Hysteroscopic surgery--complications and their prevention.

During the last few years, diagnostic hysteroscopy has become a standard procedure in the diagnosis of abnormal uterine bleeding, sonographically suspicious endometrial reflex and fertility disorders. At the same time the hysteroscopic treatment of intrauterine pathology is becoming more common. Today, hysteroscopic resection of uterine myomas, dissection of uterine septa, and endometrial ablation are standard procedures. Using monopolar cutting devices and saline-free distension media, hysteroscopic surgery bears specific risks. The knowledge of these risks is important to avoid typical compl

Fertility after hysteroscopic myomectomy.

The available data concerning hysteroscopic myomectomy for submucous myomas has been reviewed with the aim of evaluating the safety and efficacy of this technique in infertile women who wish to preserve or to enhance their fertility potential. A total of 134 infertile patients undergoing hysteroscopic myomectomy by resectoscope or by Nd: YAG laser or by scissors have been collectively recorded. Of these, 79 (58.9%) patients conceived. This clinical outcome is similar to that occurring after laparotomic submucous myomectomy. Moreover, the hysteroscopic approach has several advantages over its a

[Hysteroscopic metroplasty of uterine septa].

A series of 23 patients with septate uteri who underwent hysteroscopic division of the septum between January 1990 and March 1992 were studied. Before operation 19 of the 23 patients had 50 pregnancies with 29 spontaneous abortions, 7 late abortions, 5 premature deliveries and 9 deliveries at term. Sixteen patients who were followed up for more than 6 months conceived 9 times. Six of these went to term with 4 normal deliveries and 2 caesarean sections for obstetrical indications. There are 2 pregnancies continuing at present (at 25 and 27 weeks of amenorrhoea) and one spontaneous abortion in t

[Hysteroscopic resection of submucous myoma].

Between 1988 and 1991, 39 patients with intrauterine submucous fibroids were treated at the University of Düsseldorf, Department of Gynaecology and Obstetrics, by hysteroscopic resection. Myoma diameter ranged from 1 cm to 6 cm with a median of 2.6 cm. 29 patients had bleeding problems in form of meno-metrorrhagia; 14 patients suffered from infertility, 2 patients were treated by combined transabdominal-transcervical approach because of multiple myomata. One severe intraoperative complication occurred as an uterine perforation with injury to the small bowel. No other complication was seen. All

Hysteroscopic removal of submucous leiomyomas.

Uterine leiomyomas are benign neoplasms that, when symptomatic, may require surgical treatment. The most symptomatic leiomyomas are of the submucous variety, causing persistent or recurrent uterine bleeding. Fifty-nine women experiencing abnormal uterine bleeding, dysmenorrhea, and/or infertility were diagnosed to have submucous leiomyomas, and of these, 52 were treated by hysteroscopic removal. In all women treated, the symptoms disappeared or markedly decreased. Of 16 patients with associated infertility, 10 conceived and 8 of these delivered a viable infant vaginally. Two women had a sponta

[Hysteroscopic resection of the uterine septum].

An endoscopic alternative to the surgical treatment of uterine septum is presented. The hysteroscopic technique of septal division is described on the basis of 7 preliminary cases, with emphasis on the equipment used, the choice of the irrigation system, the problems encountered near the fundus uteri, where it is important to spare the myometrium, and the adjuvant treatments, notably in the post-operative period. Interventional hysteroscopy has a very low trans--and post-operative morbidity and seems to be a suitable procedure for the treatment of this type of uterine malformation.

Hysteroscopic treatment of the septate uterus.

Twelve patients with symptomatic septate uteri underwent hysteroscopic division of the uterine septum, monitored by concomitant laparoscopy, over a six-year period. The preoperative reproductive performance included 42 pregnancies, with only three viable deliveries. Ten of the 12 patients conceived within one year after therapy, and eight of these patients delivered a live infant at term. Two patients had a spontaneous abortion at six and eight weeks after therapy, respectively, but in each, a subsequent pregnancy was carried to term. Three patients are currently pregnant (eight, 12, and 18 we

Hysteroscopic treatment of uterine septa.

Patients with septate uterus usually undergo reproductive failures with recurrent abortions and premature deliveries. Up till now surgical therapy for such a defect has relied on different abdominal metroplasty procedures reflecting the need to establish normal anatomy and physiology of the uterus. Nowadays, wide experience accumulated in hysteroscopic management of intrauterine adhesions, has allowed the incision of the uterine septa with the hysteroscopic technique. In this report the Authors describe the surgical technique of the hysteroscopic metroplasty in eleven cases. Initial results ar

Permanent sterilization by hysteroscopic cauterization.

A small series of 22 patients who underwent hysteroscopy cauterization is reported. The uterus was distended with CO2 and dextran. Cauterization was done with a 6 mm. long, active-tip, No. 4 electrode: There was a 27 per cent failure rate for the controlled cases, this dropping to 11 per cent after recauterization. A significant complication, small bowel perforation, was treated by resection and anastomosis with complete recovery. The uterine distention with CO2 is safe but ostium visualization is not very good; Dextran distention is more cumbersone but gives excellent visualization and exposu

Complications in modern hysteroscopic myomectomy.

Uterine fibroids (UFs) are common benign tumors of the female genital tract, diagnosed in almost one-quarter of women of reproductive age. UFs may cause numerous clinical symptoms, including prolonged or heavy menstrual bleeding, pelvic pressure symptoms, pain, infertility and others. Submucous fibroids arise from the muscular part of the uterus and pen-etrate into the uterine cavity. They are mostly managed with the use of hysteroscopic myomectomy (HM), which provides direct visualization from the transcervical approach. The sheer number of HM standards and techniques is reason enough to revi

Hysteroscopic Management of Asherman's Syndrome.

In developed countries Asherman's syndrome is almost always the result of a prior intrauterine operative trauma. This is often asymptomatic but may result in hypo- or amenorrhea and can contribute to infertility and pregnancy complications. We review their etiology, clinical implications, and systems proposed to classify their extent. The numerous methods reported for performing lysis of intrauterine adhesions are summarized along with clinical results. Current strategies to prevent recurrence of intrauterine adhesions have not been conclusively shown to be clinically effective, but the potent

Hysteroscopic management of a stenotic cervix.

OBJECTIVE: To demonstrate an approach to the hysteroscopic management of a stenotic cervix. DESIGN: Step-by-step explanation of the techniques using video and animation (educational video). SETTING: Academic tertiary level referral center. PATIENT(S): Patients with cervical stenosis, inclusive of both reproductive age and postmenopausal women. Gynecologists require intrauterine access for many procedures, but a stenotic cervix can obstruct surgery. Blind dilation of a stenotic cervix can lead to a cervical laceration or uterine perforation, with concomitant complications. INTERVENTION(S):

Hysteroscopic hydrotubation of the equine oviduct.

REASON FOR PERFORMING STUDY: Diagnostic techniques for oviductal obstruction in the mare are limited and development of a more reliable and direct method to assess oviductal patency is needed. OBJECTIVE: To evaluate the feasibility of hysteroscopic selective hydrotubation of the equine oviduct via videoendoscopy in standing mares. METHODS: Using a catheter inserted under endoscopic guidance into the uterotubal junctions of 10 mares, 5 ml of indigo carmine solution (4 mg/ml) was flushed into the oviduct. After introduction of the dye, peritoneal fluid was obtained via abdominocentesis. A colp
