Plasma apolipoprotein CII levels in hypertriglyceridemia.

It is known that hypertriglyceridemia is associated with the elevation of plasma apolipoprotein CII (apoCII). In an attempt to look at the relationship between the two, the present study was conducted. We examined 30 patients with hypertriglyceridemia (TG, 210 to 9,127 mg/dL) and ten normolipidemic controls. Hypertriglyceridemic patients included 7 of type I hyperlipoproteinemia (HL), 17 of type IV, and 6 of type V. Plasma apoCII was measured by the single radial immunodiffusion method. Major cause for any difference in plasma apoCII could be attributed to differences in the TG-rich lipoprotei

Plozasiran: a small interfering RNA therapy for hypertriglyceridemia and familial chylomicronemia syndrome.

Plozasiran (Redemplo®) is a small interfering RNA that reduces hepatic production of apolipoprotein C-III and circulating triglycerides. It degrades ApoC-III mRNA and results in reduced levels of hepatic and serum ApoC-III protein, which leads to an increased clearance of serum triglycerides. In the phase 3 PALISADE trial, plozasiran 25 mg reduced fasting triglyceride levels by approximately 80% at 10 months in patients with familial chylomicronemia syndrome (FCS) and was associated with a lower incidence of pancreatitis compared with placebo. This provides supportive, yet not definitive, evid

Stability of free apolipoprotein A-1 concentration in serum, and its measurement in normal and hyperlipidemic subjects.

Free apolipoprotein A-1 (free A-1) is a low-molecular-mass complex of protein and lipid containing apolipoprotein A-1 (apo A-1). Using crossed immunoelectrophoresis, we separated free A-1 from the apo A-1 in high-density lipoproteins (HDL) and quantified free A-1 by comparison with a reference serum (containing 1.45 g of apo A-1 per liter) diluted in 9 mol/L urea solution. This latter treatment yields apo A-1 containing protein-lipid complexes of the same size and electrophoretic mobility as free A-1. Within-day precision (CV), determined by replicate analysis of two samples with mean free A-1

Frameshift coding sequence variants in the LPL gene: identification of two novel events and exploration of the genotype-phenotype relationship for variants reported to date.

BACKGROUND: Lipoprotein lipase (LPL) is the rate-limiting enzyme for triglyceride hydrolysis. Homozygous or compound heterozygous LPL variants cause autosomal recessive familial chylomicronemia syndrome (FCS), whereas simple heterozygous LPL variants are associated with hypertriglyceridemia (HTG) and HTG-related disorders. LPL frameshift coding sequence variants usually cause complete functional loss of the affected allele, thereby allowing exploration of the impact of different levels of LPL function in human disease. METHODS: All exons and flanking intronic regions of LPL were Sanger sequen

A kindred of familial combined hyperlipidemia (FCHL) with proband showing type V hyperlipoproteinemia.

A case of familial hyperlipidemia incidentally found through a 16 year-old high school girl with type V hyperlipoproteinemia and abdominal bouts consistent with this type of hyperlipemia is reported for the first time in Japan. The laboratory findings of the plasma of her father revealed typical hyperlipoproteinemia of type IIa. Nineteen of her 26 kindred were investigated. Type V was seen only in the proband, type IIa in father, paternal grandmother, two paternal aunts, and two paternal cousins, type IV in three paternal cousins. The serum apolipoprotein (apo A-I, A-II, B, C-II, C-III, and E)

Multifactorial chylomicronemia: keys to detecting severe forms.

Multifactorial chylomicronemia associated with multiple comorbidities, drugs and habits is much more common than familial chylomicronemia, an autosomal recessive disease that can be considered as "rare disease". Like the rest of hypertriglyceridemias, chylomicronemias could be classified as primary or monogenic and secondary in which, on the basis of polygenic predisposition, there is concomitant exposure to multiple triggering factors. In this brief revision, we will review its causes and management as well as the keys to its differential diagnosis of the Multifactorial Chylomicronemia.

Approach to the Adult Patient with Chylomicronemia.

Chylomicronemia, defined by fasting triglycerides ≥10 mmol/L (≥885 mg/dL), has diverse etiologies. When clinical features such as abdominal pain, lipemia retinalis, eruptive xanthomas, hepatosplenomegaly, pancreatitis, or visibly lipemic plasma accompany the biochemical disturbance, the condition is called chylomicronemia syndrome. Subtypes include rare monogenic familial chylomicronemia syndrome (FCS), the more common multifactorial chylomicronemia syndrome (MCS), autoimmune chylomicronemia, and lipodystrophy-associated chylomicronemia. Patients are at risk for acute pancreatitis and sometime

A multi-society Delphi consensus statement on the diagnosis of familial chylomicronemia syndrome.

INTRODUCTION: Familial chylomicronemia syndrome (FCS) is an autosomal recessive disorder that affects approximately 1 to 10 individuals per million and is caused by variants in the genes encoding for the lipoprotein lipase (LPL) enzyme. In addition to its heterogeneous clinical presentation, FCS is characterized by a higher risk of life-threatening, recurrent acute pancreatitis and type 3 diabetes. Since available evidence on FCS in Latin America is limited, there is a clear need for a consensus document that provides relevant recommendations to guide the management of suspected cases and opti

Familial chylomicronemia syndrome: an under-recognized cause of severe hypertriglyceridaemia.

Familial chylomicronemia syndrome (FCS) is a rare autosomal recessive disorder of chylomicron metabolism causing severe elevation of triglyceride (TG) levels (>10 mmol L-1 ). This condition is associated with a significant risk of recurrent acute pancreatitis (AP). AP caused by hypertriglyceridaemia (HTG) has been associated with a worse prognosis and higher mortality rates compared to pancreatitis of other aetiology. Despite its association with poor quality of life and increased lifelong risk of HTG-AP, few healthcare providers are familiar with FCS. Because this condition is under-recognize

[Molecular genetics and lipoprotein lipase deficiency].

Lipoprotein lipase (LPL) deficiency is the main cause of familial chylomicronemia, a disease characterised by high fasting plasma triglyceride levels, that can be complicated with acute pancreatitis. This autosomal recessive disorder is rare (1/10(6) in the general population). Classically this disorder is non atherogenic, and the heterozygotes are asymptomatic. To date, 35 gene mutations have been described throughout the world. We have studied 12 families in which the molecular basis for LPL deficiency had been established (direct sequencing of PCR products) by the presence of mutations on t

[Comparative status of dietetic and therapeutic measures in idiopathic hyperlipidaemia (author's transl)].

The indications for dietetic and therapeutic measures vary according to the type of hyperlipidaemia. A low fat diet is difficult to maintain and has exceptional and precise indications. It is indispensable in type I hyperlipidaemia, particularly since there is at present no drug capable of reducing hyperchylomicronaemia. It is also indicated in type V hyperlipidaemia in association with a low carbohydrate diet--a dietetic treatment which may become troublesome in long terme and must be supervised by highly specialized dieticians. In all other types of hyperlipidaemia systematic reduction of fa

[Beta 2 glycoprotein I analysis in patients with hyperlipoproteinemia, arteriosclerotic occlusive disease and diabetes mellitus].

Beta 2-glycoprotein-I (apolipoprotein H) is an activator of the lipoprotein lipase. The concentration of beta 2-glycoprotein-I in the blood serum was determined with the help of the radial immunodiffusion. In patients with hyperlipoproteinaemia of type IIa and IIb, arteriosclerotic obstructive disease or diabetes mellitus the beta 2-glycoprotein-I-concentrations were increased. In hyperlipoproteinaemia of type IV can be concluded to a relative beta 2-glycoprotein-I-deficiency.

Detection of dyslipoproteinemia with the use of plasma total cholesterol and triglyceride as screening tests. The Lipid Research Clinics Program Prevalence Study.

The efficiency of screening for dyslipoproteinemias associated with hyperlipoproteinemia or with hypolipoproteinemia was examined in 8449 white examinees from the Lipid Research Clinics Prevalence Study. A two-stage lipid screening approach was used. A positive screening test for hyperlipidemia was defined as an elevated level of plasma total cholesterol or triglyceride at both visit 1 and visit 2. A positive screening test for hypolipidemia was defined as a reduced plasma total cholesterol level at both visits. The syndromes of dyslipoproteinemia were defined according to the lipoprotein patt

Postprandial adipose tissue lipoprotein lipase activity in primary hypertriglyceridemia.

The fasting activity of adipose tissue lipoprotein lipase has been previously reported to be either normal or reduced in subjects with a primary form of hypertriglyceridemia. The postprandial activity of adipose tissue lipoprotein lipase has not been previously reported in these subjects. In subjects with primary hypertriglyceridemia the fasting lipoprotein lipase activity eluted from pieces of adipose tissue by heparin and the enzyme activity present in extracts of acetone--ether tissue powders were similar to the level of enzyme activity found in normal subjects. There also was no difference

The clinical, biochemical, and familial presentation of type V hyperlipoproteinemia in childhood.

Primary type V hyperlipoproteinemia was identified in two preadolescent children. The propositus (kindred N) was a 10-year-old girl with severely creamy plasma, lipemia retinalis, hypertriglyceridemia (triglyceridelevel, 6,800 mg/100 ml), and ypercholesterolemia (cholesterol level, 490 mg/100 ml). Her parents and an 8-year-old sister all had endogenous hypertriglyceridemia (type IV hyperlipoproteinemia). In kindred A, an 11-year-old boy had triglyceride levels as high as 1,100 mg/100 ml and recurrent abdominal pain. His father had type V hyperlipoproteinemia; his mother was normal. All three o

Binding of thyroid hormones to human plasma lipoproteins.

The binding of T4, T3, and rT3 to plasma lipoproteins was investigated in normal subjects and patients with abnormal lipoprotein metabolism. Gel filtration on Sepharose CL-6B demonstrated iodothyronine binding to all lipoprotein classes. In the total lipoprotein fraction (density less than 1.210 g/mL), high density lipoproteins (HDL) were the major binders, accounting for 92% of lipoprotein-bound T4, 99% of lipoprotein-bound T3, and 55% of lipoprotein-bound rT3. The estimated iodothyronine binding in normal plasma to HDL, low density lipoproteins (LDL), and very low density lipoproteins (VLDL)
