Centrally active nonhormonal hot flash therapies.

Given the problems associated with hormonal therapy, and the prominent problem of hot flashes in menopausal women, there is a need for nonhormonal agents to alleviate hot flashes. Several compounds that appear to act on the central nervous system have been investigated. Potential mechanisms for their effects on hot flashes have been described. Bellergal (no longer available on the US market, where it was known as Bellergal-S), a combination preparation sedative that consists of low-dose phenobarbital, ergotamine tartrate, and levorotatory alkaloids of belladonna, is an old agent that was popul

Cardiac autonomic function and hot flashes among perimenopausal and postmenopausal women.

OBJECTIVE: Abnormalities in autonomic function are posited to play a pathophysiologic role in menopausal hot flashes. We examined relationships between resting cardiac autonomic activity and hot flashes in perimenopausal and postmenopausal women. METHODS: Autonomic function was assessed at baseline and 12 weeks among perimenopausal and postmenopausal women (n = 121, mean age 53 years) in a randomized trial of slow-paced respiration for hot flashes. Pre-ejection period (PEP), a marker of sympathetic activation, was measured with impedance cardiography. Respiratory sinus arrhythmia (RSA), a mar

Hot flashes and cardiac vagal control: a link to cardiovascular risk?

OBJECTIVE: The understanding of the physiology of hot flashes is incomplete. The autonomic nervous system has been hypothesized to play a role in hot flashes but has received limited empirical attention. Furthermore, emerging research has linked hot flashes to cardiovascular risk. Reduced high-frequency heart rate variability (HF-HRV), an index of vagal control of heart rate, has been associated with cardiovascular events. We hypothesized that decreases in HF-HRV would occur during hot flashes relative to periods before and after hot flashes. METHODS: Thirty perimenopausal and postmenopausal

Levetiracetam for the treatment of hot flashes: a phase II study.

GOALS OF WORK: The objectives of this pilot trial were to assess the potential efficacy and safety of levetiracetam for the treatment of hot flashes, a major cause of morbidity among breast cancer survivors. PATIENTS AND METHODS: Women, aged 18 years or more, with a history of breast cancer or those who wished to avoid estrogen because of a perceived increased risk of breast cancer, who were experiencing bothersome hot flashes (more than or equal to 14 times per week, for more than or equal to 1 month before study entry), were included. During the baseline week, general demographic characteri

Nonhormonal therapies for menopausal hot flashes: systematic review and meta-analysis.

CONTEXT: Concern regarding the adverse effects of estrogen and other hormones for treating menopausal symptoms has led to demand for other options; however, the efficacy and adverse effects of nonhormonal therapies are unclear. OBJECTIVE: To assess the efficacy and adverse effects of nonhormonal therapies for menopausal hot flashes by reviewing published randomized controlled trials. DATA SOURCES: MEDLINE (1966-October 2005), PsycINFO (1974-October 2005), and the Cochrane Controlled Clinical Trials Register Database (1966-October 2005) were searched for relevant trials that provided data on

Hot flashes in patients with hypogonadism and low serum gonadotropin levels.

OBJECTIVE: To augment the number of cases in the literature describing patients with hot flashes due to hypopituitarism or hypothalamic hypogonadism. METHODS: We reviewed medical records for hot flashes in our patients with low sex hormone levels and low or inappropriately normal gonadotropin levels, and we inquired about hot flashes in new patients with these hormone patterns. A comprehensive literature review on this subject was also completed. RESULTS: In our medical records, we found 14 patients with hot flashes, low sex hormone levels, and low or inappropriately normal gonadotropin leve

The Hot Flash Related Daily Interference Scale: a tool for assessing the impact of hot flashes on quality of life following breast cancer.

To assess the psychometric properties of the Hot Flash Related Daily Interference Scale (HFRDIS), a sample of breast cancer survivors and an age-matched comparison group completed a questionnaire packet and 2-day prospective hot flash diary at an initial time point and again 6 months later. There were 71 breast cancer survivors and 63 comparators at Time 1, and 54 survivors and 46 comparators at Time 2. The HFRDIS was internally consistent, with alphas of 0.96 at times 1 and 2. Validity was supported through 1) correlations with other hot flash variables, 2) correlations with measures of affec

Clinical Hypnosis and Cognitive Behavioral Therapy for Hot Flashes: A Scoping Review.

BACKGROUND: Hot flashes can be a prevalent issue for postmenopausal women, but traditional treatments such as hormone therapy can have adverse side effects. Recommended psychotherapies for managing hot flashes include cognitive behavioral therapy (CBT) and clinical hypnosis, but an in-depth review comparing the efficacy of both treatments is warranted. OBJECTIVES: The aim of the present scoping review was to assess the clinical significance and efficacy of symptom reduction of CBT and clinical hypnosis as treatments of hot flashes. ELIGIBILITY CRITERIA: Peer-reviewed primary studies were inc

Hot flashes and sleep disruption in a randomized trial in menopausal women.

BACKGROUND: Hot flashes, common during menopause, affect up to 80% of the Western menopausal women and are reported to contribute to sleep disturbances in midlife. Few prospective data are available to confirm the specific role of hot flashes in disrupting sleep in midlife women, however, or confirm whether changes in hot flashes in response to clinical therapies result in improvement in sleep. OBJECTIVE: To examine the effects of continuous nitroglycerin therapy on sleep quality in perimenopausal and postmenopausal women with frequent hot flashes (pre-specified secondary trial endpoint) and

Using traditional acupuncture for breast cancer-related hot flashes and night sweats.

OBJECTIVES: Women taking tamoxifen experience hot flashes and night sweats (HF&NS); acupuncture may offer a nonpharmaceutical method of management. This study explored whether traditional acupuncture (TA) could reduce HF&NS frequency, improve physical and emotional well-being, and improve perceptions of HF&NS. DESIGN/SETTINGS/LOCATION: This was a single-arm observational study using before and after measurements, located in a National Health Service cancer treatment center in southern England. SUBJECTS: Fifty (50) participants with early breast cancer completed eight TA treatments. Eligible w

The association between nocturnal hot flashes and sleep in breast cancer survivors.

This study examined the relationship between objectively measured nocturnal hot flashes and objectively measured sleep in breast cancer survivors with insomnia. Twenty-four women who had completed treatment for non-metastatic breast cancer participated. All were enrolled in a study of cognitive-behavioral treatment for chronic insomnia. Nocturnal hot flashes and sleep were measured by skin conductance and polysomnography, respectively. The 10-minute periods around hot flashes were found to have significantly more wake time, and more stage changes to lighter sleep, than other 10-minute periods

Can hypnosis reduce hot flashes in breast cancer survivors? A literature review.

Hot flashes are a significant problem for many breast cancer survivors and can cause discomfort, insomnia, anxiety, and decreased quality of life. In the past, the standard treatment for hot flashes has been hormone replacement therapy. However, recent research has found an increased risk of breast cancer in women receiving hormone replacement therapy. As a result, many menopausal women and breast cancer survivors reject hormone replacement therapy and many women want non-pharmacological treatment. In this critical review we assess the potential use of hypnosis in reducing the frequency and in

The influence of breast cancer treatment on the occurrence of hot flashes.

Approximately 65% of women report hot flashes (HFs) during or following breast cancer treatment. However, few studies have compared the occurrence of vasomotor symptoms according to the type of adjuvant treatment received, and little is known about other risk factors for HFs among this population. This longitudinal study aimed to: 1) compare the frequency and severity of HFs in breast cancer patients according to treatment regimen; 2) evaluate the potential contribution of hormone therapy on the frequency and severity of HFs; and 3) identify other potential risk factors for HFs in breast cance

Acupuncture for treating hot flashes in breast cancer patients: a systematic review.

The objective of this review was to assess the effectiveness of acupuncture as a treatment option for hot flashes in patients with breast cancer. We searched the literature using 14 databases from their inceptions to August 2008, without language restrictions. We included randomised clinical trials (RCTs) comparing real with sham acupuncture or another active treatment or no treatment. Their methodological quality was assessed using the modified Jadad score. Three RCTs compared the effects of manual acupuncture with sham acupuncture. One RCT showed favourable effects of acupuncture in reducing

Newer antidepressants and other nonhormonal agents for the treatment of hot flashes.

The reluctance to use estrogen in breast cancer survivors with hot flashes has extended to its use in healthy women since the 2002 publication of the Women's Health Initiative study. This article reviews the clinical development of nonhormonal agents as alternatives to hormonal therapy for the management of hot flashes.

A Review of Hot Flash Management in Patients With Prostate Cancer.

CONTEXT: Vasomotor symptoms (VMS) including hot flashes are a common toxicity reported by men undergoing androgen deprivation therapy (ADT) and their effective mitigation remains crucial in prostate cancer survivorship. As the understanding of hot flash pathophysiology is refined, novel therapies offer promising benefits to this patient cohort. EVIDENCE ACQUISITION: The following mini-review outlines the landscape of VMS management in patients with prostate cancer as informed by a literature search using the PubMed and Embase databases. EVIDENCE SYNTHESIS: Hormonal therapies appear most effe

The Relationship Between Daily Stress and Hot Flashes in Women of Menopausal Age.

OBJECTIVE: The menopause transition is accompanied by various psychological and physical symptoms including hot flashes. There is emerging evidence on the link between stress and hot flashes, yet findings have been limited and mixed. Most studies have relied primarily on between-person relationships and have overlooked important nuances of whether stress on any given day is associated with more severe hot flashes on that same day. Our study bridges this gap by examining both the between- and within-person relationships between daily psychological stress (number of stressors, stressor severity

Hot flashes: a potential marker of deterioration of health-related quality of life.

OBJECTIVE: Hot flashes are among the most common symptoms of the menopausal transition and have traditionally been considered benign and self-limiting. However, increasing evidence suggests that they may indicate broader neurovascular and inflammatory dysregulation linked to reproductive aging. The possible effect of hot flush severity on health-related quality of life (HRQoL) remains inadequately studied, particularly in Latin American populations. This study aimed to examine the association between hot flash severity and HRQoL in middle-aged women using validated tools and a large, multicent
