Herpes zoster keratouveitis with hypopyon and hyphema.

This study aims to report two cases with an uncommon, early manifestation of herpes zoster ophthalmicus which is keratouveitis. The first patient is a 61-year-old female who had presented with painful facial skin eruption and right eye redness without impairment of vision. She was treated initially as herpes zoster blepharoconjunctivitis; however, the disease had progressed to neurotrophic keratitis with severe anterior chamber reaction manifested by a mixture of hypopyon and hyphema. The second patient is a 74-year-old female who had presented after 2 weeks of facial skin eruption with blurri

Outpatient management of traumatic hyphemas in children.

OBJECTIVE: There are no outcome reports in the literature for outpatient management of traumatic hyphemas in children. Therefore we studied such cases at our institutions. DESIGN: Retrospective chart review. PARTICIPANTS: 25 children less than 16 years of age with grade I traumatic hyphemas. INTERVENTION: Inactivity at home with frequent office visits to monitor progress and complications. Treatment otherwise varied but virtually all patients received protective eye shields and topical steroids. MAIN OUTCOME MEASURES: Rate of rebleeding and final visual acuity. RESULTS: Three patients (12

Surgical management of uveitis-glaucoma-hyphema syndrome.

AIM: To report outcomes of patients after intraocular lens (IOL) repositioning or exchange for the version of the uveitis-glaucoma-hyphema (UGH) syndrome that does not include closed loop anterior chamber IOL (nUGH). METHODS: Chart review of patients with nUGH who underwent IOL repositioning or exchange by one surgeon were reviewed. The main outcome measures were best corrected visual acuity (BCVA) as a decimal fraction preoperatively and postoperatively after IOL repositioning or exchange. Clinical findings evaluated included the presence of uveitis, hyphema, elevated intraocular pressure (I

Ultrasound biomicroscopy in uveitis-glaucoma-hyphema syndrome.

PURPOSE: To assess anterior segment anatomy in uveitis-glaucoma- hyphema syndrome. DESIGN: Retrospective case series. METHODS: Nine pseudophakic eyes (nine patients) with complete or incomplete uveitis-glaucoma-hyphema syndrome underwent ultrasound biomicroscopy. RESULTS: Ultrasound biomicroscopy revealed intraocular lens malposition in each case. Of the eight eyes with posterior chamber intraocular lenses, haptics were in contact with the iris pigment epithelium (four eyes) or the pars plicata (three eyes) or prolapsed into the angle recess near a filtration bleb internal ostium (one eye).

Aspirin and secondary bleeding after traumatic hyphema.

A randomized, controlled study was conducted in 51 patients to investigate the effect of aspirin administration on traumatic hyphema. Aspirin tablets 500 mg tid were administered for seven days. Rebleeding was found in three of 23 eyes of patients who had received aspirin and in two of 28 eyes of control patients. This difference was not statistically significant (P = .405), proving that aspirin is not an important factor in rebleeding in patients with hyphema.

[Hyphema, a risk factor in the glaucomatous eye].

A hemorrhage in the anterior chamber is a frequent phenomenon, very often treated too easily. Our investigation followed for 3 years (1985-1987) the way in which hyphema were resorbed after the eye contusions in the patients operated by cataract or glaucoma. The causes of hyphemas, their onset, the aspect of the blood and the resorption period are analyzed. The majority of hyphemas were resorbed in the first 7 days (in the case of contusions 87%, after operation of cataract 84%) but in the glaucomatous patients the resorption in the first 7 days appeared only in 50% of the cases; the blood rem

Systemic corticosteroids in the treatment of traumatic hyphema.

As a result of Yasuna's 1974 report on the desirable effect of systemic steroids on the rebleeding rate in traumatic hyphema, we added systemic steroids to our treatment protocol. In the 18 cases so treated since then, there has been only one episode of rebleeding (5.5%). A control group of 33 patients treated prior to the addition of systemic steroids to the protocol, or by other physicians not using steroids, had a rebleed rate of 33%. The difference in rebleed rate between the two groups was statistically significant.

Aminocaproic acid in the treatment of traumatic hyphema.

Of 59 patients with traumatic hyphema studied prospectively, 32 received aminocaproic acid, an antifibrinolytic agent, to prevent secondary hemorrhage. The remaining 27 patients received placebo in an identical manner. Of the aminocaproic acid-treated patients, only one (3%) rebled, while nine patients (33%) receiving placebo suffered secondary hemorrhage. The results of this study indicate a statistically significant reduction (P less than .01) in the incidence of rebleeding in patients treated with this drug.

Traumatic hyphema: clinical study of 149 cases.

The natural course, related complications and factors affecting prognosis of traumatic hyphema in 149 Thai patients were reported. Male preponderance and high prevalence of patients younger than 15 years were noted in this study. Blunt ocular trauma was the most common type of eye injury (88.59%), with penetrating injury (7.38%) and missile (4.03%) being the next in order of decreasing frequency. Duration of injury was acute in 81.88 per cent, subacute in 12.08 per cent and chronic in 6.04 per cent. Complications from secondary glaucoma accounted for 24.16 per cent, whereas, corneal blood stai

The incidence of secondary hemorrhage after traumatic hyphema.

A retrospective study was undertaken to determine the incidence of secondary hemorrhage after traumatic hyphema in a medium-sized Canadian city. This was found to be 10%, which is similar to the incidence reported in other Canadian and northern European cities but much lower than that reported in some large American cities. The different racial mix of the sample populations is the most likely reason for these differences.

Global Current Practice Patterns for the Management of Hyphema.

PURPOSE: Hyphema is a sequela of ocular trauma and can be associated with significant morbidity. Management of this condition is variable and can depend on individual institutional guidelines. We aimed to summarize current practices in hyphema management across ophthalmological institutions worldwide. METHODS: A cross-sectional online survey was conducted across North America, Asia, South America, Africa, Europe, and Australia from August 2020 to January 2021. The survey assessed the existing practices in the management of hyphema at each institution. RESULTS: For layered hyphema, topical st

Mechanisms for in-the-bag uveitis-glaucoma-hyphema syndrome.

We propose 2 mechanisms of uveitis-glaucoma-hyphema (UGH) syndrome in 2 patients with intracapsular or in-the-bag single-piece acrylic intraocular lenses (IOLs). In the first case, pseudophacodonesis secondary to zonular laxity from pseudoexfoliation syndrome caused chafing of the posterior iris by the square-edged haptic. In the second case, focal capsular fibrosis around the square-edged haptics combined with anteriorly rotated ciliary processes in plateau iris configuration caused points of chafing. Extensive capsular fibrosis of the haptic in both cases precluded IOL exchange. In the first

A case of herpes zoster uveitis with severe hyphema.

BACKGROUND: Uveitis sometimes causes hyphema, but severe hyphema as a complication following herpes zoster uveitis has rarely been reported. We report a rare case of zoster sine herpete with unusually severe hyphema. CASE PRESENTATION: A 41-year-old Japanese female developed hyphema filling almost one-half of the depth of the anterior chamber after a two-week history of unilateral anterior uveitis. Hyphema persisted for four weeks while sectorial iris atrophy became gradually apparent. Systemic prednisolone and valaciclovir resulted in prompt resolution of uveitis and hyphema. Serum anti-vari

Bilateral spontaneous hyphema with uveitis in a young girl.

A healthy 5-year-old girl presented with bilateral hyphema as an initial symptom of uveitis. The ocular findings and the patient's age suggested that the uveitis was due to juvenile rheumatic arthritis or chronic iridocyclitis.

OBJECTIVES: To determine whether topically applied aminocaproic acid, like systemic aminocaproic acid, effectively reduces secondary hemorrhage after hyphemas and to compare the safety and effectiveness of topical application with those of systemic use and a control group. DESIGN: A prospective, randomized, double-masked, multicenter study. PATIENTS: Sixty-four patients with traumatic hyphema treated with topical or systemic aminocaproic acid and compared with 54 control patients with hyphema. Traumatic total hyphema in a patient with severe hemophilia.

A 12-year-old hemophilic boy suffered from secondary glaucoma in his right eye due to total hyphema following trauma. An emergency operation was performed to evacuate the hyphema under a tight anti-hemophilic treatment. A careful postoperative treatment with factor VIII concentrate, antiglaucomatous drugs and aminocaproic acid was administered. Intraocular pressure returned to normal and remained so during the four months follow-up period. Visual acuity returned to 6/9 and there was no rebleeding into the anterior chamber. To the best of our knowledge, this is the first reported case of trauma
