Mechanism of infarction involving ipsilateral carotid and posterior cerebral artery territories.

BACKGROUND: We investigated the potential mechanism of infarction involving the territories of both the internal carotid artery (ICA) and the ipsilateral posterior cerebral artery (PCA). METHODS: Among consecutive patients with an ischemic stroke who had undergone both diffusion-weighted magnetic resonance imaging (DWI) and cerebral angiography, those who were found to have acute lesions in the ipsilateral ICA and PCA territories on DWI were selected for this study. The mechanism of infarction was sought by investigating angiographic findings and DWI lesion patterns. The frequency of patency

Capsular involvement and sensorimotor stroke with posterior cerebral artery territory infarction.

The traditional concept of a clear demarcation between the vascular supply of the internal capsule and the thalamus cannot be regarded as absolute. Two cases of sensorimotor stroke due to posterior cerebral artery territory infarction are described, with CT evidence of internal capsular involvement.

A systematic study of topographical memory and posterior cerebral artery infarctions.

OBJECTIVE: To estimate the prevalence of topographical memory impairment following posterior cerebral artery infarctions (PCAI) and define its anatomical correlations. METHODS: We recruited 15 patients (mean duration of 4 months postinfarct). We administered 2 sets of experimental tests to assess topographical memory: one set included 5 computerized tasks (CompT) and the other set consisted of one ecological topographical orientation test (EcolT) that included 4 tasks (i.e., map drawing, picture recognition and ordering, backward path). Fifteen healthy participants served as controls. Patient

[Posterior cerebral artery territory infarcts due to carotid dissection].

INTRODUCTION: About 20% of strokes in individuals under 45 years of age are due to cervicocephalic artery dissection. The magnetic resonance should be the first option in the diagnostic process when this etiology is suspected. Posterior fetal circulation is a variant in the embryogenic development that may cause posterior territory infarcts in carotid dissections or stenosis. CASE REPORT: We report the case of a 51 year-old male patient, with left occipitotemporal acute infarct and occlusion of the internal carotid and left posterior cerebral arteries as well as a probable carotid origin of t

Posterior cerebral artery territory infarct presenting as acute psychosis.

An 82-year-old man with a past medical history of hypertension was admitted to a psychiatric hospital for sudden onset of acute psychosis. He was then transferred to an acute geriatric unit for further evaluation. During the admission the patient was noted to be very restless, agitated and noisy and was shouting and screaming incessantly. This was interspersed with occasional short periods of calm and quiet. Clinically, no obvious focal neurological deficits were detected. A CT scan of the brain was performed and it revealed an acute infarct involving the area supplied by the left posterior ce

Differing mechanisms between posterior and middle cerebral artery infarctions.

Cardiac embolism has been thought to be one of the principal causes of posterior cerebral artery territory infarction. To determine stroke mechanisms and stroke risk factors in patients with posterior cerebral artery infarction, we studied 23 consecutive patients with recent infarcts in the posterior cerebral artery distribution (PCA infarcts) and compared these with a case-control group of 46 patients with recent infarcts in the middle cerebral artery distribution (MCA infarcts). All patients were similarly studied, including angiography and echocardiography. Case controls were age- and sex-m

[Amnestic syndromes and confabulation in infarction of the posterior cerebral artery area].

The Posterior Cerebral Artery (PCA) supplies the greatest part of the limbic system. Several authors have reported amnestic syndromes in PCA infarcts. Amongst 76 patients with a CT proven PCA infarct, 21 suffered anterograde amnesia. Confabulations were associated in 5 cases. We studied CT scans according to previously reported templates in the 21 amnestic patients: all patients with confabulations had a paramedian or a tuberothalamic infarct associated with a cortical infarct within the superficial territory of the PCA. None of the 16 remainders had a thalamic lesion. These results emphasize

Bilateral thalamic infarction with posterior cerebral artery variant (the arcade artery): A case report.

RATIONALE: Bilateral thalamic infarction is a rare type of posterior circulation stroke, and it often presents with a reduced level of consciousness in the elderly. Arteriosclerosis is the primary etiology of bilateral thalamic infarction, including conditions such as native vessel stenosis or arterial-to-arterial embolism. Cardiogenic or paradoxical embolism can also lead to thrombosis of the perforator branches innervating the thalamus, and these emboli tend to disintegrate and lead to multiple lesions, even in elderly patients. PATIENTS CONCERNS: A 69-year-old man presented to our emergenc

Lateral Thalamic Infarction and the Vascular Geometry of the Posterior Cerebral Artery.

BACKGROUND: The geometric properties of the parental artery affect the development of local atherosclerosis and perforator infarction. In this study, we aimed at investigating the association between vascular geometry of the posterior cerebral artery (PCA) and the development of isolated lateral thalamic infarction (LTI), the most frequent type of thalamic infarction. METHODS: The geometric properties of the corresponding PCA in LTI patients were assessed and they include the diameters of the distal basilar artery (BA) and proximal PCA, distal BA - PCA angle, first PCA angle (angle between P1

Punding following posterior cerebral artery infarction: a case report and literature review.

INTRODUCTION: Punding is a complex stereotyped behavior, characterized by excessiveness, non-goal orientation, and repetitiveness. It is mostly associated with Parkinson's disease, and very few cases in non-Parkinson's disease have been reported. We report a case of punding associated with supratentorial ischemic stroke. CASE PRESENTATION: We present a 70-year-old man with left posterior cerebral artery infarction with quetiapine-induced punding manifesting as repetitive unidirectional body turning. Remission of punding behavior ensued after cessation of quetiapine and administration of clona

Atypical Unilateral Posterior Reversible Encephalopathy Syndrome Mimicking a Middle Cerebral Artery Infarction.

Posterior reversible encephalopathy syndrome (PRES) is usually a reversible clinical and radiological entity associated with typical features on brain MR or CT imaging. However, the not-so-uncommon atypical radiological presentations of the condition are also present and they may go unrecognised as they are confused with other conditions. Here, we report a very rare case of atypical, unilateral PRES in a 49-year-old uremic, post-transplant female patient who presented with seizures. Initial MRI showed high-grade occlusion of the left middle cerebral artery (MCA) and lesions suggestive of subac

Anatomy of sensory findings in patients with posterior cerebral artery territory infarction.

BACKGROUND: Posterior cerebral arteries (PCAs) supply the ventrolateral thalamic sensory nuclei and white matter sensory tracts to the somatosensory parietal cortex. Patients with PCA territory strokes often have visual, memory, cognitive, and sensory signs. Clinicoanatomic correlation of visual, cognitive, and memory functions are well defined but, to our knowledge, no systematic study has analyzed the anatomy of sensory abnormalities. OBJECTIVE: To assess the frequency and anatomic correlation of sensory symptoms and signs in patients with PCA territory infarction. PATIENTS AND METHODS: Si

Fetal posterior cerebral artery infarction due to carotid dissection: a case report.

BACKGROUND: Fetal posterior cerebral artery is an anatomical variant that occurs in 23-29% of the population. In this variant, posterior circulation, especially to the occipital lobe and mesencephalon, is provided by the posterior communicating artery. Whether fetal posterior cerebral artery constitutes a risk factor for ischemic stroke is still controversial. Our aim was to describe an exceptional case of ischemic stroke due to internal carotid artery dissection in a patient with fetal posterior cerebral artery anatomical variant. CASE PRESENTATION: A 46-year-old North African male patient p

Pure superficial posterior cerebral artery territory infarction in The Lausanne Stroke Registry.

OBJECTIVE: To determine the patterns of clinical presentation, lesion topography, and etiology in patients with ischemic stroke limited to the superficial territory of the posterior cerebral artery (s-PCA). METHODS: In the Lausanne Stroke Registry (LSR, 1983-1998), we determined the patterns of clinical presentation, lesion topography and mechanisms of stroke, among 117 patients with s-PCA infarction (s-PCAI) on brain imaging. RESULTS: s-PCAIs accounted for 30.5 % of all PCA territory ischemic strokes. The presumed etiology was embolism in 64 (54.5 %) patients [cardiac in 51 (43.5 %) and art

Fetal-Type Variants of the Posterior Cerebral Artery and Concurrent Infarction in the Major Arterial Territories of the Cerebral Hemisphere.

Fetal-type or fetal posterior cerebral artery (FPCA) is a variant of cerebrovascular anatomy in which the distal posterior cerebral artery (PCA) territory is perfused by a branch of the internal carotid artery (ICA). In the presence of FPCA, thromboembolism in the anterior circulation may result in paradoxical PCA territory infarction with or without concomitant infarction in the territories of the middle (MCA) or the anterior (ACA) cerebral artery. We describe 2 cases of FPCA and concurrent acute infarction in the PCA and ICA territories-right PCA and MCA in Patient 1 and left PCA, MCA, and A

Bálint Syndrome From Posterior Cerebral Artery Infarctions Case Report.

OBJECTIVES: Bálint syndrome is a debilitating, rare neurological condition characterized by a triad of visuospatial distortions: simultanagnosia, optic ataxia, and oculomotor apraxia. We highlight the pathways involved with infarction of the posterior cerebral artery (PCA), which may be an important underlying factor leading to the development of clinical presentations found in Bálint syndrome. METHODS: We present a case of a middle-aged patient who presents with Bálint syndrome following a PCA infarction. RESULTS: A 49-year-old male bus driver with a recent PCA infarction presented with all

[Topographic disorientation associated with infarction in the territory of the right posterior cerebral artery].

INTRODUCTION: Topographic disorientation is defined as the difficulty to find one s way in familiar surroundings. It can be due to an amnesic or agnostic like defect, which is known as topographic amnesia or agnosia. This disorder can give rise to disability in the life of the patient and may well go undetected unless a suitable neuropsychological study is conducted. CASE REPORTS: We report the cases of two patients with infarction in the territory of the right posterior cerebral artery who began with hemianopsia and topographic disorientation. One of the cases was due to a disorder affecting
