Primary care of the premature infant discharged from the neonatal intensive care unit.

Premature infants require varying degrees of newborn intensive care and have a wide range of physical and developmental outcomes. Subsequent ambulatory care for these infants is often complex. Although tertiary hospitals often provide multidisciplinary follow-up clinics, the pediatric nurse practitioner (PNP) responsible for primary care has a unique opportunity to influence the lives of these special babies and their families. While the basic principles of well child care and health maintenance apply to this special population, there are several inherent challenges. The transition from neonat

Risk, antepartum care, and outcome: impact of a maternity and infant care project.

A Title V Maternity and Infant Care (M&I) Project has been operating from Cleveland Metropolitan General Hospital for more than 13 years. To evaluate the effectiveness of its program of antepartum care, social and medical-obstetric risk factors and perinatal outcome were evaluated for pregnant women delivered during a 2-year period. After the potential biases of patient referral and failure to obtain antepartum care were excluded, patients who received either M&I or non-M&I care were compared. Despite the similar social and antepartum/intrapartum risk of these groups, the M&I patients experien

Care of the COVID-19 exposed complex newborn infant.

As we confront COVID-19, the global public health emergency of our times, new knowledge is emerging that, combined with information from prior epidemics, can provide insights on how to manage this threat in specific patient populations. Severe Acute Respiratory Syndrome (SARS) and Middle East Respiratory Syndrome (MERS), both caused by coronaviruses, caused serious respiratory illness in pregnant women that resulted in adverse perinatal outcomes. Thus far, COVID-19 appears to follow a mild course in the vast majority of pregnant women. A significant proportion of pregnant women appear to be as

The clinical care of the HIV-1-infected infant.

Despite well-established strategies to decrease the mother-to-child transmission of HIV-1, new perinatal infections continue to occur globally, reflecting marked disparities in access to health care. Once HIV-1 infection has been established in an infant, the combination of early initiation of antiretroviral therapy and prophylaxis against Pneumocystis jiroveci pneumonia is paramount to reducing disease progression. This article reviews the recommendations and evidence for the treatment of HIV-1-infected infants.

Non-infant adoption from care: lessons for safeguarding children.

AIM: To explore use of adoption in remedying abuse and neglect, to inform child protection practice and to identify professional responsibilities to adoptive families. METHOD: A cohort of 130 children was identified for whom adoption was recommended in 1991-1996 at a mean age of 5.7 years (range 3-11). All were in local authority care for child protection reasons. BACKGROUND: information was gathered from social work records. The children were traced between 6 and 11 years later and their adopters interviewed. The Strengths and Difficulties Questionnaire and the Parent-Child Communication Sc

Infant care practices and the investigation of physiological mechanisms.

It is strange that some aspects of infant care have been strongly promoted by modern medicine while others have been neglected. Thus prone sleeping which has been strongly promoted is now related to an increase in SIDS, whereas the promotion of breast feeding in developed countries has been less successful. Unfortunately there has not been sufficient physiological investigation of many infant care practices and some of the proposed mechanisms for SIDS and prone sleeping have not been substantiated. Thus further work is needed on hypercapnia, hypothermia and periodic breathing and respiratory c

Nursing care of an osteogenesis imperfecta infant and child.

Because the child with osteogenesis imperfecta is hospitalized in the same setting many times throughout his childhood, the nursing staff become well acquainted with him and his family, and are able to assess the child's and parents' understanding of this chronic debilitating disease. The staff can help them to develop a realistic attitude toward their life and future plans. The family should be encouraged to see that the OI child attends a regular school whenever possible so that their active, excellent minds can develop. The family and child should be assured that an OI child can be educated

Extremely premature infant care: Reasonable progress or therapeutic fury?

Application of palliative comfort care versus neonatal intensive care for extremely premature births is a judgment fraught with uncertainty and risk, principally for the pregnant woman and family. Survival and good health are possible, but suffering, chronic health issues, and socio-economic inequities are burdensome to families. Survival rates of infants born at 22, 23, and 24 weeks' gestation are increasing, but morbidity rates and long-term neurodevelopmental impairments are substantial and not improving. Outcomes acceptable to some pregnant women and families are not acceptable to others.

Neural circuit plasticity transforms infant neglect into maternal care.

Infants in distress evoke strong emotional responses in adults, which help to elicit caretaking behaviors from parents to ensure infant needs are met1-3. However, neonatal care can also be challenging, and interactions with infants can lead to stress and negative affect even in potential caregivers4-7. Child neglect and maltreatment rates in human populations make it important to understand the neural mechanisms of regulating negative emotions and stress in the parental brain8-10. Here we show how rapid plasticity in female mouse central amygdala (CeA) transforms infant aversion into attentive

Surfactant replacement therapy. Innovative care for the premature infant.

Prematurity is one of the most perplexing problems in perinatal care. Since the discovery of a surfactant deficiency as the primary etiology of hyaline membrane disease, attempts have been made to supplement surfactant in the premature infant. Surfactant replacement therapy is a promising new treatment for the premature infant with hyaline membrane disease. The history of surfactant replacement therapy, current trials of this therapy, nursing care of the infant receiving surfactant replacement, and future directions for nursing research in this area are discussed. Infants receiving this therap

Infant care and feeding in an urban black population.

Health education programmes aimed at improving infant feeding must be developed from data on current practices and growth. Infants born in Pimville, Soweto, were traced when they were 6 weeks old and again at 6 months and their mothers and child-minders were interviewed. Length of tay in the area, breastfeeding and weaning intention and practice, age standard of education, understanding of Boston growth charts, and infant growth data were investigated. It is suggested that more specific health education programmes need to be developed and that community-based health workers need to be selected

Postneonatal infant mortality in infants to a neonatal intensive care unit.

The postneonatal infant mortality (PNIM) of 2,205 infants admitted to a neonatal intensive care unit from January 1971 to December 1974 was 44 in 1,000 infants who survived to age 28 days. This rate is approximately ten times that of the general population. Congenital malformations (59%), infections (12%), sudden infant death syndrome (10%), and asphyxial brain damage (10%) were the most common causes of death. One third (26) of the infants remained in the hospital whereas two thirds (52) had been dismissed prior to death. All who remained in the hospital plus 36 who had been dismissed died of

Kernicterus in late preterm infants cared for as term healthy infants.

OBJECTIVE: To compare the clinical profile and health care experiences related to management of newborn jaundice and hyperbilirubinemia in preterm infants (<37(0/7) weeks gestation) who are cared for as term infants (> or =37(0/7) weeks) and develop acute and/or chronic posticteric sequelae. METHODS: Retrospective study of a convenient sample of term and near term infants voluntarily reported to the Pilot Kernicterus Registry (1992-2003). Study infants were required to meet the clinical definitions for acute bilirubin encephalopathy (moderate or advanced severity) and/or the classical signs o

Maternity Care Deserts: Disparities in Maternal and Infant Access to Care and Potential Solutions.

Access to maternity care in the United States is becoming more disparate, leading to worsening maternal and infant outcomes in rural communities. Increases in maternal and infant morbidity and mortality are especially prominent in maternity care deserts (MCDs). These MCDs are counties that lack obstetric-specific clinicians such as obstetricians, family physicians, or certified nurse midwives and also lack birth centers or hospitals with obstetric units. This has led to an increase in out-of-hospital births, deliveries in emergency departments, or births at hospitals that are not equipped to m
