[Syphilis and pregnancy].

Syphilis is a sexually transmitted disease responsible for a congenital severe infection. Congenital syphilis is complicated by fetal loss/neonatal death (50%), prematurity (25%) and major long term sequelae in other surviving children (20%). Every woman delivering a stillborn after 20WG should be tested for syphilis. Early screening is the cornerstone of prevention, and should be repeated in women at higher risk of contamination. Maternal management relies on early benzathine penicillin administration. Neonatal management relies on early diagnosis and prompt adequate penicillin therapy.

Subclinical rubella reinfection during pregnancy followed by transmission of virus to the fetus.

We report a documented case of rubella reinfection during pregnancy in a previously vaccinated woman with residual antibody titre to rubella of 15 IU/ml. The reinfection occurred following an exposure to rubella virus (contact with 6-year-old daughter with clinical rubella) between the 7th and 10th week of pregnancy which resulted in transmission of the virus to the fetus. Umbilical cord blood drawn by cordocentesis was found to be strongly positive for rubella IgM antibody. After termination of the pregnancy rubella virus was isolated in cell culture from fetal tissues.

Congenital infectious encephalopathies from the intrapartum period to postnatal life.

Congenital infections are a common but often underrecognized cause of fetal brain abnormalities, as well as fetal-neonatal morbidity and mortality, that should be considered by all healthcare professionals providing neurological care to fetuses and newborns. Maternal infection with various pathogens (cytomegalovirus, Toxoplasmosis, Rubella virus, Parvovirus B19, lymphocytic choriomeningitis virus, syphilis, Zika virus, varicella zoster virus) during pregnancy can be transmitted to the developing fetus, which can cause multisystem dysfunction and destructive or malformative central nervous syst

Congenital Arboviruses: A Review.

Congenital arboviruses represent a significant public health concern because of their ability to cross the placental barrier and adversely affect fetal development. In this review, we delineate the epidemiology, pathogenesis, clinical manifestations, and diagnostic challenges associated with key congenital arboviral infections, including Zika, dengue, chikungunya, yellow fever, Oropouche, and West Nile virus. While vertical transmission mechanisms vary among arboviruses, inflammation, placental disruption, and viral replication within fetal tissues are common pathological features. Early diagn

Oropouche Infection: Concerns for the Obstetric Patient.

IMPORTANCE: Oropouche is an emerging virus that has become a growing cause for concern after reports of vertical transmission and detrimental effects on the developing fetus. Oropouche is an arbovirus, meaning it is transmitted by arthropods, which include mosquitos, ticks, and midges (sometimes called "no-see-ums"). Specifically, Oropouche is transmitted through the Culicoides paraensis biting midge and Culex quinquefasciatus mosquito. Infection presents most commonly with fever, headache, myalgias, and arthralgia. OBJECTIVE: The purpose of this review is to summarize current knowledge regar

Mother-to-fetus transmission of cytomegalovirus. A review.

Congenital human cytomegalovirus (CMV) infection is the leading infectious cause of mental retardation, sensorineural deafness and visual impairment. It is mainly related to a primary maternal infection. The placenta should be considered the most important site of both the protection of the fetus from CMV infection and the transmission of CMV from mother to fetus. The control of the passage of CMV across the placenta probably involves a cascade of regulatory events. Roles are played by factors relating to the host immune-selective pressures, such as local cytokines and maternal CMV-specific ne

Horizontal and vertical transmission of wild-type and recombinant Helicoverpa armigera single-nucleocapsid nucleopolyhedrovirus.

Transmission plays a central role in the ecology of baculoviruses and the population dynamics of their hosts. Here, we report on the horizontal and vertical transmission dynamics of wild-type Helicoverpa armigera single-nucleocapsid nucleopolyhedrovirus (HaSNPV-WT) and a genetically modified variant (HaSNPV-AaIT) with enhanced speed of action through the expression of an insect-selective scorpion toxin (AaIT). In caged field plots, horizontal transmission of both HaSNPV variants was greatest when inoculated 3rd instar larvae were used as infectors, transmission was intermediate with 2nd instar

Varicella in the fetus and newborn.

Varicella (chickenpox) in pregnancy is unusual because most women of childbearing age are immune. It can, however, cause significant morbidity for the pregnant woman and in rare cases cause congenital varicella syndrome. The incidence of congenital varicella syndrome after maternal varicella during the first two trimesters is <1% across multiple cohort studies. Maternal infection in the third trimester is not associated with congenital varicella syndrome, but the infant may develop herpes zoster during the first one or two years. Maternal infection just before or after delivery presents a high

Health vulnerability to vertical transmission of syphilis: validity of constitutive and operational definitions.

OBJECTIVE: to develop and analyze the validity evidence of the constitutive and operational definitions of health vulnerability for pregnant women in relation to vertical transmission of syphilis. METHODS: a psychometric study, based on the Health Vulnerability Model, protocols from the Ministry of Health, updating the scoping review and conducting a concept analysis. Content validity evidence was analyzed by ten judges. The Content Validity Ratio was used to select operational definitions with a score equal to or greater than 0.8. RESULTS: the dimensions were divided into nine subdimensions

Untangling the complex relationship between HIV-exposure and tuberculosis in children: a narrative review.

With the remarkable success of antiretroviral programmes for the prevention of vertical HIV transmission, there has been a notable reduction in the proportion of children born with HIV and, subsequently, a corresponding increase in the population of children who are HIV-exposed but uninfected (CHEU). There is growing appreciation for the increased risk of childhood morbidity and mortality among CHEU compared with children who are HIV-unexposed, particularly from infectious diseases. Given the high prevalence of tuberculosis in populations with high HIV prevalence, the effect of HIV exposure on

AASLD ISDA Practice Guideline on treatment of chronic hepatitis B.

BACKGROUND AND AIMS: Accumulating data related to prevention, surveillance and treatment of chronic hepatitis B (CHB) provided the impetus for this updated guideline, using the Grading of Recommendation Assessment, Development and Evaluation (GRADE) approach. METHODS: The guideline was developed in compliance with the National Academy of Medicine standards. The guideline panel developed structured questions following the Population, Intervention Comparison, Outcomes (PICO) framework. The panel addressed 6 PICO questions covering prevention (maternal to infant transmission and horizontal trans

Evidence of Vertical Dengue Transmission: A Case of Fever in a 4-Day-Old Neonate.

BACKGROUND Dengue is a virus transmitted by Aedes mosquitoes, with rare cases of vertical transmission occurring through infected mothers. Acute infection can be identified by molecular viral testing or by detecting dengue non-structural protein 1 (NS1) and elevated serum IgM. This report describes the case of a 4-day-old male neonate with fever, rash, and jaundice due to vertical transmission of maternal dengue virus infection, confirmed by NS1 detection. CASE REPORT We report a case of a 4-day-old male neonate admitted with high fever, rash, and jaundice. Initial laboratory investigations sh
