Injury Severity Score based estimation of height of fall in bus rolling down the cliff.

A case of bus rollover into the canyon, 40 m down the road, with 47 occupants out of which 18 were fatally injured, was used to compute the Injury Severity Score (ISS) for each passengers as well as the equivalent free fall for this particular accident, to be compared to the height of fall as estimated by the Lau's model based on ISS, resulting the conclusion whether Lau's model and the computation of ISS can be considered reliable to estimate the height of fall in any other case. Dealing with this, we would be also able to assess a protective potential of the bus on occupants while it falls f

A prospective evaluation of the Modified Hand Injury Severity Score in predicting return to work.

Hand injuries are the main cause of work-related disability in young adults. We have devised the Modified Hand Injury Scoring System to quantify hand, wrist and forearm injuries. This study aims to determine its value in predicting ability and time taken to return to work after such injury. Prospectively-assigned MHISS at presentation was compared with demographic, injury, employment and quality of life information 40-52months after acute hand or forearm injury. MHISS score was the only variable investigated found to predict ability to return to work. Factors not associated included age at inj

Concordance and Variability in Thoracolumbar Injury Classification and Severity Score (TLICS)-guided Treatment Decision-making.

STUDY DESIGN: Systematic review and pooled analysis. OBJECTIVE: To evaluate the concordance between Thoracolumbar Injury Classification and Severity Score (TLICS) recommendations and actual treatment decisions in thoracolumbar trauma. SUMMARY OF BACKGROUND DATA: TLICS is widely used to guide management of thoracolumbar fractures, recommending nonoperative treatment for scores ≤3 and operative intervention for scores ≥5. However, treatment variability may occur in cases with indeterminate scores, uncertainty in posterior ligamentous complex (PLC) integrity, or clinical modifiers not fully cap

Diagnostic accuracy of shock index, revised trauma score, and injury severity score for mortality prediction in trauma: A hierarchical meta-analysis.

BACKGROUND: Shock Index (SI) and Revised Trauma Score (RTS) are physiological tools available at the point of trauma triage. Injury Severity Score (ISS) is an anatomical score that requires completed injury characterization and is typically available only after initial resuscitation. Their comparative value for predicting mortality and whether ISS meaningfully adds to the acute tools remains unclear. AIMS: The aim of this systematic review and hierarchical meta-analysis is to pool the diagnostic accuracy of SI and RTS as acute mortality-prediction tools in adult trauma patients; to benchmark

Mangled Extremity Severity Score in the Assessment of Extremity Injuries - Is it Reliable?

The mangled extremity severity score (MESS) was established 28 years ago to decide performing an amputation in cases of limb trauma. It assigns points for 4 aspects of the injury: the extent of soft tissue and skeletal injury, limb ischemia and ischemic duration, shock, and age. A score of 7 or higher indicates that primary amputation is required. Thirty-one patients with MESS scores of 7 to 8 underwent limb salvage surgeries, with 13 limbs being saved and 18 limbs being amputated. Upper extremities trauma had a salvage rate of 62.5%, whereas lower extremity trauma had a salvage rate of 20%. M

Severe burn injury: Body Mass Index and the Baux score.

OBJECTIVE: The revised Baux score (age total body surface area (TBSA) burned and inhalation injury)) is predictive of mortality in burn patients. Our study objective was to assess whether the addition of body mass index (BMI) to the revised Baux score would be of value. We posited that increasing BMI follows a pattern similar to age and TBSA in the revised Baux score after severe burn injury. METHODS: Patient data from the burn registry was queried for patients admitted between 1/1/2013 to 8/31/2019. Patients 12 years or older with a TBSA of 20% or greater burn were included. Are multiple limb injuries in severely injured patients negligible? Evaluation of progression and outcome using a new scoring system for extremity injury: the Extremity Severity Score (ESS).

INTRODUCTION: Extremity injuries appear to have less impact on the mortality of multiple trauma patients (ISS ≥ 16). The Primary Survey of the Advanced Trauma Life Support (ATLS) only lists pelvic and femur fractures among the extremity injuries. The aim of this study was to evaluate the role of multiple extremity injuries in terms of lethality, progression, and complications in multiple trauma patients and the actual influence of concomitant blood loss. The Extremity Severity Score (ESS) was developed as a central instrument for this purpose. MATERIAL & METHODS: This investigation is a retro

The Thoracolumbar Injury Classification and Severity Score Appropriately Predicts Treatment in Children Aged 10 and Under.

STUDY DESIGN: A retrospective cohort study. OBJECTIVE: This study aims to assess the validity of the Thoracolumbar Injury Classification and Severity Score (TLICS) in patients ≤10 years old. SUMMARY OF BACKGROUND DATA: TLICS is a validated measure developed to help facilitate clinical decision-making regarding thoracolumbar spinal trauma in adults. Studies examining the utility of TLICS in children skew toward older pediatric patients, where the spine's biomechanical properties are more similar to adults. Due to differences in a preadolescent spine compared with a more mature, adolescent spi

Changes of serum trace elements in early stage trauma and its correlation with injury severity score.

BACKGROUND: Severe trauma can cause secondary multiple organ dysfunction syndrome and death. The absolute and relative concentrations of trace elements in both critical care and conventional treatment, which can lead to acute trace element deficiency, constitute an important mechanism of multiple organ dysfunction syndrome (MODS)/multiple organ failure (MOF). METHODS: We investigated the changes in serum Cu, Zn, and Fe in early stage trauma of patients with the high injury severity score (ISS) and correlated the change in trace elements with ISS. Blood samples were collected within an hour of

Effect of anticoagulation on isolated traumatic brain injury mortality using TQIP database: a propensity score analysis stratified by head injury severity.

INTRODUCTION: The impact of anticoagulation (AC) on traumatic brain injury (TBI) outcomes remains varied in the literature. Previous studies often relied on single-center data, small sample sizes, or lacked adjustments for key confounding variables. This study used the population-based Trauma Quality Improvement Program (TQIP) database to analyze the effect of AC on isolated TBI mortality, hypothesizing that AC's impact varies with TBI severity. METHODS: This retrospective cohort study analyzed data from 2020 to 2021 TQIP database. Patients with isolated TBI were identified using Internationa

A prospective evaluation of the clinical utility of the lower-extremity injury-severity scores.

BACKGROUND: High-energy trauma to the lower extremity presents challenges with regard to reconstruction and rehabilitation. Failed efforts at limb salvage are associated with increased patient mortality and high hospital costs. Lower-extremity injury-severity scoring systems were developed to assist the surgical team with the initial decision to amputate or salvage a limb. The purpose of the present study was to prospectively evaluate the clinical utility of five lower-extremity injury-severity scoring systems. METHODS: Five hundred and fifty-six high-energy lower-extremity injuries were pros

Association between the maxillofacial injury severity score to helmet types, neutrophil lymphocyte ratio and length of stay.

BACKGROUND: Traffic accidents are a major etiological factor in maxillofacial injuries, exhibiting a high incidence rate. The Maxillofacial Injury Severity Score (MFISS) is utilized to assess the severity of maxillofacial trauma. Helmet use is a preventive measure against more serious injuries. Patients with maxillofacial trauma are at risk of complications and infections. Elevated neutrophil to lymphocyte ratio (NLR) in hospitalized patients correlates with a longer length of stay (LOS). OBJECTIVES: This study aimed to evaluate the relationship between helmet types and MFISS scores, as well

Factors influencing the injury severity score and the probability of survival in patients who fell from height.

In Japan, falls from height result in the second highest trauma mortality rate after traffic motor vehicle collisions and the highest trauma-related mortality rate amongst young people. We aimed to identify factors that worsen injury severity and lower survival probability of patients who fell from height and to contribute to the improvement of their prehospital and in-hospital care. This retrospective analysis retrieved hospital records of 179 patients aged ≥ 15 years who were transported to our hospital after a fall from height during April 2014-March 2020. On multiple regression analysis, f
