Examining the effectiveness of dissuasive taxes as a policy tool for reducing tobacco and alcohol consumption in Cameroon: A welfare and microsimulation analysis.

Deterrent taxes are a crucial policy tool for reducing the consumption of harmful products like tobacco and alcohol. However, assessing dissuasive taxes impact different income groups is important to ensure that their burden is not disproportionately borne by low-income households. This study examines the effectiveness of deterrent taxes as an economic policy tool for reducing tobacco and alcohol consumption in Cameroon. We analyse the impact on household welfare and distributional effects using microsimulation analysis. The data come from the Cameroon Household Living Conditions Survey and th

Estimated health impact, cost, and cost-effectiveness of taxation on unhealthy packaged foods in the Philippines: a modelling study.

BACKGROUND: In 2018, the Philippines implemented a tax on sugar-sweetened beverages. A broader tax on unhealthy foods is being considered. We aimed to estimate the effect of a tax on unhealthy packaged foods. METHODS: In this modelling study, we used a multiple-cohort, proportional multistate life table model to estimate the effect of a 20% tax on packaged foods exceeding WHO thresholds for sodium or sugar, excluding beverages already taxed. Using nationally representative nutrition, sales, and Global Burden of Diseases, Injuries, and Risk Factors Study data, we projected changes in sodium an

Impact of cigarette tax increase on health and financing outcomes in four Indian states.

Background: In India, about one million deaths occur every year due to smoking. Tobacco taxation is the most effective intervention in reducing smoking. In this paper, we examine the impact of a one-time large cigarette price increase, through an increase in excise tax, on health and financing outcomes in four Indian states. Methods: We used extended cost-effectiveness analysis to estimate, across income quintiles, the life-years gained, treatment cost averted, number of men avoiding catastrophic health expenditures and extreme poverty, additional tax revenue collected, and savings to the Ayus

Retained Earnings or External Financing in Tax Systems with Deduction for Notional Interest on Equity

Abstract This paper analyzes the effects of financing new investment projects with retained earnings, newly raised equity, or debt under corporate taxation and personal income taxation of dividends, interest income, and realized capital gains. The analysis is based on a stylized model that extends the classical tax system by introducing a deduction for notional interest on equity – either at the corporate level (Allowance for Corporate Equity, ACE) or at the shareholder level (Shareholder Income Tax, SIT). We show that financing neutrality can be achieved under both tax systems, but only under

Heterogeneous disability shocks and the dynamics of income, employment, and partial insurance.

This study examines the long-run economic consequences of disability, distinguishing conditions by the activities they impair. Using linked Canadian survey and administrative tax data, I estimate the effects of disability onset on detailed income components over a ten-year horizon and assess gaps in partial income insurance across disability types. Mental-cognitive disabilities lead to larger and more persistent losses in market income than physical disabilities. Despite this, both groups experience similar levels of partial insurance and comparable declines in after-tax income. Importantly, s

The financial impact of unrepaired cleft lip and palate in the Philippines.

OBJECTIVES: There are over 96 million people in the Philippines. Close to 77 thousand persons in this country have orofacial clefting. We estimate nearly 64 thousand are unrepaired. Unemployment and underemployment because of the communication disorders associated with unrepaired clefts will affect family income and hence tax revenues. The purpose of this study is to understand the financial impact of unrepaired cleft lip and/or palate on families and identify how that would translate to the Philippine government in tax revenues. METHODS: The incidence of orofacial cleft in the Philippines wa

Fat vs. Sugar: The Case for a Saturated Fat Tax in Italy.

When judging the distributional impact of unhealthy food taxes, what matters is not just how much low income people would pay but how much the such taxes would benefit or harm them overall. In this paper, we assess the consumer welfare impact of a fat tax net of its expected benefits computed as savings from weight loss. Using Italian data, we estimate a censored Exact Affine Stone Index (EASI) incomplete demand system for food groups, simulating changes in purchases, calorie intake, consumer welfare, and the monetary value of short-run health benefits. While the Italian government has propose

Long-term impact of unhealthy food tax on consumption and the drivers behind: A longitudinal study in Hungary.

Several countries have introduced public health product taxes with the objective of reducing the absolute amount of consumption of unhealthy food and tackling obesity. This study aims to estimate the long-term impact of the Hungarian public health product tax introduced in 2011. To achieve this, a unique consumer purchase dataset was analysed to examine daily fast-moving consumer goods purchases from a representative sample of 2,000 households from 2010 to 2018. The results indicate that the tax has been fully reflected in consumer prices. A decline in consumption was observed initially, consi

Transplantation and Employment Earnings in Kidney Transplant Recipients.

IMPORTANCE: Kidney transplantation improves survival and quality of life for individuals with end-stage kidney disease (ESKD). However, its effect on objective economic outcomes, such as employment income, remains insufficiently studied. OBJECTIVE: To quantify changes in employment income before and after kidney transplantation in Canada using linked administrative health and tax data. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study included nationwide data from Canadian adults receiving a kidney transplant between 2007 and 2016. Data were from the Canadian Hospitalizat

Differential impacts of cigarette tax increases on health equity in the United States.

OBJECTIVES: Our study investigates the health equity implications of higher cigarette taxes by examining the differential responses among various sociodemographic groups in the United States. STUDY DESIGN: Repeated observational study. METHODS: This study utilizes a nationally representative, cross-sectional dataset (n = 1,236,126) from the Tobacco Use Supplement to the Current Population Study (1992-2019). We employ a logistic regression model with two-way fixed effects analysis to examine the response of smoking participation to combined federal and state cigarette taxes, stratified by soc

Equity in healthcare financing: A review of evidence.

This review summarises empirical studies on the progressivity and redistributive effects of healthcare financing mechanisms. The evidence varies significantly across countries and financing sources. Tax-based systems exhibit high progressivity, as direct taxes contribute to a favourable redistribution toward low-income households, often offsetting the regressive nature of indirect taxes. Social insurance systems are found to be progressive but may be regressive in practice due to contribution ceilings and exemptions for high-income earners. This creates disparities where high-income taxpayers

Differential price responses for tobacco consumption: implications for tax incidence.

Increasing tobacco taxes is considered the most effective an cost-effective policy to reduce tobacco consumption. However, a common objection to tobacco taxes is that they tend to rely disproportionately on the poorest individuals since less affluent smokers incur proportionately greater expenditures on cigarettes compared with more affluent smokers. Such objections usually assume that all smokers throughout the income distribution react similarly to an increase in tobacco prices. But, if less affluent smokers are more sensitive to price changes (ie, they have a higher demand price elasticity),

Methods of generating state revenue for breast cancer prevention and early detection: an analysis of pink dollars.

OBJECTIVE: We assessed income tax check-offs, specialty license plates, and lottery tickets as ways states raise funds for the early detection and prevention of breast cancer. METHODS: We conducted an inventory of state legislation allowing these revenue-generating methods, and collected and compared information on each initiative. We conducted logistic regression analyses to compare these methods with state breast cancer mortality rates. RESULTS: Eighteen states had programs for contributions through an income tax check-off. Revenue for tax check-offs and plates is influenced by state popul

Does It Really Reduce Obesity? Substitution Effects of Sugar-Sweetened Beverage Tax-Empirical Evidence From China.

BACKGROUND: In China, the increasing prevalence of obesity has become a significant public health challenge, prompting the government to explore various intervention measures, including tax policies. Although the Healthy China 2030 plan emphasizes the importance of promoting health, a tax on sugar-sweetened beverages (SSBs) has not yet been implemented in China, and there is a significant lack of localized empirical research on its actual effects in the country. This study aimed to fill this gap by exploring the potential impact of an SSB tax within China's specific socioeconomic and cultural

(No) time to be healthy: Optimal policy with time and monetary investments in health.

Time is often an important ingredient of a healthy lifestyle. Starting from this observation, this paper studies optimal public policy concerned with promoting healthy choices taking into account both monetary and time investments in health. Individuals tend to underestimate the importance of their health while making decisions, which makes their investments too low compared to the first-best. While the decentralization of the first-best requires personalized subsidies on both time and monetary investments, this may be problematic due to informational issues. In a second-best world where indiv

The effect of tobacco tax increase on price-minimizing tobacco purchasing behaviours: A systematic review and meta-analysis.

BACKGROUND AND AIMS: Tobacco product excise taxes are a cost-effective method for reducing tobacco consumption, but industry pricing and marketing strategies encourage consumers to engage in price-minimizing behaviours (PMBs). We investigated the relationship between tobacco tax increases and PMBs, measuring whether PMBs intensify following tax increases, whether low-income consumers with higher nicotine dependence are more likely to engage in PMBs and whether PMBs are negatively related to smoking cessation. METHOD: This was a systematic review with meta-analysis of cross-sectional and longi

Take-up of the 2021 child tax credit expansion among disadvantaged households in California: Continuing barriers to health equity.

BACKGROUND: In 2021 Congress expanded the Child Tax Credit (CTC), one of the largest US economic supports for working families with proven benefits for health. Information on CTC take-up among eligible families is lacking. Understanding barriers to anti-poverty programs is an urgent policy issue, as low take-up is an established barrier to health equity. METHODS: Among a longitudinal cohort of primarily Latino and Black California families with low income (N = 380), we used data from individuals' 2019 and 2021 tax returns to compare CTC take-up (i.e., receipt among eligible individuals) pre-

Safety Net Program Participation Patterns, Sociodemographic Factors, and Health.

INTRODUCTION: U.S. safety net programs provide critical support to Americans with low income. This cohort study examined patterns of safety net program take-up over time and associations with sociodemographics and health. METHODS: Surveys among California households with low income (N=380) conducted in 2020-2021 and 2023 captured take-up of federal assistance programs (Medicaid; Supplemental Nutrition Assistance Program [SNAP]; Special Supplemental Nutrition Program for Women, Infants, and Children; Earned Income Tax Credit; Child Tax Credit) in 2019 and 2021 and health in 2023. Do alcohol taxes improve traffic safety? Specific alcohol excise taxes and drunk-driving crashes in Türkiye.

INTRODUCTION: Alcohol taxation is a widely used public policy tool to reduce alcohol-related harm. However, studies directly examining the impact of alcohol taxes on alcohol-related crashes remain limited, particularly in middle-income countries. This study investigates how changes in specific alcohol excise taxes in Türkiye affect the number of drunk drivers involved in traffic crashes. METHOD: For this purpose, the study uses a monthly province-level panel dataset covering 81 provinces and 10,692 observations between 2013 and 2023. Cigarette taxation and neonatal and infant mortality: A longitudinal analysis of 159 countries.

Previous studies on the associations between cigarette taxes and infant survival have all been in high-income countries and did not examine the relative benefits of different taxation levels and structures. We evaluated longitudinal associations of cigarette taxes with neonatal and infant mortality globally. We applied country-level panel regressions using 2008-2018 annual mortality and biennial WHO tobacco taxation data. Complete data was available for 159 countries. Outcomes were neonatal and infant mortality. We conducted analyses by type of taxes (i.e. specific cigarette taxes, ad valorem
