This case of Boerhaave's Syndrome had several unusual features including a delayed presentation and right-sided esophageal perforation. The patient's initial episode of hematemesis may have been caused by a small mucosal laceration in the area of the Barrett's lesion that later ruptured. On the other hand, if initially there was an esophageal rupture, it did not violate the parietal pleura or mediastinum. The overlying pleura remained intact until digested by gastric contents, thereby causing a right-sided hydropneumo thorax and a marked increase in symptoms, which promoted the patient to come

In this case report, we describe a rare case of spontaneous diaphragmatic hernia with perforation of the incarcerated ascending colon and subsequent formation of tension pneumothorax. A 73-year-old male with a past medical history of chronic right pleural effusion, restrictive ventilatory impairment and hypertension presented to us for evaluation of severe right chest pain of few days' duration and severe dyspnoea. The chest radiograph revealed the presence of a loop of bowel in the basal right hemithorax with associated air/fluid levels. A CT scan of the chest confirmed the hydropneumothorax

INTRODUCTION: Hydropneumothorax is an abnormal presence of air and fluid in the pleural space. Even though the knowledge of hydro-pneumothorax dates back to the days of ancient Greece, not many national or international literatures are documented.
AIM: To study clinical presentation, etiological diagnosis, and management of the patients of hydropneumothorax.
MATERIALS AND METHODS: Patients admitted in a tertiary care hospital with diagnosis of hydropneumothorax between 2012 and 2014 were prospectively studied. Detailed history and clinical examination were recorded. Blood, pleural fluid, spu

A previously healthy Caucasian male developed hydropneumothorax and a pleural peel filled with pleomorphic, septate hyphae. The only organism grown from cultures of the lung and pleural fluid was Coccidioides immitis, confirmed by exoantigen testing. Spherule-endospore forms were produced, however, following injection of the arthroconidial tissue isolate into BALB/c mice. The patient had a positive immunodiffusion complement-fixation test and developed a positive coccidioidin skin test during therapy. He recovered following thoracotomy and wedge resection of the ruptured coccidioidal cavity, a

Clostridia species are rare causes of pleuropulmonary infections. This report describes an immunocompromised patient who had a renal transplant, had multiple risk factors for anaerobic pleuropulmonary infection, and developed an acute empyema with hydropneumothorax that was associated with Clostridium sporogenes bacteremia. Therapy included antibiotics and surgical drainage of the empyema. Species identification of clostridia can usually be limited to whether the species is perfringens or nonperfringens because the majority of clinically significant clostridial infections are caused by Clostri

A case of hydropneumomediastinum and bilateral hydropneumothorax from a central venous catheter is described. The complication is related to the position of the catheter after placement through the left internal jugular vein. Proper placement of a central venous line into the superior vena cava is essential to avoid this serious complication.

Bochdalek hernia (BH) in an adult may manifest clinically with a myriad of abdominal or chest symptoms or a combination of them. Diagnosis of an adult BH is usually delayed in view of rarity of the lesion and its varied presentation. A 30-year-old adult gentleman presented to us with a left thoracostomy which was draining pus and ingested food particles. The tube thoracostomy had been performed in another hospital for an apparent left hydropneumothorax before he arrived in our hospital. Computed tomography of Chest and abdomen revealed a left diaphragmatic defect with herniation of stomach, sp

A 95-year-old female presented to the emergency department with chest pain and difficulty breathing. She had a left-sided tension hydropneumothorax secondary to a spontaneous rupture of a hiatus hernia close to the gastro-oesophageal junction. This has not previously been described in the literature and therefore, the purpose of this report is to pique clinicians' interest and highlight this rare phenomenon as a possible differential diagnosis in patients with a hiatus hernia who present with chest pain. It also demonstrates the importance of the use of oral contrast when using computerised to

We reported and reviewed 8 cases including two authors' cases in the Japanese literature, the incidence of pneumothorax associated with primary pulmonary neoplasms was less than 0.05%, however, in the case of malignant pleural mesothelioma, was as high as 10.36%. Mean age of patients was 67 year-old and recurrent hydropneumothorax was characterized in the clinical course. For the elderly with hydropneumothorax, it was necessary to rule out the malignant mesothelioma based on the past history of asbestos exposure, analysis of value of hyaluronic acid in the pleural effusion and the CT scan find

Lymphangioleiomyomatosis (LAM) is a rare, idiopathic, cystic disease that affects lungs of young women at childbearing age. Usually, LAM clinical manifestations are pneumothorax, progressive dyspnea and chylous pleural effusions. In many cases, due to unusual and nonspecific symptoms, LAM is mis-recognized and patients, who are affected by such disease, receive delayed diagnosis. This case report focuses on a 45-year-old woman patient with asymptomatic lymphangioleiomyomatosis who presented a big cyst which makes it looks like hydropneumothorax condition. Although chest radiograph and followin

BACKGROUND Invasive pulmonary aspergillosis (IPA) is a severe form of the fungal infection with relatively high mortality rates. Risk factors that lead to IPA include immunosuppression through corticosteroid use. IPA complicated by hydropneumothorax is rare and its mechanism of formation is unknown. CASE REPORT A 72-year-old woman recently diagnosed with a right frontal meningioma that was managed with dexamethasone presented with a new 3-day history of nonproductive cough, chest pain, and dyspnea and was managed for pneumonia. The patient failed to improve, prompting a follow-up computed tomo

Our hospital is located in the Tono region in the southeastern district of Gifu Prefecture in which there are forests and inhabitants who still hunt and eat game meat. Therefore, boar meat increases the risk of contracting paragonimiasis. We treated two patients who were infected by Paragonimus westermani after eating boar meat. They developed hydropneumothorax in association with eosinophilic pleural effusion. For patients who have pneumothorax with concomitant pleural effusion and eosinophilia in the pleural fluid analysis, it is necessary to take a detailed history, which includes flesh foo

Hydropneumothorax represents an unusual presentation of pulmonary tuberculosis (TB), posing diagnostic and therapeutic challenges. We report the case of a 36-year-old woman who presented with productive cough, dyspnea, right-sided pleuritic chest pain, and constitutional symptoms. Chest imaging revealed a large right-sided hydropneumothorax with extensive parenchymal and cavitary disease. Sputum smear microscopy was positive for acid-fast bacilli, and molecular testing of both sputum and pleural fluid detected Mycobacterium tuberculosis complex, confirming pulmonary TB with pleural involvement

Aspiration-related pneumonia can progress to empyema and massive hydropneumothorax, potentially fatal complications not frequently documented in the literature. Patients with structural lung disease and poor dentition can be particularly vulnerable to severe polymicrobial infections arising from seemingly minor aspiration events. We report the case of a man in his early 70s with chronic bronchiectasis, heart failure with reduced ejection fraction, coronary artery disease status post coronary artery bypass grafting, type 2 diabetes mellitus, chronic kidney disease, and poor dentition who presen

Right-sided diaphragmatic hernias in adults are rare and often pose diagnostic challenges due to their atypical clinical and radiological presentations. The coexistence of a spontaneous hydropneumothorax can obscure the underlying pathology, initially diverting attention toward pleural management. We report a rare case of a spontaneous right-sided diaphragmatic hernia discovered incidentally beneath a spontaneous hydropneumothorax in an elderly patient presenting with chest pain, respiratory distress, and abdominal discomfort. Clinical examination revealed decreased breath sounds on the right

A hydropneumothorax is an uncommon complication of pneumonia, particularly in pediatric patients, and typically arises secondary to conditions such as malignancies, esophageal-pleural fistula, thoracic trauma, or thoracocentesis. While pneumothorax is rarely reported in adults with COVID-19 and is even less common in children, isolated cases have been noted in those with Multisystem Inflammatory Syndrome in Children (MIS-C). A recent alert has also been issued about increased Group A Streptococcus (GAS) infections in Europe. Against this background, the primary aim of this case report is to de

COVID-19 is a pandemic viral disease with a catastrophic global impact. The severity of COVID-19 symptoms ranges from very mild to severe and affects mainly the respiratory system. Spontaneous pneumothorax and pleural effusion are rarely seen in spontaneously breathing COVID-19 patients. We herein report a case of a patient with mild COVID-19 disease presenting to the emergency department with hydropneumothorax. Due to persistent air leak, the patient was managed with video-assisted thoracoscopic surgery (VATS) bullectomy and talc pleurodesis. Clinicians managing these patients should be alert

This is a case of a 69-year-old man with a history of asbestos exposure who presented with acute shortness of breath. His chest x-ray showed bilateral hydropneumothorax. Further investigations including CT chest and video-assisted thoracoscopic surgery revealed bilateral pleural thickening and histology confirmed epithelioid mesothelioma. This case highlights the need for clinicians to be aware of atypical presentations of malignant pleural mesothelioma as well as the importance of considering underlying secondary causes such as malignancy in the older patient presenting with spontaneous pneum
