Reducing duplicate hand and wrist series in the emergency room: experience at Madigan Army Medical Center.

OBJECTIVE: To demonstrate that repetitive hand or wrist series could be minimized with minor refinements of triage. POPULATION: 2,119 consecutive trauma patients requiring hand or wrist series. METHOD: In baseline phase I, radiographs were ordered by triage corpsmen. In phase II, RNs or physicians performed triage and ordered films. Phase III followed inservice training on strategies to minimize duplicate studies. Phase IV required the RN or physician to provide written justification for duplicate studies. The number of duplications in each phase were compared to baseline using the chi-squar

Does the voice improve with vocal hygiene education? A study of some instrumental voice measures in a group of kindergarten teachers.

This study investigated the efficacy of a program of vocal hygiene education designed for an occupational group at risk for dysphonia, namely, kindergarten teachers. Twelve female teachers explored concepts and knowledge of vocal abuse and vocal hygiene in a workshop session and attempted to practice vocal hygiene for 2 months. They showed significant voice improvement as assessed instrumentally in terms of three acoustic and electroglottographic parameters. There was no significant change of voice in a control group of 13 teachers. The results suggest that teachers were able to improve their

[Reducing the Incidence of Back Pain in Post-Percutaneous Coronary Intervention Patients With Femoral Sheath].

BACKGROUND & PROBLEMS: Patients undergoing percutaneous coronary interventions (PCIs) are restricted to bedrest and immobilization of the affected leg for sheath retention. These result in high incidences of back pain and emotional irritation, which may negatively affect cardiac rehabilitation. One-hundred percent of the post-transfemoral PCI patients in our unit had a back-pain score of 3 or higher, with an average pain score of 6. Analysis showed the main causes to be: poor awareness of nurses regarding back pain and physical activity; lack of physical activity guidelines and no regular audi

Vascular Access Specialist Training, Experience, and Practice in the United States: Results From the National PICC1 Survey.

Vascular access specialists are increasingly responsible for insertion and care of peripherally inserted central catheters in the United States. Despite proliferation of the specialty, little is known about the training, practice patterns, and experiences of these clinicians. In partnership with a vascular access and an infusion nursing organization, a Web-based survey of 1698 clinicians was conducted. Substantial variation in practices related to insertion, use of technology, dressing and flushing, and management of complications was observed. Some reported practices were inconsistent with av

What Residents Say About Communicating with Patients: A Preliminary Examination of Doctor-to-Doctor Interaction.

This article describes the implementation and initial assessment of a training blog created within a family medicine department and used as a feedback mechanism for residents. First-year residents (n = 7) at a large private East Coast university hospital had an interaction with a patient recorded and posted to a training blog. The residents then watched this, and posted a reaction to their interaction with the patient. Within this reaction the residents offered self-reflection on the experience and were provided an opportunity to solicit advice from their colleagues to improve their communicat

Resident education in the radiological sciences: what now?

OBJECTIVE: With the dizzying changes in the rapidly evolving profession of radiology, the structure of resident education in the associated sciences of imaging, physics, radiobiology, and radiation effects must be reevaluated continually. What roles do these basic radiologic sciences play in bolstering the neophyte radiologist on a career of patient care? How should we define the spectrum of material that should be learned? How should that spectrum be taught? Who decides these things? With the impending changes in the radiology board certification process, questions have been raised as to how

Building grass roots capacity to tackle childhood obesity.

AIMS: In recognition of the increasing problem of child obesity in London and elsewhere, we were commissioned to build capacity to tackle this major public health concern. This paper describes one of the outputs of this work: to develop and deliver effective brief intervention training on the subject of childhood healthy/unhealthy weight and obesity to be used by anyone who works with children and families, regardless of their job title or level of educational achievement. METHODS: A literature review informed the process. The slim evidence derived was combined with the expertise of an expert

Innovative training for new surgical roles--the place of evaluation.

BACKGROUND: This paper describes the central role of 'external' evaluation, provided by an independent researcher, in developing an innovative curriculum for new professional roles with surgery. Workforce changes affecting the National Health Service provided an opportunity to develop 2 new roles and design training programmes to support them. The perioperative specialist practitioner (PSP) role was designed from scratch, while surgical care practitioner (SCP) training built on existing practice. Training programmes combined formal modules at Imperial College London (approximately 48 days over

Interprofessional simulation-based education program: a promising approach for changing stereotypes and improving attitudes toward nurse-physician collaboration.

An effective working relationship between physicians and nurses is enhanced by fostering positive perceptions and collaborative attitudes between the two professions. This brief paper examines the effect of an interprofessional simulation-based communication education program in enhancing medical and nursing students' perceptions of each other's profession and their attitudes toward nurse-physician collaboration. Pretest-Posttest design was conducted on 96 medical and nursing students who demonstrated the existence of professional stereotypes in the baseline data. This study showed that by pro

Making acute hospital exercises more realistic without impacting on healthcare delivery.

Exercises can suffer from a lack of realism that reduces the value of the exercise in terms of the positive experience of the participant and the possibility that outcomes are based on artificialities created by the exercise environment. It is important to minimise these so that participants actively engage and recommendations are based on robust observations. Field exercises provide the most realistic format in which to exercise but are disruptive to normal working and expensive. In a health environment, anything but the most minimal disruption to normal service would be considered unacceptab

Focused echocardiographic evaluation in resuscitation management: concept of an advanced life support-conformed algorithm.

Emergency ultrasound is suggested to be an important tool in critical care medicine. Time-dependent scenarios occur during preresuscitation care, during cardiopulmonary resuscitation, and in postresuscitation care. Suspected myocardial insufficiency due to acute global, left, or right heart failure, pericardial tamponade, and hypovolemia should be identified. These diagnoses cannot be made with standard physical examination or the electrocardiogram. Furthermore, the differential diagnosis of pulseless electrical activity is best elucidated with echocardiography. Therefore, we developed an algo

Oklahoma City and Tulsa Metropolitan Medical Response System.

The tragic loss of lives on September 11 followed by the Anthrax contaminations reinforced the need for work already underway through the Metropolitan Medical Response System (MMRS). Through leadership from the Medical Director for the Medical Control Board and EMSA, Oklahoma City and Tulsa have been the beneficiaries of nearly $1.5 million dollars in funding to prepare for nuclear, chemical or bioterrorism. The two Oklahoma cities were among the second wave of cities invited in 1999 to participate in the federally funded program for Metropolitan Medical Response. Many of the requirements of t

Strengthening Libya's primary healthcare workforce: a replicable model for health system resilience in fragile states.

BACKGROUND: Settings characterized by significant systemic challenges face profound difficulties in building a resilient health workforce, a critical component for achieving Universal Health Coverage (UHC). In Libya, a period of prolonged instability has fragmented the primary health care (PHC) system, resulting in uncoordinated training and a workforce ill-equipped to deliver essential services. This study evaluates a multi-year initiative designed to establish a standardized, nationally-owned in-service training (IST) system to strengthen PHC capacity. METHODS: A phased, multi-method interv
