Embracing Enhanced Recovery After Cardiac Surgery Program.

Enhanced recovery after cardiac surgery (ERAS) advocates for consistent and standardized delivery of evidenced-based perioperative care. Multidisciplinary teams deliver integrated patient care elements across all phases of care. Elements include risk-screening, prehabilitation, nutritional support, organ protection, patient blood management, early mobilization, optimal analgesia, and prevention of complications. Implementation of an ERAS program has been shown to improve patient recovery and reduce complications, length of stay, and resource utilization. However, the evidence is inconsistent a

Consultative total parenteral nutrition teams: the effect on the incidence of total parenteral nutrition-related complications.

A two part, prospective study was undertaken to establish the existing frequency of total parenteral nutrition-(TPN) related mechanical, metabolic, and septic complications in an institution with a consultative TPN team (group A) and to determine if increasing the involvement of the TPN team in patient monitoring and verifying adherence to TPN infection control guidelines would reduce the incidence of these complications (group B). The initial 28 consecutive patients were managed entirely by their primary physicians with the role of the TPN team limited to consultative activities while the nex

Corporatization of Academic Medical Centers and Its Impact on the "Esprit-De-Corps" of Surgical Teams.

BACKGROUND: Accelerating corporatization of US healthcare has reshaped academic medical center governance, with for-profit investor-owned organizations comprising 23.7% of market share in 2,023 and 56.1% of hospitals owned by corporations with 3 or more hospitals. Although intended to enhance financial sustainability, centralized, productivity-driven management may conflict with the professional culture underpinning high-performing surgical teams. STUDY DESIGN: Narrative perspective synthesizing literature in health policy, organizational psychology, and surgical culture to examine how corpor

Improving preventive locust management: insights from a multi-agent model.

BACKGROUND: Preventive management of locust plagues works in some cases but still fails frequently. The role of funding institution awareness was suggested as a potential facilitating factor for cyclic locust plagues. We designed a multi-agent system to represent the events of locust plague development and a management system with three levels: funding institution, national control unit and field teams. A sensitivity analysis identified the limits and improvements of the management system. RESULTS: The model generated cyclic locust plagues through a decrease in funding institution awareness.

The Mount Sinai Clinical Pathway for the Diagnosis and Management of Hypercortisolism due to Ectopic ACTH Syndrome.

Neoplasms that secrete ectopic adrenocorticotropin (ACTH) may cause severe, life-threatening hypercortisolism. These tumors are often difficult to localize and treat, requiring a comprehensive and systematic management plan orchestrated by a multidisciplinary team. The Mount Sinai Adrenal Center hosted an interdisciplinary retreat of experts in adrenal disorders and neuroendocrine tumors (NETs) with the aim of developing a clinical pathway for the management of Cushing syndrome due to ectopic ACTH production. The result was institutional recommendations for the diagnosis, localization, surgica

Surviving institutional contradictions: routine data use practices at the subnational levels.

Routine Health Information System data are widely promoted as a foundation for evidence-informed planning, supervision, and service improvement in low- and middle-income countries. Yet their use in decentralized health systems is often shaped by contradictory institutional logics. In this study, contradictory institutional logics refer to conflicting expectations about what actors should prioritize, how data should be used, and which resources are made available to do so. We examined how these contradictions shape routine data use at subnational levels of the Tanzanian health system and how lo

Student psychological well-being in higher education: The role of internal team environment, institutional, friends and family support and academic engagement.

Psychological well-being of students is an area of concern in higher education institutes across the world. Although several studies have explored the factors associated with students' psychological well-being, limited research has focused on the relation between the overall support for students and psychological well-being. Students of higher education may get formal support, in the form of team environment and institutional support; and informal support, in the form of family and friends' support. The purpose of this study is to examine the relation of these four kinds of support with psycho

The Making of a Skull Base Team and the Value of Multidisciplinary Approach in the Management of Sinonasal and Ventral Skull Base Malignancies.

The management of sinonasal and ventral skull base malignancies is best performed by a team. Although the composition of the team may vary, it is important to have multidisciplinary representation. There are multiple obstacles, both individual and institutional, that must be overcome to develop a highly functioning team. Adequate training is an important part of team-building and can be fostered with surgical telementoring. A quality improvement program should be incorporated into the activities of a skull base team.

Operational impact of canine teams in the fight against drug trafficking in Colombia: review of their contribution, strategies and recent trends.

As part of national and international cooperation to fight the drug-trafficking system, the Colombian National Police (PNC) has created institutional operational synergy to combat organised armed groups. To this end, it uses police service dog and handler teams which are distributed across Colombia in 75 canine units and which are specialised in different policing tasks. The focus of canine teams is to counter actions that undermine peaceful co-existence and citizen security by checking for explosive substances around coca, marijuana and poppy fields. In 2016, these teams discovered 16% of the
