Clinical and economic outcomes of a prospective antimicrobial stewardship program.

PURPOSE: A pre-post analysis of an antimicrobial stewardship program (ASP) involving the use of data-mining software to prospectively identify cases for ASP intervention was conducted. METHODS: The investigators evaluated clinical outcomes and cost metrics before and after implementation of the ASP, which entailed daily physician review of summary reports on all adult inpatients receiving antimicrobial therapy. The primary outcome measures were annual antimicrobial expenditures and rates of infections due to common nosocomial pathogens; secondary outcome measures included patient survival and

Agreement in classifying bloodstream infections among multiple reviewers conducting surveillance.

BACKGROUND: Mandatory reporting of healthcare-associated infections (HAIs) is increasing. Evidence for agreement among different reviewers applying HAI surveillance criteria is limited. We aim to characterize agreement among infection preventionists (IPs) conducting surveillance for central line-associated bloodstream infection (CLABSI) with each other and as compared with simplified laboratory-based definitions. METHODS: Abstracted electronic health records were assembled from inpatients with positive blood cultures at a tertiary-care Veterans Affairs (VA) hospital over a 5-year period. Iden

Report of working group 3: specialist training and continuing medical education/professional development in the infection disciplines.

The European Union of Medical Specialists (UEMS) core curricula for training in infectious diseases and medical microbiology are adequate with the exception of one deficiency which is the absence of training in epidemiology, public health and infection control. Infectious disease curricula should include training in HIV, tuberculosis, hepatitis and sexually transmitted diseases. There is a need for a core curriculum in infection control. Infection control should have a basis in both medical microbiology and infectious diseases, and should become a specialty dealing with healthcare hygiene in h

Precautions in dentistry against the outbreak of corona virus disease 2019.

The outbreak of Coronavirus Disease 2019 (COVID-19) has become a severe global acute respiratory pandemic around the world in just a few months with an increasing number of infections and deaths. COVID-19 is a highly contagious and fatal disease. Almost everyone in the population is susceptible, and the incubation period is 1-14 days, mostly 3-7 days. The clinical symptoms of the COVID-19 are fever, dry cough and fatigue. Some patients are accompanied by symptoms such as nasal congestion, runny nose, sore throat, myalgia and diarrhea. Severe patients could even develop acute respiratory distre

Skin problems associated with routine wearing of protective gloves in dental practice.

Guidelines on cross-infection control recommend the wearing of operating gloves by dental practitioners whilst carrying out routine examinations and treatment on all patients. However, there are a number of dermatological problems that may be associated with the routine wearing of protective gloves in dental practice. This paper describes the aetiology and clinical features of three types of contact dermatitis, ie irritant contact dermatitis, allergic contact dermatitis and contact urticaria. The management of these skin conditions is discussed. Advice is given concerning routine handcare for

Maintaining women's oral health.

Women must adopt health-promoting strategies for both general health and the oral cavity, because the health of a woman's body and oral cavity are bidirectional. For general health-maintenance strategies, dental practitioners should actively advise women to minimize alcohol use, abstain from or cease smoking, stay physically active, and choose the right foods to nourish both the body and mind. For oral health-maintenance strategies, dental practitioners should advise women on how to prevent or control oral infections, particularly dental caries and periodontal diseases. Specifically, women nee

Why do infection preventionists leave a job? A qualitative evaluation of infection preventionist attrition in health care.

BACKGROUND: Infection preventionists' (IPs) importance has increased over the years for various reasons. IP attrition results in risks to both patients and staff. The goal of this project was to determine the drivers of IP attrition and retention through focus groups. METHODS: Focus groups constituted IPs who have been in the same role for > 5years (Tenured IPs), IPs who have left one IP role for another IP role in the last 3 years (Transitional IPs), IPs who have left the field of infection prevention (Former IPs), and IP leaders discussed drivers of attrition and retention. Preventive control of AIDS by the dental profession: a survey of practices in a large urban area.

The purpose of this study was threefold: (1) to report the proportion of dental practitioners adhering to the 1987 Centers for Disease Control (CDC) procedures for using infection control techniques (ICTs); (2) to identify attitudes toward infection control and disease; and (3) to establish whether certain practitioner characteristics or use of certain ICTs were related to willingness to treat HIV-positive patients, willingness to volunteer for an HIV specialty clinic outside of regular practice, vaccination against hepatitis B, and a felt need for a specialty clinic within the practice to tre

Surveillance of hospital water and primary prevention of nosocomial legionellosis: what is the evidence?

Hospital-acquired Legionnaires' disease may be sporadic or may occur as part of an outbreak. As Legionella spp. are ubiquitous in many water systems, it is not surprising that hospital water may be colonized with Legionella pneumophila and other species. However, there is some controversy about the relationship between the presence of legionella in hospital water systems and nosocomial legionellosis. Primary prevention, i.e. measures to prevent legionella in a hospital or healthcare facility with no previous documented cases of nosocomial legionellosis, includes heightened awareness of hospita

A multidisciplinary approach to end external ventricular drain infections in the neurocritical care unit.

External ventricular drain (EVD) placement is a common practice in neurocritical care units. The EVDs are the "gold" standard and the most cost-effective treatment for intracranial hypertension and hydrocephalus caused by a variety of neurological conditions. The EVD drains excess cerebrospinal fluid in an attempt to maintain balance within the cranial vault and provides intermittent monitoring of intracranial pressure. This invasive procedure, as with any other procedures, carries an increased risk of a device-related infection that can lead to devastating consequences such as meningitis, ven

An innovative approach to training hospital-based clinicians for bioterrorist attacks.

The recent attacks of September 11, 2001, and the subsequent dissemination event of anthrax in the United States demonstrated the necessity for hospitals to initiate bioterrorism education for clinicians. Events such as the release of sarin gas into the Tokyo subway by the Aum Shinrikyo cult provided some insight into how quickly emergency medical personnel may be overwhelmed by causalities of unconventional weapons. Educational interventions to prepare hospital-based practitioners for such disasters must fit among the demands of patient care, administrative duties, and continuing education wi

HIV infection and Scottish general practice: workload and current practice.

To estimate the effect of human immunodeficiency virus (HIV) infection on general practice, a postal survey was undertaken of one in three of all principals in Scotland. Of the 834 general practitioners who responded (78% response rate), 31% were working in practices with patients known to be infected with HIV. The estimated prevalence of known HIV infection in general practice was 19 per 100,000 population, and the estimated annual consultation rate for HIV related problems (including consultations by the 'worried well') was seven per 1000 population. Both statistics showed considerable varia
