Hemiarthroplasty for comminuted proximal humeral fractures.

PURPOSE: To evaluate early functional outcomes of hemiarthroplasty for 3- or 4-part proximal humeral fractures. METHODS: 16 men and 11 women aged 27 to 83 (mean, 56) years underwent hemiarthroplasty for comminuted 3-part (n=13) or 4-part (n=14) proximal humeral fractures. All the patients reported normal shoulder function prior to injury. The range of shoulder motion and muscle power were evaluated, as were subjective pain and satisfaction (using the UCLA scoring system). RESULTS: At the final follow-up, the mean maximum abduction was 111º (SD, 47º; range, 30º-180º), and the mean maximum for

Periprosthetic humeral fractures after shoulder arthroplasty.

BACKGROUND: Currently, there is little information concerning periprosthetic humeral fractures after shoulder arthroplasty. Therefore, we reviewed our experience with these fractures to determine the results of treatment, the risk factors for periprosthetic fracture, and the rates of reoperation. METHODS: Between 1976 and 2001, nineteen postoperative periprosthetic humeral fractures occurred among 3091 patients who had undergone shoulder arthroplasty at our institution. Sixteen patients had a complete series of radiographs and were included in this study. The average time from the arthroplast

Nerve lesions in proximal humeral fractures.

In the literature nerve injury is not frequently considered a problem in proximal humeral fractures. Only a few studies exist concerning traction injury of nerves in fractures of the proximal humerus after low-velocity trauma. Almost all of them are retrospective and did not use electromyography. Patients with identical fractures can show quite different outcomes, which vary between complete recovery and severely limited shoulder function. On the assumption that nerve lesions can play a role in the recovery of conservatively and operatively treated proximal humeral fractures, we started a pros

Internal fixation of proximal humeral fractures.

Fractures of the proximal humerus are common, especially in elderly individuals, and are usually classified according to Neer into fractures having one, two, three, or four parts with or without an associated dislocation or an injury splitting the humeral head. This article is intended to assist the surgeon in selecting and using techniques for internal fixation of these fractures. The use of open reduction and internal fixation using plates and screws has become less popular because of the extensive dissection required and the tendency for the plate to impinge. Use of multiple percutaneous pi

[Ender nailing of humeral shaft fractures].

From March 1991 to June 1992, thirteen patients with humeral shaft fracture were managed at our hospital with closed reduction and internal fixation with Ender nails. These patients were mainly victims of traffic accidents and falls and had fractures, head injuries, and chest trauma. The follow-up period was more than 1 year. Operations were performed under C-arm guide and, according to the method of Hall and Pankovich, divided into antegrade and retrograde methods. The average operation time was 104 minutes, ranging from 70 to 160 minutes, and blood loss was less than 100 ml. Twelve patients

Distal humeral fractures in the adult.

Distal humeral fractures in the adult often are complicated by pseudarthrosis, pain, and limitation of motion. Of 29 adult patients with distal humeral fractures treated in teaching hospitals, 22 obtained acceptable results. Twenty-two percent of those treated by open methods and 36% of those treated by closed methods had unacceptable results. Ninety percent of the patients starting physiotherapy within six weeks of injury recovered well, while over half of those starting physiotherapy after six weeks had unsatisfactory results. Even comminuted fractures healed when treated by internal fixatio

Plate fixation of proximal humeral fractures.

We performed open reduction and internal fixation with a T-buttress plate in 32 fractures of the proximal humerus with severe displacement and/or fracture dislocation. In 27 cases acceptable reduction was achieved. In four cases infection developed, and the implant was removed in five cases because of impingement of the plate under the acromion and in two cases because of loosening. There was no nonunion, but four humeral head necroses. At the re examination of 20 patients after 2-7 years, nine were excellent or satisfactory and eleven unsatisfactory or poor. We conclude that the buttress plat

Lateral humeral condylar fractures in children.

Fifty-three patients with 56 fractures of the lateral humeral condyle were reviewed and the results were assessed to determine the amount of displacement that could be relied on to produce satisfactory results. Closed treatment resulted in satisfactory results if the initial displacement did not exceed 2 mm. This required close follow-up for detection of displacement. Percutaneous pinning of nondisplaced and minimally displaced fractures is an acceptable alternative in any situation in which close scrutiny cannot be ensured. Those fractures with greater than 2 mm of displacement should be redu

Fractures of the proximal humeral epiphysis.

Displacements of the proximal humeral epiphysis at the epiphyseal plate result from extension injuries. The configuration of the epiphyseal plate and the thickness of the periosteum surrounding the epiphysis make slight to moderate displacements relatively stable injuries, which can be treated successfully with external support. In patients with more severe displacement, flexion, abduction, and slight external rotation of the distal fragment bring the metaphysis into alignment with the proximal fragment, correcting the anterior angulation. The thick periosteal sleeve attached to the physis thr

Skeletal management of humeral shaft fractures associated with forearm fractures.

Nine cases of ipsilateral humeral shaft and forearm fractures in multiply injured patients were reviewed in an attempt to ascertain the preferred management of these complex injuries. Satisfactory anatomic and functional results were achieved only in those cases treated by rigid internal fixation of the humeral fractures. Nonoperative management is associated with a significant risk of malunion or nonunion of the humerus.

The Radiographic Union Score for HUmeral fractures (RUSHU) predicts humeral shaft nonunion.

AIMS: The primary aim of this study was to develop a reliable, effective radiological score to assess the healing of humeral shaft fractures, the Radiographic Union Score for HUmeral fractures (RUSHU). The secondary aim was to assess whether the six-week RUSHU was predictive of nonunion at six months after the injury. PATIENTS AND METHODS: Initially, 20 patients with radiographs six weeks following a humeral shaft fracture were selected at random from a trauma database and scored by three observers, based on the Radiographic Union Scale for Tibial fractures system. After refinement of the RUS

Distal Humeral Hemiarthroplasty Versus Total Elbow Arthroplasty for Acute Distal Humeral Fractures.

For acute distal humeral fractures not amenable to open reduction and internal fixation, total elbow arthroplasty has become an established alternative. However, lifelong activity restrictions designed to prevent early mechanical failure make this a poor option for some patients. This has led to a renewed interest in distal humeral hemiarthroplasty. Using modern implants and techniques, distal humeral hemiarthroplasty has shown outcomes comparable to those of total elbow arthroplasty at short- to mid-term follow-up, with an overall higher but different complication rate. Long-term data are nee

[Humeral head fracture and cuff].

During the past decade the evaluation of the rotator cuff in the management of proximal humeral fractures has received increasing attention. Different studies have investigated the pathomorphology, prevalence and impact of rotator cuff lesions on the outcome of non-operative or surgical treatment of proximal humeral fractures. Tendon defects, either chronic or trauma related, are observed mainly in the anterosuperior or posterosuperior aspect of the rotator cuff and present as partial- or full-thickness tears. Structural changes of the rotator cuff muscles including atrophy and fatty infiltrat

[Humeral shaft fracture in childhood].

In a collective study of twelve surgical clinics 222 diaphyseal fractures of the humerus in children were examined. 159 children were checked up two to 14 years after the accident. Priority was given to conservative treatment: Désault or Gilchrist dressing (24%), arm cast (34%), extension (29%), change from primary extension to secondary dressing or cast (27%), osteosynthesis (10%). Late results were excellent in 85%, whereas in 15% minor anatomical lesions persisted, such as axial deviations, but without functional impairment. There were eight transitory primary nerve lesions (3.6%), seven co

[Arthroscopic fracture management in proximal humeral fractures].

Arthroscopy has become increasingly more established in the treatment of proximal humeral fractures. In addition to the known advantages of minimally invasive surgery fracture and implant positioning can be optimized and controlled arthroscopically and relevant intra-articular concomitant pathologies (e.g. biceps tendon complex and rotator cuff) can be diagnosed and treated. Arthroscopic techniques have proven to be advantageous in the treatment of various entities of greater tuberosity fractures, lesser tuberosity fractures (suture bridging technique) and subcapital humeral fractures (arthros
