Intramedullary nailing of humeral diaphyseal fractures. Is distal locking really necessary?

PURPOSE: Distal interlocking is regarded as an inherent part of the antegrade humeral nailing technique, but it exposes both the patient and surgeon to radiation, is time consuming, and has a potential risk of damaging neurovascular structures. We have presented our technique of diaphyseal humeral nailing without any distal interlocking in this paper. MATERIALS AND METHODS: We have presented a series of 64 consecutive patients (33 male and 31 female, mean age: 41.5 years) with humeral shaft fractures treated with antegrade rigid intramedullary nailing without distal interlocking following a s

Biomechanical evaluation of a modified proximal humeral locking plate application for distal extra-articular diaphyseal humeral fractures.

The extra-articular distal humerus locking plate (EADHP) is widely used for distal extra-articular diaphyseal humeral fracture fixation. However, it occasionally causes skin prominence and discomfort. The upside-down use of a proximal humerus internal locking system (PHILOS) plate is suggested as an alternative option, but it lacks biomechanical evidence. The purpose of this study was to compare the biomechanical performance between two different fixation methods: the modified use of the PHILOS plate on the anterior cortex versus conventional use of an EADHP on the posterior cortex. Twelve pai

Lateral Para-Olecranon Approach for the Treatment of Distal Humeral Fracture.

PURPOSE: To assess the outcomes of the lateral para-olecranon triceps-splitting approach for the treatment of distal humeral fracture. METHODS: Ten patients (3 males, 7 females) with a mean age of 59 years were retrospectively reviewed. There were 2 A2, 3 C1, and 5 C2 fractures according to the AO/ASIF classification. Types B3 and C3 fractures were excluded from this study because the olecranon osteotomy approach was indicated to visualize the anterior fragment. The triceps was split at the midline, and the anconeus muscle was incised from the proximal ulna. The lateral half of the triceps al

[Approaches to distal humeral fractures in adults and comparison of treatment results].

OBJECTIVES: We compared the results of surgical techniques used in the treatment of distal humeral fractures. METHODS: The study included 22 patients (15 men, 7 women; mean age 52 years; range 21 to 80 years) who were treated for closed distal humeral fractures. The fractures were classified according to the Müller's classification (18 type 3, 4 type 4) and Mehne-Matta's classification of intraarticular fractures. Following open reduction, internal fixation was performed with the use of double reconstructive plates in seven patients (32%), multiple K-wires in eight patients (36%), and multipl

Radiographic diagnosis of fracture-separation of the entire distal humeral epiphysis.

Twenty children with fracture-separation of the entire distal humeral epiphysis have been reported. The key to the radiographic diagnosis is the relationship between the radius and capitellum. A line extending from the shaft of the radius will always pass through the capitellum regardless of the position in which the radiograph is obtained. Posteromedial displacement of the humeral fracture fragment together with the ulna and the radius most commonly occurs. The injury must be distinguished from dislocation of the elbow, fracture of the lateral condyle, and supracondylar fracture of the distal

A simple technique for double plating of extraarticular distal humeral shaft fractures.

Plate fixation of extraarticular distal humeral shaft fracture is often difficult. Traditional techniques do not allow for stable fixation. A single DCP plate may have inadequate purchase in the distal fragment. The use of large plates extending distally over the lateral supracondylar ridge is often associated with pain and sometimes interferes with elbow range of motion. In this study, 22 patients with extra articular distal humeral fractures were managed with dual plating using a paratricipital approach. The first plate--a narrow DCP--was fixed on the dorsal surface of the humerus. The secon

Postoperative management after open reduction and internal fixation of distal humeral fractures.

Postoperative care is a critically important component in treating patients with complex distal humeral fractures. The immediate, intermediate, and late phases of the postoperative period present different issues that require specific management to maximize the eventual functional outcome for patients with these injuries.

Comminuted intraarticular fractures of the distal humeral condyles. Surgical vs. nonsurgical treatment.

Forty-two comminuted intraarticular fractures of the humeral condyles treated between 1975 and 1981 were analyzed in order to compare the results of operative versus nonoperative management. Twenty-nine patients were treated by open reduction and internal fixation; the remainder were treated by nonoperative techniques. Functional results were evaluated by the method of Bickel and Perry. Of the patients treated by open reduction and internal fixation, 76% had an excellent or good result. Of the nonsurgically-treated patients, only eight percent had a satisfactory result. Minimally invasive anatomical fixation of distal humeral fractures: Direct reduction through the anterior MIPO distal window.

Conventional surgical treatment for distal humeral shaft fractures consists of osteosynthesis through an open posterior approach. This technique requires a wide incision, extensive muscular dissection, and direct handling of the radial nerve, resulting in considerable surgical morbidity. Although the anterior MIPO technique represents a less invasive alternative and avoids these drawbacks, it has traditionally been associated with indirect reduction and relative stability alone. In distal humeral shaft fractures, the distal window of the anterior MIPO approach is located directly above the fra

Distal humeral fractures treated with ORIF or hemiarthroplasty: A matched-pair analyses.

INTRODUCTION: Fractures of the distal humerus are common in older patients with osteoporotic bone, often presenting as complex, multi-fragmentary injuries involving the articular surface. This complexity complicates the decision between open reduction and internal fixation (ORIF) and total elbow arthroplasty (TEA), as both procedures carry specific risks. Hemiarthroplasty (HA) may be a viable alternative, yet few studies have compared its outcomes with those of ORIF. In this retrospective matched-pair study, we aimed to compare primary HA versus ORIF for complex distal humerus fractures. Our h

Radial nerve palsy in mid/distal humeral fractures: is early exploration effective?

BACKGROUND: Radial nerve palsies are a common complication with displaced distal humeral fractures. This case series examines the outcomes of early operative exploration and decompression of the nerve with fracture fixation with the view that this provides a solid construct for optimisation of nerve recovery. METHODS: A total of 10 consecutive patients with a displaced distal humeral fracture and an acute radial nerve palsy were treated by the senior author by open reduction and internal fixation of the distal humerus and exploration and decompression of the radial nerve. Motor function and s

[Osteosynthesis in distal intra-articular humeral fractures. External fixation with motion capacity].

The rising incidence of distal intra-articular humeral fracture in elderly patients associated with osteoporosis frequently leads to difficulties in acute reconstruction and may result in early failure of internal fixation. The biomechanics of the elbow joint and especially the flexor group as well as the brachioradialis muscle as part of the extensor group is analyzed. The complex motion pattern of the transarticular muscle groups increases the load on internal fixation and may affect the interface between implant and osteoporotic bone. An external fixator with motion capacity serves to prote

Elbow arthroplasty is safe for the management of simple open distal humeral fractures.

BACKGROUND: Elbow arthroplasty (EA) is an established technique for the treatment of select distal humeral fractures, yet little data exists regarding the safety and outcome of EA in the presence of an open distal humeral fracture where the risk of periprosthetic infection is an even greater concern. We hypothesized that EA does not carry an increased risk of infection or other postoperative complications when performed for simple open distal humeral fractures. METHODS: Seventeen patients underwent total EA (n = 9) or hemiarthroplasty (n = 8) for an open distal humeral fracture. The open frac
