimed at defining the indications, the adverse effects and the risks involved, the prescribing methods in order to reassure professionals and promote the safe use of these molecules.

Quality of hospital care evaluated by Danish nurses and doctors--based on experience from their own or a close family member's hospitalization.

INTRODUCTION: No studies have systematically asked larger groups of health professionals about their own experience as patients. This study estimated the level of satisfaction with hospital care among health professionals based on experience from their own hospital admission or that of a close family member. METHODOLOGY: A cross-sectional questionnaire study of 1995 doctors (41% women) and 1472 nurses (98% women) drawn randomly from union registers. Response rate: 70%. RESULTS: Twenty-seven percent had themselves been hospitalized within the preceding five years, and 31% had followed a hospi

Patient recall versus physician documentation in report of smoking cessation counselling performed in the inpatient setting.

OBJECTIVES: To determine rates of patient reported and physician documented counselling; to identify predictors of each report; and to identify the impact of each report on smoking cessation attempts after discharge from the hospital. DESIGN: Stickers on subjects' charts prompted physicians to give brief smoking cessation counselling to patients in the hospital. Patients reported counselling received and quit attempts in a phone interview conducted 7-18 days after discharge. Rates of counselling and correlations were calculated, and multivariate analysis identified predictors of patient repor

Factors related to prescription dosage in Japanese psychiatric hospitals.

A point prevalence study of prescription dosage was made among psychiatric inpatients in Japan with the following aims: (i) to analyze whether total chlorpromazine equivalent dose was related to patient characteristics or hospital and physician characteristics; and (ii) to analyze whether the usage of the two major categories of antipsychotics, butyrophenone and phenothiazine, were related to their expected pharmacological reactions. A comprehensive database of prescription contents and characteristics of patients, physicians and hospitals was developed from a survey of 1674 patients in 18 psy

The Perception of the Scope of Practice of Podiatric Medicine in a Large Teaching Hospital Demonstrated by Internal Medicine Residents and Patients.

BACKGROUND:: The scope of podiatric practice has changed significantly in the past couple of decades. Despite the increased quality of training, many people outside of podiatry may not realize what our scope of practice entails. METHODS:: We conducted a survey consisting of 10 items and asked internal medicine residents at Rush University Medical Center and patients whether they would feel comfortable consulting podiatrists, or being treated for each issue. RESULTS:: The results for residents are as follows: 1) toenail fungus, 35% yes and 65% no; 2) diabetic wound care, 87.5% yes and 12.5% n

Acquired immune deficiency syndrome: psychological impact on health personnel.

Eight nurses and four physicians involved in the care of an acquired immune deficiency syndrome (AIDS) patient and a matched, non-AIDS patient responded to three self-report measures that assessed the psychological distress associated with working with each patient. While working with the AIDS patient the physicians and nurses experienced increased anxiety, greater interference in nonwork activities, more frequent negative ruminations, and more negative perceptions regarding the AIDS patient's behavior than they did with the non-AIDS patient. Because of the fears and concerns engendered in med

Trends in naloxone prescribed at emergency department discharge: A national analysis (2012-2019).

BACKGROUND: While having access to naloxone is recommended for patients at risk for opioid overdose, little is known about trends in national naloxone prescribing rates in emergency departments (EDs) both for co-prescription with opioids and for patients who presented with opioid abuse or overdose. This study aims to evaluate the change in naloxone prescribing and opioid/naloxone co-prescribing at discharge using national data. METHODS: We conducted an IRB exempt retrospective review of data collected by the National Hospital Ambulatory Medical Care Survey from 2012 to 2019. The primary outco

The role of the psychiatrist in the general hospital--II. Psychiatric patients with somatic complaints.

In part I, one of the important roles of the psychiatrist in the general hospital was pointed out with respect to the management and treatment of psychiatric patients with somatic complaints (PPSC) referred from other nonpsychiatric departments of the hospital. Part II deals with the way in which a PPSC is brought to the psychiatrist, and the basic principles concerning the initial interview which is crucial to the management and treatment of such patients. The principles for the initial interview are to clarify: (1) the processes through which the patient came to the psychiatrist, (2) the pat
