OBJECTIVE: To study the serum Ig E levels in patients presenting with wheezing under the age of 24 months.
METHODS: The study was conducted on 38 cases and 35 controls. The children presenting with first attack of wheezing were clinically considered as bronchiolitis and those with recurrent attacks with positive history of atopy in the patient or in their first degree relatives were considered as bronchial asthma. Disodium cromoglycate (DSCG) has been shown to inhibit the release of mediators from mast cells. In the present study, the effect of DSCG on active anaphylactic reaction was studied in mice. DSCG dose-dependently inhibited the active systemic anaphylactic reaction and serum immunoglobulin (Ig)E production induced by immunization with ovalbumin, Bordetella pertussis toxin and aluminum hydroxide gel. Five patients with Wegner's granulomatosis were found to have significantly raised serum immunoglobulin E (IgE) levels. The rise in IgE was not related to the extent of clinical involvement, was not part of a generalized serum immunoglobulin rise, and was not associated with eosinophilia. Raised serum IgE may be a clue to the pathogenesis of this disease.

The authors report a rare case of conjunctival molluscum contagiosum in a 4-year-old male with hyperimmunoglobulin E syndrome and disseminated molluscum contagiosum infection. The patient presented with bilateral periorbital and right conjunctival lesions, along with lesions on the face, neck, ears, trunk, back, and genitalia. BACKGROUND: Immunoglobulin E (IgE)-mediated allergy has repeatedly been reported after solid organ transplantation, apparently affecting approximately 10% of pediatric organ transplant recipients. Interestingly, type 1 allergy has not been described in transplanted adults, suggesting a particular propensity in childhood.

A 37-year-old female with Graves' disease was reported. An abnormally high concentration of serum IgE was observed by radioimmuno-sorbent test before treatment. Patients harbouring worm infections usually have very marked elevations of serum immunoglobulin E (IgE) concentration. However, in a group of twenty ex-Far East prisoners of war with strongyloidiasis only two (10%) had raised levels due to the worm, and these were only moderately elevated. This poor IgE response may be related to the very longstanding nature of the infection (at least 30 years). The patients may have a form of immunological incompetence, or may have become immunologically tolerant to the parasite.
