Incidence and prevention of ventral incisional hernia.

OBJECTIVE: Ventral incisional hernia is a common complication of abdominal surgery. The incidence ranges from 2% to 20% and varies greatly from one series to another. The goal of this study was to determine the incidence, risk factors, and preventive measures for ventral incisional hernia. MATERIALS AND METHODS: An analysis of the surgical literature was performed using the search engines EMBASE, Cochrane Library, and PubMed with the keywords: abdominal hernia, wound dehiscence, incisional hernia, incidence, trocar site hernia, and hernia prevention. RESULTS: The overall incidence of incisio

[Use of Permacol in complicated incisional hernia].

Polypropylene mesh repair is the gold standard for primary inguinal hernia and incisional hernia. Wound infection and small bowel fistulas are contraindications to polypropylene mesh repair. In addition, synthetic meshes are known to cause severe peritoneal adhesions and enteric fistulas if located close to the bowel. Porcine intestinal submucosa has been used successfully in experimental studies in dogs and rats to repair large abdominal wall defects. A new porcine dermal collagen graft has been used in man for groin hernia repair, incisional hernia repair and other surgical procedures withou

Controversies in laparoscopic repair of incisional hernia.

BACKGROUND: Incisional hernias can be a significant problem after open abdominal surgery. Laparoscopic incisional hernia repair (LIHR) is conceptually appealing: a large, abdominal wall re-incision with potential wound-related ill effects is avoided and an intra-peritoneal onlay mesh is expected to provide security that is equivalent to open, retro-muscular mesh repair. As such, LIHR has gained substantial popularity despite sparse, randomised clinical data to compare with conventional, open repair. AIM: To enumerate and discuss important, controversial issues in patient-selection, technique

[Incisional hernias after operative laparoscopy].

The objective of this study was to determine the possible risk factors of incisional hernias after operative laparoscopy. Complications involving the abdominal wall, particularly incisional hernias, were not expected when laparoscopic procedures were first introduced. With the increasing number of laparoscopies in the abdominal surgery, more incisional hernias were observed. Of the approximately 3,145 patients who underwent laparoscopy from march 1993 through march 2000 in the First Surgical Clinic Iassy, a total of 15 hernias occurred, which is an incidence of 0.477%. There were no acute comp

Laparoscopic incisional hernia repair in obese patients.

BACKGROUND AND OBJECTIVES: Laparoscopic incisional hernia repair is coming to the forefront as a preferred method of repair due to the advantages offered by minimally invasive techniques. To evaluate safety and feasibility of this approach in obese patients when performed by a general surgeon trained in basic laparoscopy with no prior experience in this technique, we reviewed our early experience in the first 18 patients. METHODS: All patients with incisional hernias presenting to a single surgeon from 2000 to 2002 were offered laparoscopic repair. Patients were informed about the limited exp

Breaking Bad: Spontaneous Rupture of Incisional Hernia.

Spontaneous bowel evisceration from a ruptured, long-standing abdominal wall hernia is a very rare complication with significant morbidity and mortality, usually occurring in incisional or recurrent groin hernias. In this case report, we elucidate an unexpected scenario of spontaneous incisional hernia rupture leading to bowel evisceration, detailing the clinical presentation, diagnostic workup, and surgical management. By highlighting this rare complication, we emphasise the significance of vigilance in monitoring hernia patients and the necessity of expedited surgical intervention to prevent

Uncommon case of pseudo inflammed incisional hernia.

An incisional herniaIncisional hernia after an abdominal operation develops in approximately 10% of the cases. However, incarceration of an incisional hernia is relatively low. On the other hand presence of an inflamed appendix, in one hernia sac is a rare entity which may cause mis or delayed diagnosis. This case is an example of acute appendicitis which is atypically located in an incisional hernia sac and mimick incarceration. KEY WORDS: Appendicitis, Incisional hernia, Perforation.

Incisional hernia appendicitis: A unique case report.

INTRODUCTION AND IMPORTANCE: Incisional hernia appendicitis is very rare. Very few papers have been published reporting about Pfannenstiel incision hernia with appendicitis. Here we report one such case of Pfannenstiel hernia with appendicitis. CASE PRESENTATION: A 20 years old lady presented with a tender mass in the right iliac fossa after Pfannenstiel incision. Ultra-sonography showed an incisional hernia with a 12.1 mm defect at the site of the incision. A provisional diagnosis of strangulated hernia was made. Acute appendicitis was diagnosed intra-operatively. Appendectomy followed by pr

Incarceration of transplanted kidney through incisional hernia.

Transplanted allograft kidney herniation through an incisional hernia resulting in incarceration is a rare condition with only one other similar case reported in the literature. The primary imaging modalities used to diagnose kidney herniation are graft ultrasound, abdominal computed tomography and abdominal magnetic resonance imaging [Sugi et al. (Imaging of renal transplant complications throughout the life of the allograft: comprehensive multimodality review. Radiographics 2019;39:1327-1355)]. Treatment should be based on patient's symptoms. This case report highlights the initial presentat

Stomal Closure: Strategies to Prevent Incisional Hernia.

Incisional hernias following ostomy reversal occur frequently. Incisional hernias at the site of a previous stoma closure can cause significant morbidity, impaired quality of life, lead to life-threatening hernia incarceration or strangulation and result in a significant financial burden on health care systems Despite this, the evidence base on the subject is limited. Many recognised risk factors for the development of incisional hernia following ostomy reversal are related to patient factors such as age, malignancy, diabetes, COPD, hypertension and obesity, and are not easily correctable. Open incisional hernia repair under local anaesthesia.

BACKGROUND: The Authors give an accurate description of a method of incisional hernia repair with an open technique, under local anaesthesia. The aim of this work was to show the feasibility and safety of an open preperitoneal mesh technique, as well as giving a description of technical details and the results of incisional hernia repair under local anaesthesia. METHODS: Between January 1994 and February 2008, 129 patients underwent an open mesh surgical procedure for incisional hernia. Using some simple selection criteria: maximal diameter of the hernia orifice not greater than 10 cm, the re

Laparoscopic incisional hernia repair after liver transplantation.

Incisional hernias occur in up to 17% of patients after liver transplantation. Laparoscopic ventral hernia repair is associated with fewer wound complications and a decreased incidence of recurrence when compared to open hernia repair in nontransplant patients. This is a retrospective review of 13 patients who underwent laparoscopic incisional hernia repair (LAP group) after liver transplantation compared to 14 patients who had open repairs (OP group; all but one with mesh). Primary immunosuppression in both groups at the time of transplantation was tacrolimus, but more patients in the LAP gro

Light weight meshes in incisional hernia repair.

Incisional hernias remain one of the most common surgical complications with a long-term incidence of 10-20%. Increasing evidence of impaired wound healing in these patients supports routine use of an open prefascial, retromuscular mesh repair. Basic pathophysiologic principles dictate that for a successful long-term outcome and prevention of recurrence, a wide overlap underneath healthy tissue is required. Particularly in the neighborhood of osseous structures, only retromuscular placement allows sufficient subduction of the mesh by healthy tissue of at least 5 cm in all directions. Preparati

An unusual form of incisional hernia: A case report of Littre's hernia.

INTRODUCTION: Littre's hernia (LH) is a rare condition involving Meckel's diverticulum within a hernia sac, with an incidence of <0.1 % of all complicated hernias. To this day, only a few case reports have been published concerning this entity. CASE PRESENTATION: A 48-year-old patient with a history of four midline C sections and an incarcerated incisional midline hernia was admitted to the emergency department for small bowel obstruction evolving for 12 h. Examination revealed an incarcerated midline incisional hernia. An abdominal CT scan was performed, showing a multi-orifice incisional he

Acute appendicitis within an incisional hernia: a type of Amyand's hernia.

A man in his 60s presented to the emergency department with generalised abdominal pain and distention associated with the inability to pass stool or gas. The patient had undergone a laparoscopic partial right nephrectomy due to renal cell carcinoma a year ago. The workup diagnosis suggested an incarcerated incisional hernia. Surgical treatment was proposed. Intraoperatively, we found an ileocecal appendix with a macroscopically gangrenous appearance inside the hernia sac. An appendectomy was performed, and the hernia defect was closed using a tension repair. Amyand's hernia, a rare condition,

[Planned Ventral Hernia After Open Abdomen Therapy: Complex Incisional Hernia Repair].

A planned ventral hernia after open abdomen therapy is a rare hernia entity because the fascial closure rate has been increased due to established concepts for open abdominal treatment. Nevertheless, fascial closure is not always successful, and a planned ventral hernia has to be treated later. Preoperative optimisation and planning are essential for such challenging abdominal wall repairs. In a single centre retrospective analysis, all incisional hernias from 2013 to 2023 (n = 632) were identified and planned hernias after a laparostomy were selected (n = 11). The data on surgical management w

EARLY INCISIONAL HERNIA AFTER LIVER TRANSPLANTATION: RISK FACTORS AND HERNIA REPAIR RESULTS.

BACKGROUND: Liver transplantation is a complex and valuable therapy. However, complications that burden postoperative quality of life, such as incisional hernia, are to be better elucidated, such as risk factors and prophylactic measures. AIM: This study aimed to define the rate of incisional hernia in patients who underwent liver transplantation in a population in southern Brazil and to assess the related risk factors in order to establish measures for prior optimization and specific prophylactic care in the future. METHODS: Patients undergoing adult Liver transplantation from January 2004

PD catheter malfunction due to incisional hernia entrapment.

A 69-year-old male with end-stage kidney disease secondary to ischemic cardiomyopathy and peripheral arterial disease presented with early peritoneal dialysis (PD) catheter dysfunction, 14 weeks after laparoscopic insertion of a Di Paolo catheter. The patient experienced slow and incomplete effluent drainage (800 mL out of 1000 mL instilled). Clinical examination revealed a reducible, non-painful right incisional hernia at the site of a prior aortofemoral vascular bypass. Abdominal imaging, including X-ray and CT scan, identified catheter malposition in the right iliac fossa, with the catheter
