Lead Times of Optometry Services in Selected Public Hospitals in Rural South Africa.

PURPOSE: Efficient service delivery remains a critical benchmark of public sector performance, with lead and waiting times serving as essential indicators of responsiveness and accessibility. Long waiting times not only diminish user satisfaction but can also exacerbate inequalities in access, particularly in resource-constrained contexts. This study investigated the lead times of optometry services in selected hospitals in rural South Africa. By evaluating lead and waiting times and identifying contributing factors, the research further aimed to inform strategies for improving service deliver

Association of Rural Residence With Surgical Outcomes Among Infants at US Children's Hospitals.

OBJECTIVES: Disparities in pediatric health outcomes are widespread. It is unclear whether rurality negatively impacts outcomes of infants with surgical congenital diseases. This study compared outcomes of rural versus urban infants requiring complex surgical care at children's hospitals in the United States. METHODS: Rural and urban infants (aged <1 year) receiving surgical care at children's hospitals from 2016 to 2019 for esophageal atresia, gastroschisis, Hirschsprung's disease, anorectal malformation, and congenital diaphragmatic hernia were compared over a 1-year postoperative period us

The evacuation of two rural hospitals following a tsunami warning: What happened next.

This paper describes the lessons gained and actions taken following the evacuation of two small rural hospitals following a tsunami warning. These evacuations were unique in that the normal protocol to transfer within the health system was not feasible due to their geographical isolation. The after action review process provided valuable insight to all stakeholders, who entered into discussions to ensure procedures, supplies, equipment and arrangements would be put in place to support the safe and effective evacuation of patients in the event of a future emergency. A key component of this proc

Low-cost single foot operated mechanical suction machine for rural health centers and hospitals.

INTRODUCTION: Healthcare practitioners in low and middle-income countries encounter numerous challenges, including insufficient staffing, an unreliable electrical infrastructure, and constrained resources. Unstable availability of electricity constitutes a significant impediment to the efficacy of public health initiatives that depend on technology requiring electrical power. More than 95% of medical apparatus is procured from developed nations, with an only 13% of medical device manufacturers situated within LMIC. Non-profit organizations have developed innovative medical technologies that of

Obstetrical epidural anaesthesia in a rural Canadian hospital.

Few rural hospitals offer obstetric epidural analgesia services and of those that do, there is a paucity of information about these anaesthetics. A retrospective review was conducted of all obstetrical epidurals from 1984-1988 in an 85-bed hospital in Saskatchewan to examine the indications, complications, and infant outcomes. During that period there were 1224 deliveries. From a total of 915 vaginal deliveries, 42 (4.6%) received an epidural. Caesarean sections numbered 309: 183 (59.3%) were with epidural analgesia of which 69 were urgent and 114 elective. The overall complication rate was 23

Gastroscopy in a West African rural mission hospital.

The results of 1075 fibreoptic gastroscopies performed in the Northwest Province of Cameroon are presented. Three hundred and fifty-three examinations showed pyloroduodenal ulcer disease, 111 showed macroscopic gastritis, and 37 had gastric carcinoma. Sixteen other diagnosis were made, with a positive finding in 620 cases. The benefits of the 'high-technology' gastroscope in a low technology setting are discussed. Included are examinations of a series of 46 patients with haematemesis and/or melaena, and 43 who had previous gastric surgery.

Inpatient cardiology in a rural district general hospital.

The pattern of cardiac admissions to a rural district general hospital has been analysed. Coronary artery disease is the major disorder, both as an acute and chronic problem. Other forms of heart disease are relatively uncommon. The absence of hypertension and hypertensive heart disease is striking. The deaths were mainly related to coronary artery disease.

Vacuum extraction: use in a small rural hospital.

The effectiveness of vacuum extraction with the Silastic Obstetrical Vacuum Cup (SOVC), which has a soft, maleable cup, was assessed by two family physicians in a small rural hospital. Vacuum extraction was attempted in 35 of 231 deliveries over an 18-month period, with an overall success rate of 66%. The main indications for vacuum extraction were fetal distress, followed by a prolonged second stage of labour and malrotation of the occiput. The efficiency of the technique improved with experience. The effects of vacuum extraction on the fetus and mother compared favourably with those reported

Urban hospital and rural village smallpox in Bangladesh.

Smallpox mortality at Dacca, Bangladesh Infectious Diseases Hospital during 1972 and 1973 was 46 per cent. To determine if this was the actual rate within the population, data were compared to those collected from village populations in Noakhali District. Age/sex adjusted smallpox mortality for the rural population was 23 per 100 cases as compared to 52 per 100 for the hospital population. Analysis of the difference identified a selection bias of the hospital for severe disease.

Index cholecystectomy in a rural hospital: it can be done.

BACKGROUND: Index cholecystectomy (IC) refers to an operation during a patient's first hospital admission with symptomatic gallstone (GS) disease. There are proven reductions in cost, hospital bed days and GS-related complications while awaiting elective surgery. IC has not been universally adopted, particularly in smaller centres where logistics can present a barrier. The aim of this paper is to describe the introduction of routine IC at Hastings Hospital and the effects in terms of waiting time until surgery; GS-related re-presentations and complications while awaiting surgery; operative com
