Hand infections.

Hand infections are commonly seen by orthopedic surgeons as well as emergency room and primary care physicians. Identifying the cause of the infection and initiating prompt and appropriate medical or surgical treatment can prevent substantial morbidity. The most common bacteria implicated in hand infections remain Staphylococcus aureus and Streptococcus species. Methicillin-resistant S aureus infections have become prevalent and represent a difficult problem best treated with empiric antibiotic therapy until the organism can be confirmed. Other organisms can be involved in specific situations

[Cutaneous infections].

The epidermis, the human being's primary protection against infection and aggression, can crack or split, and result in microbial bacteria penetrating it. Infections may, however, stem from other sources, such as viruses which enter the body via the respiratory tract, and then appear on the skin.

[Hantavirus infections].

Hantaviruses are enveloped single-stranded ribonucleic acid (RNA) viruses that belong to the family Bunyaviridae. They are widely distributed and maintained in populations of specific insectivores or rodent host species (mice or rats). In their natural host, hantaviruses develop persistent and asymptomatic infections with lifelong virus shedding in excreta. Transmission to humans occurs via direct contact with faeces, urine and saliva of infected rodents, in particular by inhaling virus contaminated aerosol. There are a variety of virus types that occur in different geographic areas, dependent

[Chlamydial infections].

Chlamydiae are a singular group of bacteria, with a vital cycle that is intracellular in part. These microorganisms adhere to epithelia, where they may provoke infection. There are three pathogenic species for humans: Chlamydia trachomatis, which produce infections mainly in the genital and urinary tracts, and C. psittaci and C. pneumoniae, which mostly produce respiratory infections. The last two microorganisms are now classified in a different genus, known as Chlamydophila.

[Mycoplasma infections].

Mycoplasmas are prokaryote microorganisms without cellular wall, that usually colonize the respiratory and urogenital mucosae. The pathogenic species for mankind are Mycoplasma pneumoniae, that produce respiratory infections, among them pneumonia, and M. genitalium, M. hominis and Ureaplasma urealyticum, that produce urinary and genital infections. The diagnosis of these infections is fundamentally based on serology, since the culture of these germs is very laborious. In a near future, the antigen or DNA detection molecular techniques will probably permit a much faster and reliable diagnosis.

[Intrauterine infections].

A broad variety of microorganisms are capable of causing fetal infections. Among viral agents prominent are the human cytomegaly virus (HCMV), herpes simplex virus (HSV), human immundeficiency virus (HIV), varicella, herpes zooster, rubella, parvovirus B19, measles and the hepatitis B and C viruses. Protozoa such as toxoplasma and spirocheta pallida, causing congenital syphilis are equally important. Bacterial infections are responsible for in uterus aquired listeriosis, tuberculosis, and group B streptococcus infections. Fungi including candida albicans complete the circle of infections patho

[Spinal infections].

Regardless of the compartment involved, imaging of spinal infections and other spinal inflammatory diseases must be performed by magnetic resonance imaging (MRI). This allows early detection of changes in disk space or vertebral bodies. Associated paraspinal and epidural masses are depicted. However, in spinal cord inflammation MRI has a very limited specificity and cannot usually differentiate between the wide array of possible causes of intramedullary lesions. This article covers a spectrum of inflammatory lesions of the spinal cord.

Nosocomial infections.

Nosocomial infections and antimicrobial resistance are topics that have been intensely studied in human medicine because of their significant impact on human health. In recent years, concerns have been raised that the use of antibiotics in veterinary medicine, animal husbandry, and agriculture may be contributing to the development of resistance in common bacterial species affecting human beings. Although there is inadequate proof at this time that the resistance is transmitted from animals to people, if antibiotics continue to be used indiscriminately in veterinary medicine, veterinarians may

Spinal infections.

Spinal infections can occur in a variety of clinical situations. Their presentation ranges from the infant with diskitis who is unwilling to crawl or walk to the adult who develops an infection after a spinal procedure. The most common types of spinal infections are hematogenous bacterial or fungal infections, pediatric diskitis, epidural abscess, and postoperative infections. Prompt and accurate diagnosis of spinal infections, the cornerstone of treatment, requires a high index of suspicion in at-risk patients and the appropriate evaluation to identify the organism and determine the extent of

Hepatobiliary infections.

The major hepatobiliary infections (excluding the viral hepatitides) include amebic and pyogenic liver abscess and cholangitis. Little new information has been published in the area of cholangitis during the last several years. In contrast, the clinical presentation and management of liver abscess have evolved considerably, not only in the last several years but also during a more extended period spanning the last two decades. In the United States, amebic liver abscess occurs largely in individuals from endemic areas or in those traveling to endemic areas. Recent data suggest that patients wit

Peritoneal infections.

Peritoneal dialysis related infections include infection of the catheter exit site, subcutaneous pathway, or effluent. Exit-site infections, predominately owing to Staphylococcus aureus, are defined as purulent drainage at the exit site, although erythema may be a less serious type of exit-site infection. Tunnel infections are underdiagnosed clinically, and sonography of the tunnel is useful to delineate the extent of the infection and to evaluate response to antibiotic therapy. S aureus infections occur more frequently in S aureus carriers and immunosuppressed patients and can be reduced by m

Monkeypoxvirus infections.

During and after the smallpox eradication campaign, human cases of monkeypox appeared in West and Central Africa, as isolated cases or as small epidemics. Since inter-human transmission has never or only very exceptionally been documented, monkeypox does not represent a serious threat to humans. The virus reservoir is among tree squirrels living in the tropical rain forests of Africa and humans are infected by hunting, killing and skinning these animals. However, the modernization of society lessens human contact with the virus reservoir. Since the eradication of smallpox, stocks of variola vi

Although cholangitis is the most prevalent hepatobiliary infectious process, liver abscess represents the most serious and conspicuous hepatobiliary infection. Amebic liver abscess typically occurs in individuals from endemic areas or those traveling to endemic areas. It is associated with an excellent prognosis when managed expediently with antiamebic antibiotics. Recent reports emphasize a possible association between amebic liver abscess and HIV infection. Drainage or surgery for amebic liver abscess is rarely necessary. In contrast, pyogenic liver abscess is associated with significant mor

Foodborne infections.

The role of foodborne infections in the health of the population has become of major concern recently. Numerous agents are transmitted in food and water and typically result in acute gastroenteritis, although long-term complications such as reactive arthritis (due to Salmonella, Yersinia, and Shigella organisms), Guillain-Barré syndrome (due to Campylobacter organisms), and renal failure (due to Escherichia coli) are now well recognized. The development of FoodNet to follow the epidemiology of select foodborne infections in the United States has been a major advance in recent years and is now
