[Physical examination of the knee after trauma].

The practice guideline 'Traumatic knee complaints' from the Dutch College of General Practitioners is aimed at differentiating between intra-articular and extra-articular lesions. The diagnosis is based mainly on a combination of patient history and a limited physical examination of the knee. Specific tests for hydrarthrosis, injuries to the collateral or cruciate ligaments, and meniscal pathology have only a low diagnostic accuracy. Few reliable studies have been conducted into the diagnostic value of specific tests; most studies employed poor methodology, had considerable inter-observer vari

Total knee arthroplasty in a spinal cord-injured patient: a case report.

Patients with spinal cord injuries are prone to knee hydrarthrosis (also known as "water on the knee"), which can cause pain, functional impairment and the restriction of social activities. Total knee arthroplasty is a potentially appropriate treatment. Here, we report on a patient presenting partial T12 AIS grade C paraplegia who was able to walk with two forearm crutches, an ankle-foot orthosis on the right leg and a knee-ankle-foot orthosis on the left leg. Thirteen years after the spinal cord injury, the patient presented with septic arthritis of the right knee, complicated by recurrent hy

Risk of nerve injury during elbow arthroscopy: ultrasonographic evaluation of preoperative patients.

BACKGROUND: To clarify the real risk of nerve injury during elbow arthroscopy, the distances of the radial and median nerves to the elbow joint were investigated using ultrasonography in patients who underwent surgery. METHODS: A total of 35 patients who underwent arthroscopic surgery of the elbow were investigated. The distances of the nerves to the capsule and bony landmarks were measured using ultrasonography. The radial nerve distances were measured at the capitellum, joint space, radial head, and radial neck levels. The median nerve distances were measured at the trochlear, joint space,

[Another case of articular hydrops in dermatitis herpetiformis Duhring].

In addition to the 1968 report on two patients suffering from Duhring's disease (DD) associated with hydrarthrosis by Zaun, we now present a third case of the same combination of signs. In all three patients, painful joint swelling regularly preceded the manifestation of cutaneous eruptions. Joint pains and swelling did not respond to usual antirheumatic treatment, but always remitted together with the skin eruptions under medication of sulphones.

The treatment of persistent knee effusions with intra-articular radioactive gold: preliminary report.

Eighteen chronic knee effusions unresponsive to the usual methods of therapy were treated by intra-articular injections of radioactive gold ((198)Au) and followed up for one year. Ten patients had classical rheumatoid arthritis; three intermittent hydrarthrosis (both knees were treated in one patient); two ankylosing spondylitis, and one synovitis of undetermined cause. In 12 knees the effusion completely disappeared, usually within three months. Temporary increased pain and swelling occurred during the first week in five cases. Radiation dosimetry is discussed in detail.

Palindromic rheumatism and other relapsing arthritis.

Patients with recurrent or relapsing arthritis are frequently seen in rheumatological practice. Besides crystal arthritis, the most frequent cause of recurrent arthritis, there are several diseases that may present clinically as intermittent mono- or polyarthritis. Palindromic rheumatism is the paradigm of this type of condition, but other diseases such as systemic autoinflammatory disorders (periodic fever syndromes), Whipple's disease, arthritis associated with hyperlipidemia, intermittent hydrarthrosis and other diseases should be taken into account in the differential diagnosis of patients

Significance of arthroscopy in children with knee joint injuries.

Clinical and arthroscopic findings after knee joint injury were evaluated in 35 children aged 4-15 years. Arthroscopy revealed a rupture of the anterior cruciate ligament in 9 patients; two lesions were only partial. Eight children suffered from patellar dislocation. Eleven patients showed minimal synovial and cartilaginous lesions as the only cause of hemarthrosis, hydrarthrosis and clinical symptoms. Three had chondral lesions due to a dysplastic patella. The clinical diagnosis was confirmed in only 12 of 35 cases. Arthroscopic therapy was performed in 15 and open surgical intervention in 16

Popliteal cysts, femoral erosion and degenerative changes in the knee joint.

Positive correlations obtained between the presence of anterior femoral erosion, enlargement of the gastrocnemius-semimembranous bursa and osteoarthritis in the knee suggests that these are all manifestations of a degenerative process developing with increasing age. Popliteal cysts are considered to act as a decompressing factor for raised intraarticular knee pressure. The association of popliteal cysts with femoral erosions suggests that both conditions may be attributed to similar mechanisms, namely, hydrarthrosis and raised joint pressure secondary to degenerative articular disease. The dou

Demonstration and characterization of a transient arthritis in rats following sensitization of synovial mast cells with antigen-specific IgE and parenteral challenge with specific antigen.

We have developed a model of IgE-dependent, mast cell-mediated arthritis in rats. One knee joint (test joint) of a Sprague-Dawley rat was injected with 1 micrograms of a monoclonal IgE specific for dinitrophenol, and the contralateral (control) joint was injected with the same amount of an irrelevant monoclonal IgE in phosphate buffered saline or with phosphate buffered saline alone. Within 5 minutes of intravenous injection of antigen, an acute, transient arthritis occurred in the test joints only, with swelling and extravasation of intravascular blue dye and 125I-labeled albumin, decreased n

[The diagnostic value of so-called Reiter cells in joint effusions].

In 1967, Pekin, Malinin and Zvaifler described macrophages with one or more phagocytized granulocytes. These authors declared these cell phagocytes as a characteristic cytologic sign of Reiter's syndrome. Because we could observe this phenomenon in other joint diseases, too, we studied systematically different inflammable and non-inflammable joint effusions for the diagnostic value of these cells. We counted 1,000 cells in each joint effusion and determined the numbers of phagocytizing and the numbers and sorts of phagocytized cells and heterophagic vacuoles with cell fractions, respectively.

[Elements of diagnosis in rheumatology. "Early" diagnosis in flight personnel].

The various stages in the diagnostic process are discussed with particular reference to rheumatology. Consideration is given to recent "early" and "preclinical" diagnosis concepts, and these are applied to the aircrew category. The main "rheumatological emergencies" are listed and described for the purposes of that "specialised" prevention constantly needed to a greater extent, not only in the complex, demanding activity of aircrew, but also in the wider "world of labour", as currently conceived.

Common causes of knee effusions in spinal cord injury: a random study.

Patients with spinal cord injury are predisposed to knee effusions owing to osteoporosis, heterotopic ossification, trauma, and benign hydrarthrosis. This retrospective review discusses 11 patients with spinal cord injury and knee effusions seen during two years. One objective is to correlate the initial diagnosis based on clinical findings with the final diagnosis based on synovial fluid analysis and radiographic studies. Another is to describe the variety and complexity of clinical situations that involve knee effusions in spinal cord injury. The initial diagnosis was different from the fina

[Chronic instability of the knee].

During the last few years, major advances have been achieved in our understanding and treatment of chronic instability of the knee joint. The central hub (ACL and PCL) is the most important stabilizing element, and its rupture usually results in slipping knee, pain and recurrent hydrarthrosis, which are particularly frequent and severe in athletes. However, in some circumstances the loss of the anterior cruciate ligament can be compensated for by other stabilizing elements or by an excellent musculature. In patients with slipping knee, clinical, radiological and even arthroscopic examinations

Treatment of persistent knee effusions with intra-articular radioactive gold.

Patients with a total of 112 chronic knee effusions unresponsive to the usual conservative methods of therapy were treated with intra-articular injections of radioactive gold and followed from 6 months to 5 years. Most patients in the study had rheumatoid arthritis but others had ankylosing spondylitis, psoriatic arthritis, intermittent hydrathrosis and undiagnosed synovitis. After 6 months 81% showed improvement. This figure diminished to approximately 70% at 1 and 2 years, 60% at 3 and 4 years and 50% at 5 years. The mose beneficial results were seen in patients with intermittent hydrarthros

Evaluation of the knee pathology by ultrasonography.

Fourteen patients undergoing total knee replacement for the arthritic conditions of the knee were evaluated by ultrasonography (USG) prior to surgery. USG findings (synovium, fluid, femoral cartilage, cyst and loose bodies) were compared with surgical findings. USG and surgical findings matched for synovial pathology in 57%, for presence fluid in 71% and for cartilage damage in 45%. Loose bodies and cyst present in one patient was not picked up by USG. Baker's cyst was detected in one patient.
