Endoscopic anterior maxillotomy: infratemporal fossa via transnasal approach.

OBJECTIVES/HYPOTHESIS: To evaluate our initial experience with a novel technique, endoscopic anterior maxillotomy (EAM), for improved access to the anterior-lateral skull base. Clinical and radioanatomic data are presented to describe and define this novel technique. STUDY DESIGN: Case series. METHODS: Surgical patients with lesions of the pterygopalatine fossa, infratemporal fossa, and anterior-lateral maxilla treated from 2006 to 2008 are reviewed. Demographic data and surgical technique are presented. A radioanatomic analysis pre- and post-EAM is performed to describe increased access. Ma

Base-of-skull/infratemporal fossa shell fragment retrieval.

PURPOSE: Shell fragments and bullets are frequently observed to have settled in the skull base and infratemporal fossa (ITF) with the impact resulting in a new surgically complicated trauma. This compact region is one of the most difficult and dangerous locations for lodged objects. When called upon to retrieve these objects, the surgeon faces a complicated challenge. The complexity of the task depends on many factors, such as size, deformation, and surface hooks, as well as identification of the precise anatomic location. MATERIALS AND METHODS: New surgical approaches and successful methods

Solitary fibrous tumors of the infratemporal fossa: case series.

Solitary fibrous tumors (SFTs) were first described in the pleura. They are rare mesenchymal tumors of fibroblastic origin. An increasing number of SFTs has been described in the literature over the past years worldwide, particularly in the head and neck region, which is now considered the third most common site of occurrence. We report two cases of SFTs of the infratemporal fossa in two adult patients that were initially diagnosed as schwannomas on magnetic resonance imaging: one female patient with a small nodular lesion of the maxillary retromolar region and one male patient with a giant ma

Infratemporal fossa metastasis from carcinoma of the uterine cervix.

INTRODUCTION: Carcinoma of the uterine cervix is the fourth commonest malignant tumor in women. The disease spreads into the surrounding tissues by direct infiltration whereas spread by hematogenous dissemination is relatively unusual, and most commonly involves the lung, bone, and liver. The involvement of oral and maxillofacial region is extremely rare in gynecological cancer. CASE REPORT: In this paper, we present an unusual case of a metastatic squamous cell carcinoma originating in the infratemporal fossa of a 37-year-old woman who had underwent a subtotal hysterectomy, for squamous cell

The orbitozygomatic infratemporal fossa approach: a quantitative anatomical study.

Using detailed cadaveric dissections this study has demonstrated and quantified the increase in exposure and additional access gained by using the orbitozygomatic infratemporal fossa approach for neurosurgical access. The surgical window of exposure can be increased by up to 300% when this technique is utilised to facilitate access via either a subtemporal (to access the P2 segment of the posterior cerebral artery) or transsylvian (to access the basilar bifurcation) approach. In addition the distance between the surgeon and the operative field can be decreased by approximately 2-3 cm. The orbi

An endoscopic transnasal prelacrimal recess transmaxillary approach to the pterygopalatine fossa and infratemporal fossa.

OBJECTIVE: In this paper, the goal of the authors is to present the anatomic nuances and their clinical experience with lesions of the pterygopalatine fossa and infratemporal fossa using an endoscopic transnasal prelacrimal recess transmaxillary approach (PLRMA). METHODS: An endoscopic anatomical dissection of three fresh cadaveric heads was performed bilaterally to evaluate the feasibility of the PLRMA. Prior to dissection, stereotactic computed tomography scans were obtained for each head to obtain anatomical measurements. The area of exposure on the posterior wall of the maxillary sinus wa

[Renal clear cell carcinoma transferred to infratemporal fossa and pterygopalatine fossa: A case report].

SummaryRenal clear cell carcinoma is prone to distant metastasis, especially in the head and neck, but it is rare to transfer to infratemporal fossa and pterygopalatine fossa. We reported a 62-year-old male patient with complains of numbness and burning sensation on the left side of the face for more than 3 months. Left kidney removal was performed 8 years ago due to renal cancer. Preoperative enhancement CT showed a large blood-rich occupation in the left nasopharyngeal and pterygopalatine with adjacent paranasal sinus and skull base bone destruction. Under the general anesthesia, the anterio

[A case of infratemporal fossa abscess cause by facial acupuncture].

The infratemporal fossa abscess is an irregularly shaped cavity, situated below and media to the zygomatic arch. It consists of masseter and nervus vascularis, etc. Comparatively speaking, the low position of the infratemporal fossa has almost freed the patient from dental, nasal, ocular and aural source of pain. So facial acupuncture is more likely to cause the infection. But it is also possible that the patient get cryptogenic infection of maxillofacial cavity, the treatment of which needs further study.

Safe surgical access to the mandibular nerve at the infratemporal fossae.

Some anatomic patterns formed by the anterior border of the ascending ramus relative to the mandibular canal can cause nerve complications during surgery. We determined the frequency of obstructive anatomy in patients undergoing jaw surgery, and we described a perioperative method for a bilateral sagittal split osteotomy that ensured inferior alveolar nerve (IAN) protection. The anatomy of the anterior border of the ascending ramus of the mandible was examined on axial and cross-sectional cone beam computed tomographic images of 114 consecutive patients undergoing bilateral sagittal split oste

[Vegetable foreign body of the infratemporal fossa: a case report].

INTRODUCTION: Limited amplitude of mouth opening due to a foreign body in the infratemporal fossa is a rare event. We report a trismus due to a foreign body of vegetable origin located in the infratemporal fossa. OBSERVATION: A 44-year-old female patient presented with a left endobuccal wound made by an oak branch. Limited amplitude of mouth opening was noted on clinical examination. A first CT scan did not reveal any foreign body. Despite an adequate treatment, the patient presented with acute cellulitis. A second CT scan revealed a parapharyngeal and masticatory compartment collection. One

[Dissection of the cranial base en bloc with the infratemporal fossa].

A high incidence of morbidity and mortality was related with skull-base neoplasm surgery. Several advances have permitted, in recent years, the total excision of such neoplasms with minimal patient morbidity. Due to an improved understanding of the surgical anatomy of the skull-base and to the collaboration of the neurosurgeon and maxillofacial surgeon and, moreover, to the improvements of imaging (CT and MR) in the past decades new combined approaches were planned and performed to allow en bloc resections of extensive lesions. Extensive exposure of the tumor, improved management of the intern

Rhabdomyosarcoma of the infratemporal fossa: diagnostic dilemmas and surgical management.

We present a rare case of embryonal rhabdomyosarcoma in the infratemporal fossa presenting as an atypical facial pain syndrome. Radiographic imaging of the patient is discussed, and magnetic resonance imaging is recommended as the diagnostic modality of choice when mass lesions of the infratemporal fossa are suspected. Finally, a temporal approach to the infratemporal fossa is described, along with its advantages as an application for tumor resection in this anatomic region.

Removal of infratemporal fossa foreign body under C-arm.

A 12-year-old boy presented to our emergency department after being shot in the face. A computed tomographic scan revealed a bullet through the posterolateral wall of the maxillary sinus into the right infratemporal area, just adjacent to the skull base. We elected transantral approach with the help of endoscopy and C-arm. The bullet was successfully removed. Little is known on the best strategy for removing the infratemporal foreign body. Our experience in this case provides a safe and effective way for such injury.

Posttraumatic epidermal inclusion cyst of the deep infratemporal fossa.

The authors report a case of an epidermal inclusion cyst found in the deep infratemporal fossa 12 years after the patient sustained blunt trauma to that region. Posttraumatic epidermal inclusion cysts are rare and occur mainly in the fingers, palms, and soles. Introduction of the epidermal elements into the dermis during the trauma is thought to be the cause. This case is rare in presentation, with few reports in the English literature that describe an epidermal inclusion cyst in the deep infratemporal fossa. Review of the English literature disclosed no other cases of epidermal inclusion cyst

Selection of an Endoscopic Endonasal or Transoral Approach to the Infratemporal Fossa.

BACKGROUND: Both endonasal and transoral approaches can provide minimally invasive access to the infratemporal fossa (ITF); however, the appropriate indications for each corridor have not been sufficiently defined. This study aimed to explore the indication for selection of an endonasal or a transoral approach to the ITF. METHODS: Cadaveric dissection was performed on 6 specimens, one side for an endonasal approach and the contralateral side for a transoral approach.
