BACKGROUND: Human T-Lymphotropic Virus Type-1 (HTLV-1) is an oncogenic retrovirus that causes adult T-cell leukemia/lymphoma (ATLL). The virally encoded Tax protein is thought to be necessary and sufficient for T-cell leukemogenesis. Tax promotes inappropriate cellular proliferation, represses multiple DNA repair mechanisms, deregulates cell cycle checkpoints, and induces genomic instability. We reported the findings of micturitional histories and urodynamic studies in five patients with human T-lymphotropic virus type-1-associated myelopathy. Histories showed that all patients had obstructive as well as irritative micturitional symptoms, and four of their micturitional symptoms appeared from the onset of the disease. A 67-year-old man who had necrobiotic xanthogranuloma associated with paraproteinemia is described. He was a human T-lymphotropic virus type 1 (HTLV-1) carrier who had a high titer of circulating anti-HTLV-1 antibodies and neurologic abnormalities that suggested HTLV-1-associated myelopathy. The patient's necrobiotic xanthogranuloma and neurologic symptoms improved after he received four 5-day courses of melphalan, 2 mg/day, and prednisolone, 20 mg/day, at 4-week intervals.

INTRODUCTION: Neurological involvement of Human T-lymphotropic virus type 1 (HTLV-1) mainly results in myelopathy (tropical spastic paraparesis). However, cranial nerve impairment, including facial nerve damage, is rare in patients with HTLV-1 infection.
OBSERVATION: We report the case of a patient, originally from Caribbean islands, who developed recurrent bilateral facial palsy (six recurrences during the 7-year follow-up). Both blood and cerebrospinal fluid serologies were positive for HTLV-1. The diagnosis of recurring bilateral facial palsy revealing HTLV-1 infection was made.

We present a case of human T-cell lymphotropic virus, type 1 (HTLV-1)-associated myelopathy, also known as tropical spastic paresis. While the literature has previously described the neuroradiological findings of spinal atrophy and hyperintense T2 lesions involving the cerebral white matter, little information is available about musculoskeletal imaging findings. Our patient demonstrated diffuse muscle atrophy of the thigh muscles, particularly the posteromedial groups. Our case supports HTLV-1-associated myelopathy as a disease of neurological etiology with resulting muscular denervation.

Thalassemia is an inherited genetic disorder requiring multiple transfusions to treat anemia caused by low hemoglobin levels.
