Sedative-hypnotic drugs and the risk of hip fracture.

A retrospective cohort study was conducted in individuals 65 years of age and older using Medicaid-reimbursed claims to assess the risk of hip fracture in users of two sedative-hypnotic drugs, triazolam and temazepam. Using the triazolam cohort as the referent group, the rate ratio was 0.92 (95% confidence interval, 0.72 to 1.17) for hip fracture with temazepam. Stratifying by age, sex, race, residence, time enrolled in Medicaid, prescription number, combinations of these, and several other potential confounding variables did not materially change the results. Compared with the short-acting be

Sedative and hypnotic midazolam-morphine interactions in rats.

The midazolam-morphine interactions in relation to the sedative effect and in relation to the hypnotic effect were studied in rats. Two series of experiments (sedative and hypnotic) were performed. In the sedative series, doses that inhibited locomotor activity to 10% or more of the control level were determined when the agents were given singly or in combination. Dose-response curves were determined with a probit procedure. The ED50 values of both agents and their combination were compared with algebraic (fractional) and isobolographic analyses in one subseries of experiments. The effect of a

Tranquilizers and sedative/hypnotics: appropriate use in the elderly.

In treating the elderly patient, it is generally true that the fewer pharmacologic agents needed, the less the potential for drug interactions and puzzling toxic reactions. Nevertheless, sedative/hypnotics and anxiolytic drugs (mostly the benzodiazepines) are safe and effective therapy for the older patient, when the appropriate indications and dosages are observed. On the other hand, the toxic potential of nonbenzodiazepines (eg, barbiturates and pseudobarbiturates) and of even the relatively safe alternatives (eg, chloral hydrate, diphenhydramine, and hydroxyzine) render them less useful for

Effects of antidepressants, anesthetics and sedatives/hypnotics on sleep-associated memory consolidation - A systematic review.

Considering the wide and increasing utilization of drugs which act upon the central nervous system, a pressing, infrequently investigated question is their potential impact on sleep-associated memory consolidation. Here the influence of approved pharmaceuticals on memory consolidation during sleep in healthy humans, rats, and mice was investigated. A systematic review of the literature was conducted according to PRISMA guidelines. Criteria to be met included healthy subjects, drug application after learning and a comparable placebo condition. Exclusion criteria included but were not limited to

General practitioners' concepts on issuing out-of-pocket prescriptions for hypnotics and sedatives in Germany.

BACKGROUND: In Germany, almost 50% of prescriptions for benzodiazepines and drugs as Zolpidem and Zopiclone are as out-of-pocket (OOP) presc … LTS: Knowing the significant medical risks and insecurity about regulations makes GPs wish to avoid hypnotics and sedatives. They achieve this by 'Creating a barrier' (central phenomenon) and employing the strategy 'Using an out-of-pocket prescription', which not only generates costs for the patient but also reduces the physicians´ legal and financial accountability. The …

Trends in Stimulant and Sedative/Hypnotic Dispensing: An Exploratory Study.

OBJECTIVE: To investigate patterns and trends of co-prescriptions of stimulants and sedatives within the last 6 years at a tertiary care center. METHOD: Patients 18 years of age and older who were dispensed at least one stimulant prescription from an institutional pharmacy between 1/1/2015 and 7/1/2021 were included. Prescription data for any co-prescribed sedative/hypnotic were collected. RESULTS: Both the number of stimulant dispenses and the number of patients with stimulant dispenses increased significantly with yearly incidence rate ratios of 1.115 (95% CI [1.110, 1.119]) and 1.090 (95%

Patterns of sleep disorders and sedative hypnotic use in seniors.

Adequate sleep is required for good physical and psychological health. Sleep disturbance is common and its prevalence increases with advancing age. Physiologically, sleep in elderly adults differs from that in younger adults, both in terms of quantity and quality. Sleep disturbance in old age may be associated with many physical and psychological conditions, and less commonly can occur as a primary disturbance. It must be distinguished from the understandable but unrealistic expectations of many elderly people that they will sleep for as long and as soundly as when they were younger. The evalu

Paradoxical excitement to sedative-hypnotics in mentally retarded clients.

The relationships among "paradoxical" excitement to sedative--hypnotic medication, self-injurious behavior, and perinatal trauma were evaluated. Mentally retarded patients were classified as either paradoxical or normal responders to sedative-hypnotics. Paradoxical responders to these medications have a lower MA, a history of perinatal trauma, self-injurious behavior (SIB), and aggressive behavior when compared to normal responders. These findings confirmed and extended previous reports that a type of SIB may be indexed by paradoxical response to sedative-hypnotics. Results also suggested that

[Dynamics of mental disorders in poisoning with hypnotic and sedative preparations].

In order to study some mental disorders due to poisoning by sleeping and sedative drugs 258 patients with mental and boderline conditions (who had taken these drugs in toxic doses) were studied in the Sklifosovskiy Moscow Research Institute of Emergency Medical Aid. The revealed mental disorders on different stages of poisoning were less differentiated in the acute period (clouded consciousness, coma) and most diverse in pre- and postcomatose stages, where they were characterized by disturbed consciousness, hallucinatory and delusional experiences, epileptiformal syndromes, emotional, motor an

Area Deprivation Index and Coprescription of Opioids and Sedative Hypnotics.

Background and objectives The coprescription of an opioid and a sedative-hypnotic (i.e., benzodiazepine or Z-drug) is associated with negative patient outcomes. Although multiple guidelines recommend minimizing this combination, studies demonstrate high coprescribing patterns. The Area Deprivation Index (ADI), a validated surrogate measure of socioeconomic status, was used to assess socioeconomic disadvantage. The objective of this study was to evaluate the current prevalence and the relationship between coprescriptions and ADI scores of patients. Methods A single-center retrospective observat

The mechanism study of YZG-331 on sedative and hypnotic effects.

YZG-331 is a synthetic novel derivates of N6-(4-hydroxybenzyl) adenine riboside (NHBA), which has potent sedative and hypnotic effects based on our previous study. We are now aiming to investigate the mechanism of YZG-331. In this research, the behavioral studies showed that YZG-331 (4, 8, 16 mg/kg, i.g.) could reduce the spontaneous locomotor activity in mice, which could be blocked by AM (non-selective adenosine receptor antagonist), DPCPX (adenosine A1 receptor (A1R) antagonist), and SCH58261 (adenosine A2a receptor (A2aR) antagonist). Moreover, YZG-331 no longer exerted sedative effect in

Genomic and functional conservation of sedative-hypnotic targets in the zebrafish.

OBJECTIVES: The zebrafish is an ideally suited vertebrate animal model for large-scale genetic screens and is emerging as a model organism in pharmacological and behavioral research. We investigated the effects of sedative hypnotics commonly used in humans on zebrafish locomotor activity and identified the corresponding genomic and receptor binding targets. METHODS: We studied radioreceptor binding and behavioral responses to compounds with known sedative hypnotic properties representing multiple pharmacological classes. These included GABAergic hypnotics such as benzodiazepines, barbiturates

No effect of albinism on sedative-hypnotic sensitivity to ethanol and anesthetics.

BACKGROUND: Studies using the long-sleep (LS) X short-sleep (SS) (LSXSS) recombinant inbred mice and inbred long-sleep (ILS) by inbred short-sleep (ISS) intercrosses have found genetic linkage between Tyr albinism (c/c) and differential sensitivity to sedative-hypnotic doses of ethanol and general anesthetics. This linkage could be due to a gene or genes near Tyr or Tyr itself. With regard to the latter possibility, the absence of tyrosinase activity (encoded by Tyr) in albinos could alter tyrosine availability and thus the rate-limiting step in catecholamine synthesis. In addition, albinism i

Zaleplon: a pyrazolopyrimidine sedative-hypnotic agent for the treatment of insomnia.

BACKGROUND: Insomnia is the subjective complaint of poor sleep or an inadequate amount of sleep that adversely affects daily functioning. For the past 4 decades, treatment of insomnia has shifted away from the use of barbiturates toward the use of hypnotic agents of the benzodiazepine class. However, problems associated with the latter (eg, next-day sedation, rebound insomnia, dependence, and tolerance) have prompted development of other agents. OBJECTIVE: This review describes the recently approved nonbenzodiazepine agent, zaleplon. METHODS: Studies of zaleplon were identified through a sea

The use of sedative-hypnotic drugs in a university teaching hospital.

We reviewed the charts of 476 patients admitted to a university teaching hospital to determine whether sedative-hypnotic drugs (SHDs) were being used excessively and to examine the use of SHDs as hypnotics. The frequency of medical and surgical indications for barbiturates and benzodiazepines or other minor tranquillizers as well as the use of such drugs were compared among different groups of patients and specialty wards. Of the patients 29% had a regular order and 40% had a PRN order; only 77% of the PRN orders were administered. A total of 215 patients (45%) received an SHD during their hos
