Safety of Intra-articular Hip Corticosteroid Injections: A Matched-Pair Cohort Study.

BACKGROUND: Recent studies have suggested there is an increased risk of avascular necrosis (AVN), subchondral insufficiency fracture (SIF), femoral head collapse, and osteoarthritis (OA) progression in the 12-month period after hip corticosteroid/anesthetic injection (CSI); however, these studies have failed to account for preinjection OA severity or preexisting AVN/SIF. PURPOSE: To compare these complication rates in patients treated with versus without hip CSI, while minimizing the aforementioned forms of selection bias present in previous investigations. STUDY DESIGN: Cohort study; Level

A case of necrotizing fasciitis following intra-articular injections - Iatrogenic or spontaneous?

Here, we report a case of necrotizing fasciitis following intra-articular injection of hyaluronic acid. A 73-year-old female received intra-articular injections of hyaluronic acid due to arthralgia at the left shoulder and knee, and was found dead in her living room at one day. At the forensic autopsy, injection marks with bullae and erythema were found at the left shoulder and knee and liquefactive necrosis of muscle tissues was observed in the left but not right extremities. Histopathological examinations of the left upper arm and thigh revealed severe rhabdomyolysis with lots of bacterial c

Diagnosing the unrecognized systemic absorption of intra-articular and epidural steroid injections.

OBJECTIVE: To present a case series of patients who were misdiagnosed with endocrine disorders because of failure to recognize the systemic absorption of intra-articular and epidural steroids and to discuss the utility of performing a urine screen to detect synthetic glucocorticoids. METHODS: In this case series, we describe the clinical, laboratory, and imaging findings of 3 patients referred to our clinic, each with a presumed endocrine disorder. RESULTS: Patient 1 was a 54-year-old woman with weakness, loss of appetite, and a hormonal profile suggestive of hypopituitarism. Patient 2 was a

Weekly intra-articular injections of bone morphogenetic protein-7 inhibits osteoarthritis progression.

INTRODUCTION: We investigated the ability of a weekly intra-articular injection of bone morphogenetic protein (BMP)-7 to prevent osteoarthritis in rabbits with anterior cruciate ligament transections. METHODS: First, 36 knee joints were randomly divided into four groups: 50, 500, 5,000 ng BMP-7, and control. Knee cartilage was evaluated at 4, 8, and 12 weeks. Then, in order to control for individual differences, 500 ng BMP-7 was injected into one knee and phosphate-buffered saline (PBS) into the other, and the two knees were compared at 4, 8, and 12 weeks (n = 5). For pharmacokinetic analysis

In vivo effects of single intra-articular injection of 0.5% bupivacaine on articular cartilage.

BACKGROUND: Single intra-articular injections of local anesthetics are commonly used clinically. Recent in vitro studies have demonstrated chondrotoxic effects of local anesthetics, with the greatest emphasis on bupivacaine toxicity. This in vivo study was conducted to determine whether a single intra-articular injection of 0.5% bupivacaine results in chondrocyte morbidity and rapid chondrolysis. METHODS: Forty-eight Sprague-Dawley rats received a 100-microL injection of sterile 0.9% saline solution (negative control) into one stifle joint and 100 microL of either preservative-free 0.5% bupiv

Intra-Articular Atelocollagen Injection for the Treatment of Articular Cartilage Defects in Rabbit Model.

BACKGROUND: Atelocollagen is widely recognized as a biomaterial for regenerative medicine because of its good compatibility and low antigenicity. Injury of the outermost layer of articular cartilage, known as the lamina splendens, can lead to osteoarthritis (OA) and eventually full-thickness cartilage loss. The intra-articular injection of atelocollagen has been designed to restore the cartilage layer and cartilage defects in OA joints. In this study, we investigated the efficacy of atelocollagen as a cartilage supplement for joint defects. METHODS: In this study, we evaluated the therapeutic

Nanoscopic gel particle for intra-articular injection formulation.

Hyaluronic acid (HA) based nanogels showed effective intracellular delivery efficacy for anti-cancer and anti-inflammatory drugs, characterized by their ability targeting relevant cell receptors. In the present study, we demonstrate the ability of hyaluronic acid-polyethyleneimine (HA-PEI) nanogels as a promising dual-functional interfacial active for intra-articular injection to intervene arthritis. Nanomechanical measurements on both model substrates and human cartilage samples confirm that the HA-PEI nanogels can significantly improve interfacial lubrication, in comparison to HA molecules,

Intra-articular Steroid Injection as the Etiology of Acute Pancreatitis.

Acute interstitial pancreatitis is typically caused by gallstones and alcohol use. Less common causes include infection and drugs. Patients present with epigastric pain and often require pain medications and hospitalization depending on severity. We present a unique case of drug-induced pancreatitis likely caused by intra-articular corticosteroid injections on two separate occasions in the same patient. In both instances, other etiologies were ruled out. Given the temporal relationship between the intra-articular corticosteroid injection and presentation of pancreatitis, the corticosteroid inj

Placement of intra-articular injection verified by the backflow technique.

OBJECTIVE: To develop and to assess a simple, inexpensive method for ascertaining, without any imaging procedure, the intra-articular placement of the needle in the knee for intra-articular injections. METHODS: Outpatients referred for intra-articular treatment with "dry" symptomatic knee osteoarthritis were included in this prospective study. "Dry" knee disease was defined as a knee without any clinically detectable effusion. Once intra-articular positioning of the needle considered adequate using the backflow technique, contrast solution was injected using the same needle without changing i

Clostridial myonecrosis following intra-articular steroid injection.

Fatal clostridial myonecrosis occurred in a 65-year-old man after intra-articular injection of steroids to the shoulder joint. Clinical evidence of the possibility of clostridial infection is sufficient reason to initiate treatment. The opportunity to save a life may be lost by waiting for paraclinical data for definitive diagnosis to initiate treatment. "Surgical skin prep" for every intra-articular injection is a preventive measure. Increasing joint pain after intraarticular injection should be considered a serious complaint and requires careful clinical follow-up observations.

PSMA Uptake Due to Recent Intra-articular Injection.

We report a case involving a 66-year-old man with prostate adenocarcinoma, referred for initial staging using 99m Tc-PSMA and 99m Tc-MDP bone scan. The whole body PSMA scan revealed foci of increased tracer uptake in the right knee and the surrounding periarticular space above the knee. Subsequent 99m Tc-PSMA SPECT/CT imaging demonstrated evidence of arthritis characterized by effusion in the periarticular region, connected to the joint in the right knee. Notably, the patient had received an intra-articular corticosteroid injection just 2 days before the scan. This case highlights the necessit
