INTRODUCTION: Menière's disease is a chronic inner ear disorder marked by episodic vertigo, tinnitus, fluctuating sensorineural hearing loss, and aural fullness, all of which significantly impair quality of life. Intratympanic corticosteroids (ITCs) have emerged as a potential therapy, but their clinical efficacy and safety remain uncertain.
METHODS: A systematic review was conducted of randomized controlled trials (RCTs) comparing ITC to placebo in adult MD patients. Comprehensive searches were performed across multiple electronic databases. Six RCTs met the inclusion criteria, evaluating fo

Serous otitis media (SOM) occurs in children and constitutes one of the most significant causes of hearing loss in young age, posing as an important risk factor for long-term hearing loss. SOM is underdiagnosed, most frequently in infants, or the appointment to the ENT doctor is delayed due to non-acute symptomatology. The aim of the present study was to assess 285 patients with SOM diagnosed within a two-year span. The etiology and pathology of hearing loss in patients with different age groups were examined. The importance of a clinical examination and tympanometry was emphasized as absolute

OBJECTIVE: Intratympanic dexamethasone injection (ITDI) is widely used for inner ear disorders. However, optimal head positioning to maximize drug absorption through the round window membrane (RWM) while minimizing Eustachian tube (ET) washout remains undefined. This study aimed to determine the ideal head position for ITDI using a multimodal approach and to compare it with the traditional 45-degree head rotation.
METHODS: A multimodal analysis incorporating cadaveric dissection, 3-dimensional (3D) anatomic modeling, and clinical perilymph sampling from patients was performed. Twelve sagittal

CONCLUSION: Intra-tympanic dexamethasone injection (ITD) could serve as a first-line intra-tympanic (IT) treatment for Meniere's disease (MD), regardless of hearing level. Even if the response to initial ITD course was unsatisfactory, a repeated course may be beneficial with some patients.
OBJECTIVES: This study examined the effect of repeated courses of ITD administered on demand and investigated the possibility of ITD as an initial IT treatment for medically intractable MD patients.
METHOD: Fifty-one patients who had been diagnosed with definite MD and given course(s) of ITD treatment were

Safe and efficient drug delivery to the inner ear has always been the focus of prevention and treatment of sensorineural deafness. The rapid development of nanodrug delivery systems based on hydrogel has provided a new opportunity. Among them, thermo-sensitive hydrogels promote the development of new dosage form for intratympanic injection. This smart biomaterial could transform to semisolid phase when the temperature increased. Thermo-sensitive hydrogel nanodrug delivery system is expected to achieve safe, efficient, and sustained inner ear drug administration. This article introduces the key

Introduction: One major obstacle in validating drugs for the treatment or prevention of hearing loss is the limited data available on the distribution and concentration of drugs in the human inner ear. Although small animal models offer some insights into inner ear pharmacokinetics, their smaller organ size and different barrier (round window membrane) permeabilities compared to humans can complicate study interpretation. Therefore, developing a reliable large animal model for inner ear drug delivery is crucial. The inner and middle ear anatomy of domestic pigs closely resembles that of humans

Intratympanic steroid treatment for the sudden sensorineural hearing loss (SSNHL) has a long history and many techniques have been developed. The efficacies are varied in different studies owing to different criteria, steroid type and dose, delivery methods, or absence of comparison groups. Recently, animal experiments suggested that continuous delivery systems produce the higher inner ear drug concentrations than other ways. This study was aimed at evaluating the efficacies of intratympanic dexamethasone perfusion versus injection for treatment of refractory sudden sensorineural hearing loss

Objective: This study aimed to investigate the optimal head position (OHP) following intratympanic injection, a critical intervention in treating inner ear disorders. Identifying OHP is essential to maximize drug retention in the middle ear, thereby significantly enhancing the therapeutic efficacy by mitigating the significant issue of injectate leakage through the eustachian tube (ET). Exploratory various positions of ET orifice and round window (RW) were investigated and associated with head movements. Methods: Twenty-two (10 males and 12 females) anonymized high-resolution computed tomograp
