Preventing nonvertebral osteoporotic fractures with extended-interval bisphosphonates: regimen selection and clinical application.

CONTEXT: Nonvertebral fractures (NVFs) are the most costly and disabling type of osteoporotic fractures. Bisphosphonate therapy effectively reduces the risk for NVFs; however, fracture protection depends critically on adherence and persistence. Approved bisphosphonate regimens with extended dosing intervals increase patient convenience, help patients remain on therapy, and improve fracture protection in clinical practice. EVIDENCE ACQUISITION: To assess evidence for NVF reduction with extended-interval bisphosphonates, we searched PubMed for phase 3 clinical trials, meta-analyses, and reviews

Flare Phenomenon After 177 Lu-DOTA-IBA Therapy in Bone Metastases of Lung Cancer.

The flare phenomenon is a transient increase in the number or intensity of lesions on bone scans after treatment, signifying curative effect. DOTA-ibandronic acid (DOTA-IBA) is a new prodrug that targets bone metastases and can be labeled with 177 Lu. Here, we report the case of a 58-year-old woman with bone metastasis, in whom the flare phenomenon was observed after 4 cycles of 177 Lu-DOTA-IBA treatment. No adverse effects were observed during the treatment and follow-up periods.

The effect of monthly ibandronate on bone mineral density and bone turnover markers in patients with haemophilia A and B and increased risk for fracture.

Haemophilia A and B have been associated with increased prevalence of low bone mineral density (BMD). However, no study has so far evaluated the effects of anti-osteoporotic therapy on BMD in haemophilia. The primary endpoint of this prospective study was to estimate the effect of 12-month therapy of oral ibandronate 150 mg/month on BMD in patients with haemophilia A and B. Secondary endpoint was its effect on turnover markers (BTM) of bone resorption [serum C-terminal telopeptide of type 1 collagen (sCTX), tartrate-resistant acid phosphatase band 5b] and bone formation (osteocalcin and bone-sp

Gastrointestinal tolerability with ibandronate after previous weekly bisphosphonate treatment.

Data from two open-label trials (PRIOR and CURRENT) of women with postmenopausal osteoporosis or osteopenia were evaluated to assess whether monthly oral and quarterly intravenous (IV) ibandronate dosing improved self-reported gastrointestinal (GI) tolerability for patients who had previously experienced GI irritation with bisphosphonate (BP) use. In PRIOR, women who had discontinued daily or weekly BP treatment due to GI intolerance received monthly oral or quarterly IV ibandronate for 12 months. The CURRENT subanalysis included women receiving weekly BP treatment who switched to monthly oral

The first reported case of Atypical Femoral Fracture caused by daily ibandronate prescribed for bone metastases in breast cancer.

A 55-year-old woman with bony metastatic breast cancer was commenced on daily ibandronic acid to prevent skeletal related events (SREs). Four years later, she began to experience new lower limb and groin pain with investigations leading to the suspicion of further metastatic spread to her left femur. While awaiting radiotherapy for this, she unfortunately tripped and fell sustaining a fracture to the proximal third of her left femur. Radiographic findings of her femur from both before and after the fall were suggestive of an atypical femoral fracture, presumed secondary to her bisphosphonate t

Efficacy of bisphosphonates in reducing fracture risk in postmenopausal osteoporosis.

Bisphosphonates have been available for more than a decade. Currently, 4 bisphosphonates--alendronate, risedronate, ibandronate, and zoledronic acid--are approved in the United States. Alendronate and risedronate are oral agents, ibandronate is available in oral and intravenous formulations, and zoledronic acid is an intravenous drug. This review summarizes results from pivotal clinical trials in which these bisphosphonates have been shown to reduce risk for osteoporotic fractures. Also reviewed are results of "bridging" studies designed to demonstrate the comparable efficacy of less frequent

Bisphosphonate-associated adverse events.

The adverse events of the nitrogen-containing bisphosphonates are reviewed. Oral bisphosphonates (alendronate, risedronate and ibandronate), mainly used for the treatment of osteoporosis, have been associated with adverse events from the upper gastrointestinal tract, acute phase response, hypocalcaemia and secondary hyperparathyroidism, musculoskeletal pain, osteonecrosis of the jaw and ocular events. Intravenous bisphosphonates (pamidronate, ibandronate and zoledronic acid), used in oncology and for the treatment of osteoporosis, have been associated with all the above adverse events, except

Combination of bisphosphonates and antiangiogenic factors induces osteonecrosis of the jaw more frequently than bisphosphonates alone.

BACKGROUND: The use of bisphosphonates is associated with osteonecrosis of the jaw (ONJ). Antiangiogenic agents are used with increasing frequency and may induce the risk of ONJ, especially when administered concurrently with bisphosphonates. PATIENTS AND METHODS: We retrospectively reviewed data of 116 patients receiving bisphosphonates, 78 zoledronic acid and 38 ibandronic acid, with or without antiangiogenic agents for osseous metastases from various tumors in our department from June 2007 to June 2008. RESULTS: ONJ developed in: 2 patients with breast cancer and 1 with colon cancer recei

Medication-related osteonecrosis of the jaw following osteoporosis therapy: A real-world retrospective cohort study using the TriNetX global collaborative network.

INTRODUCTION: Medication-related osteonecrosis of the jaw (MRONJ) was first reported in cancer patients receiving high-dose intravenous bisphosphonates and later linked to other anti-resorptive agents such as denosumab and romosozumab. However, large-scale real-world data on MRONJ risk following these therapies remain limited. This study focuses on the widely used parenteral anti-resorptive agents-ibandronate, denosumab, and zoledronic acid-chosen for their clinical popularity, cost-effectiveness, and convenient dosing. We aimed to assess the risk of MRONJ associated with each agent using real

New and emerging treatments for osteoporosis.

PURPOSE OF REVIEW: This review will focus on three new treatments for postmenopausal osteoporosis which have either been recently released (intravenous ibandronate), or have completed (zoledronic acid) or are currently in (denosumab) phase III trials. RECENT FINDINGS: A number of agents have demonstrated fracture risk reduction in randomized clinical trials, however, successful treatment of osteoporosis in the individual patient remains a challenge. Adherence to, and persistence with, all current osteoporosis medications are poor, being approximately 50% at 1 year for weekly bisphosphonates.

Examination of Risk Factors and Expression Patterns of Atypical Femoral Fractures Using the Japanese Adverse Drug Event Report Database: A Retrospective Pharmacovigilance Study.

Atypical femoral fracture (AFF) is a rare complication related to the use of bisphosphonates (BPs). Herein, we analyzed the risk factors and onset patterns of AFF using the Japanese Adverse Drug Event Report database and reported the findings. First, the independent risk factors for AFF were gender (female), high body mass index, and medical history of osteoporosis, arthritis, and systemic lupus erythematosus (SLE). Drug-related risk factors for AFF included BPs (i.e., alendronic acid, ibandronic acid, etidronic acid, zoledronic acid, minodronic acid, risedronic acid), denosumab, prednisolone,

Prospects for treatment of Porphyromonas gingivalis-mediated disease - immune-based therapy.

Bisphosphonates are the primary pharmacological agents used for the management of osteoporosis and hypercalcaemia of malignant bone disease. The efficacy of these agents in these two conditions has been demonstrated in many well designed trials published over the past 2 decades. The variety of bisphosphonates currently available to us provides a wide range of tolerability and dosing profiles thus necessitating a thorough comparison of the most recent oral and intravenous bisphosphonates to differentiate the clinical context in which they should be used. Despite the fact that bisphosphonates ar
