Abnormalities of immunoglobulin A1 (IgA1) glycosylation have been described in patients with IgA nephropathy (IgAN), whether primitive or secondary to Henoch-Schönlein purpura. The Wiskott-Aldrich syndrome, an X-linked recessive disorder, is associated with abnormalities of IgA. Renal involvement with mesangial IgA deposition identical to that found in IgAN has been reported during this affection. We report the case of a female carrier of the Wiskott-Aldrich syndrome presenting with Henoch-Schönlein purpura and abnormalities of IgA glycosylation, as previously reported in patients with IgAN. T

With the rapidly expanding therapeutic landscape in IgA nephropathy, that incorporates new drugs with varied mechanisms of action, many of which may lead to modification of the fundamental pathophysiology of the world's commonest glomerular disease, thoughts are now turning to how we should define disease modification in IgA nephropathy. In the absence of validated biomarkers that reflect the pathogenic cascade we currently have to rely on proteinuria and eGFR, some would also advocate for inclusion of haematuria, and others for galactose-deficient IgA. In this review the approach to defining

Immunoglobulin A nephropathy (IgAN) remains the leading primary glomerular disease worldwide, with a majority of patients reaching kidney failure within their lifetimes. IgAN is a heterogeneous glomerular disorder characterized by mesangial deposition of galactose-deficient immunoglobulin A1 containing immune complexes that induce glomerular injury and nephron loss. The primary therapeutic goal in treating IgAN is reduction of nephron loss from the time of diagnosis. Management ideally incorporates interventions targeting both immunologic and nonspecific chronic kidney disease pathomechanisms.

Immunoglobulin A (IgA) nephropathy is the most frequent glomerulonephritis in adults in Central Europe. It is characterized by microhematuria and occasionally macrohematuria, proteinuria and a chronic loss of kidney function. The diagnosis is made based on a kidney biopsy. The progressive kidney damage must always be slowed down by normalizing blood pressure, using angiotensin inhibitors and consistently avoiding additional toxic substances. In many cases this is not sufficient and then sodium-glucose transporter 2 (SGLT-2) inhibitors and immunomodulators are used. In particular, the SGLT-2 in

Immunoglobulin A nephropathy (IgAN) has been identified in patients with various malignancies. Although membranous glomerulonephritis and minimal change disease have been described in patients with mesothelioma, to our knowledge IgAN associated with mesothelioma has not been reported. We present a case of IgAN, characterized by progressive deterioration of renal function from normal and confirmed by kidney biopsy. Despite improvement of renal function following treatment with cyclophosphamide and prednisone, the patient succumbed to acute respiratory failure 8 months later. We conclude that Ig

Chronic meningococcaemia is an unusual clinical presentation of Neisseria meningitidis infection. We describe the case of a patient, who presented with total IgA deficiency and partial IgM deficiency with a low switched memory B cells count, suggestive of a borderline form of common variable immunodeficiency (CVID). The role of IgA in the protection against Neisseria meningitidis, and the link between IgA deficiency and CVID are discussed.

The mixed agglutination technique with tissue sections has been applied to psoriatic lesions using different indicator cell systems. Sections from all of twenty-five patients contained immunoglobulins of IgG and IgM classes in the epidermis. Apparently the maximum concentration was around the dermal-epidermal junction and above the dermal papillae. Anti-Ig activity consistent with rheumatoid factor of various Ig classes was also present at these sites. The anti-IgG factors were eluted from the sections by acid buffer. Such sections would again absorb anti-IgG factors whereby the reaction with

BACKGROUND: Multiple myeloma (MM) is a common malignant tumor of the hematological system caused by the malignant proliferation of plasma cells, characterized by the production of M proteins and CRAB symptoms. Among them, the Immunoglobulin G MM is the most common, while the IgG4-MM is extremely rare. Smoldering multiple myeloma refers to a state where there are no clinical symptoms. However, bone marrow plasma cell infiltration reaches 10%-59%, and previously, there were no reports of Immunoglobulin G4 smoldering multiple myeloma (IgG4 SMM) internationally.

Light chain (LC) deposition disease (LCDD) is a rare disorder characterized by glomerular and peritubular amorphous deposits of a monoclonal immunoglobulin LC, leading to nodular glomerulosclerosis and nephrotic syndrome. We developed a transgenic model using site-directed insertion of the variable domain of a pathogenic human LC gene into the mouse immunoglobulin κ locus, ensuring its production by all plasma cells (PCs). High free LC levels were achieved after backcrossing with mice presenting increased PC differentiation and no immunoglobulin heavy chain production. Our mouse model recapitu

OBJECTIVE: Pathogenesis of Behçet disease (BD) has not yet been clearly revealed and there is no ideal test for the estimation of disease activation at present. This study aimed to assess the efficiencies of IgG/IgM and IgA/C3 ratios in determining activation of BD.
METHOD: This retrospective cohort study consisted of 140 patients with BD. Patients were divided into two groups: (1) active BD (n = 89) and (2) inactive BD (n = 51) and were compared in terms of demographic features, clinical characteristics and laboratory test results. IgA/C3 and IgG/IgM ratios were compared according to organ

BACKGROUND: Cholera is a severe dehydrating illness of humans caused by toxigenic strains of Vibrio cholerae O1 or O139. Identification of immunogenic V. cholerae antigens could lead to a better understanding of protective immunity in human cholera.
METHODS: We probed microarrays containing 3652 V. cholerae antigens with plasma and antibody-in-lymphocyte supernatant (ALS, a surrogate marker of mucosal immune responses) from patients with severe cholera caused by V. cholerae O1 in Bangladesh and age-, sex-, and ABO-matched Bangladeshi controls. We validated a subset of identified antigens usin

We report a case of glomerulonephritis with monoclonal immunoglobulin (Ig) A deposits as a form of monoclonal gammopathy of renal significance (MGRS) caused by monoclonal immunoglobulins without blood disorders in a 41-year-old woman. She developed lower leg oedema and was hospitalized because of nephrotic syndrome. Serum and urine were negative for M protein, and the free light chain κ/λ ratio was within the normal range. Renal histopathological findings included mesangial proliferation, endocapillary cell proliferation, and a double-contour appearance of the capillary walls. Immunofluorescen

Complement-mediated lysis of sheep erythrocytes coated with optimal concentrations of rabbit IgG hemolysin was inhibited by euglobulin fractions from the sera of patients with seropositive rheumatoid arthritis. That this was due to direct interaction with the IgG coat on the red cell rather than a nonspecific reaction with complement in the fluid phase was confirmed by controls using cells coated with IgM hemolysin. The inhibitory activity was recovered in purified IgM rheumatoid factor preparations and could be absorbed out with insoluble aggregated human IgG. The inhibitory potency of the rh

The immunoglobulin A/polymeric immunoglobulin receptor (IgA/pIgR) transport axis orchestrates mucosal immunity, epithelial integrity, and microbiota homeostasis, and its dysregulation promotes the development and progression of inflammatory bowel disease (IBD). However, therapeutic strategies aimed at restoring this axis remain underdeveloped. In this study, we developed an innovative oral therapy utilizing hybrid extracellular vesicles to specifically modulate the IgA/pIgR transport axis for IBD treatment. Our findings demonstrate that this oral targeting strategy effectively upregulates epit

Immunoglobulin (Ig) A nephropathy (IgAN) is a known autoimmune disease due to abnormal glycosylation of IgA1, and occasionally, IgG co-deposition occurs. The prognosis of IgG co-deposition with IgAN is adverse, as shown in the previous studies. However, in the clinical setting, monoclonality of IgG co-deposition with IgAN has not been observed. We describe a case of proliferative glomerulonephritis with monoclonal IgG deposits (PGNMID) combined with IgAN in a renal allograft. A-21-year-old man developed end-stage renal failure with unknown aetiology and underwent living-donor kidney transplant
