Over the last decade a number of idiotypes (Id) have been identified on monoclonal IgM anti-DNA antibodies which represent germ-line genes. One idiotype family 4.6.3 derived from a human hybridoma IgML monoclonal anti-DNA antibody of normal origin is commonly expressed (approximately 85%) in the serum of patients with systemic lupus erythematosus but not in normal controls, where its level appears to fluctuate with clinical disease activity. In contrast, the IgM released by normal cultured lymphocytes in vitro expresses this idiotype family but does not bind DNA. Both idiotype 4.6.3 and anti-DNA antibodies are frequently detectable in the serum of patients with monoclonal IgM but not IgG. Therefore, it is likely that idiotype 4.6.

Anti-myosin antibodies are found in acute rheumatic fever (ARF), a sequela of group A streptococcal infection. Both in animal models and in human systemic lupus erythematosus (SLE) the occurrence of nephritogenic autoantibodies bearing dominant idiotypes has been described. In this study we investigate the relation between the induction pathway of polyclonal B cell activation and the production and glomerular deposition of nephritogenic antibodies with shared dominant idiotype(s). Polyclonal B cell activation was induced in several experimental models characterized by glomerular immune deposit formation. A specific immune response against the individual idiotypic determinants of clonal immunoglobulins can be elicited in patients with B-cell malignancies. We analyze the clinical outcome and the presence of tumor cells in the blood of 8 patients with follicular lymphoma, vaccinated with autologous idiotype protein from their lymphoma cells. AIM: Rheumatoid factor (RF) is known to be heterogeneous, and RFs detected by various methods exhibit different characteristics. In addition to interacting with the Fc region of immunoglobulin G (IgG), certain RFs are able to recognize idiotypes of antibodies. Given the important role of idiotypic interactions in regulating autoimmunity, we hypothesize that RF is involved in regulation of lymphocyte activity against autoimmune disease-inducing antigens via idiotype-anti-idiotype interactions with these lymphocytes.
