A prospective study of long-term care institutionalization among the aged.

A statewide probability sample of 1,625 elders living in Massachusetts are studied prospectively to identify key determinants of long-term care (LTC) institutionalization. One-hundred forty-seven elders, 9 per cent of the original cohort, entered a LTC institution during the six-year investigation. Using logistic multiple regression, we examine the predictive power of 19 independent variables grouped into six categories: demographic characteristics, attitude, social context, long-term care needs, physical disability, and mental/emotional disability. Five variables are significantly related to

Riboflavin status in a group of institutionalized elderly.

We studied the riboflavin intake and biochemical status in a representative sample (20 males and 52 females) of institutionalized elderly people (aged 68-95) living in the Autonomic Community of Madrid. The riboflavin density of the diet was in general higher than the recommended amount, so the quality of the diet, related to riboflavin, can be considered satisfactory, and probably the caloric deficit is the major cause of the low intakes found. We found no correlation between the intake and biochemical status of riboflavin. The elderly with erythrocyte glutathione reductase greater than 1.2 a

Determinants of the desire to institutionalize in Alzheimer's caregivers.

BACKGROUND: The desire to institutionalize is an important predictor of future institutionalization. Few studies have examined potentially modifiable caregiver characteristics which might be the focus of future interventional strategies. METHODS: A total of 102 patient/caregiver dyads with Alzheimer's disease (n = 84) or mild cognitive impairment were recruited through a memory clinic. Cross-sectional analyses of a range of patients, caregivers, and context of care-related characteristics were conducted. RESULTS: Caregiver desire to institutionalize was significantly associated with a number

Restless legs syndrome in institutionalized elderly.

Restless legs syndrome (RLS) is characterized by disturbing leg sensations associated to sleep complaints and excessive daytime somnolence. In the elderly, it affects 10 to 35%. Our objective was to determine the prevalence of RLS in institutionalized elderly, analyzing its relationship with clinical, laboratorial and neurophysiological features. We conducted a cross-sectional study of all the subjects under treatment on a chronic-care geriatric service by using face-to-face interviews, which include sleep complaints, the Epworth Sleepiness Scale and standardized questions addressing the four

Prevalence of movement disorders in institutionalized elderly.

The proportion of the population over age 65 years is steadily increasing in Canada and the elderly who are unable to live independently are frequently institutionalized in chronic care facilities. We report our observations on movement disorders (MD) in an institutionalized elderly population of Saskatchewan. A representative sample of 67 subjects aged > or = 65 years had a detailed neurological evaluation on three separate occasions. MD were detected in 13 (19%) cases. The majority, 11 (16%), were females. Seven (10%) had essential tremor, 4 (6%) Parkinson's disease and 2 (3%) had drug-induc

Alcohol-related dementia in the institutionalized elderly.

This study examined the distribution of alcohol-related and other dementias in a sample of 130 cognitively impaired residents of long-term care facilities in a Northern Ontario community. Study procedures entailed standardized psychiatric, neurological, and neuropsychological evaluations. Diagnoses of dementia of the Alzheimer type (DAT) and vascular dementia were based on criteria of the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association. The diagnosis of alcohol-related dementia (ARD) was based on extensive

[A study of falls experienced by institutionalized elderly].

This study was conducted to determine what factors are related to falls, conditions contributing to falls and the associated injuries. Subjects who had lived in 3 institutions for the elderly in the western region of Shizuoka Prefecture were chosen. Subjects (n = 181) had an average age of 76.4 +/- 7.8 years, and all were interviewed regardless of their history of falls. For those who had experienced falls further interviews were conducted. The following results were obtained: 1) There were 64 subjects (35.4% of the subjects) who had experienced falls, some having experienced multiple falls (f

Depression and mortality among institutionalized aged.

This study examined the association between depression and mortality among a group of nursing home and congregate apartment residents (initial n = 898) over a 30-month period. Baseline [Time 1 (T1)] and 1-year follow-up [Time 2 (T2)] assessments yielded research-based diagnoses of possible major, minor, or no depression, along with measures of functional disability, cognitive status, and physician-rated health. Event history analyses were used to assess differential mortality as a function of level of depression after T1 and of change in depressive status from T1 to T2. Significant effects for

Accidents in the institutionalized elderly: injuries and consequences.

A total of 308 major and 519 minor accidents involving 588 patients occurred during one year at a somatic long-term care hospital and its associated nursing homes. In relation to the number of patients, the incidence of accidents was nearly three times higher in the nursing homes than in the hospital. The most prevalent injuries in female patients were fractures and hematomas, while open wounds were more frequent among male patients. Hip fractures were about as common in women as in men, while fractures of the extremities were three times more frequent in women. The majority of the accidents o

The epidemiology of skin tears in the institutionalized elderly.

While skin tears are a common occurrence in the institutionalized elderly population, nothing has been written about this problem. We retrospectively studied all incident reports during a 1-year period at a large, urban, long-term-care facility to identify residents with skin tears. The overall incidence of skin tears was 0.92 per patient per year. The incidence rate for females, but not for males, increased significantly with age (P = 0.012). The mean length of the skin tear was 1.9 cm +/- 1.4 (mean +/- SD). Eighty percent occurred in the upper extremities, with the most frequent location bei

[Institutionalized treatment of agoraphobia with panic anxiety].

We describe an integrated behavioral-psychodynamic treatment programme for inpatients suffering from agoraphobia and panic anxiety. This programme emphasizes intensive drug-free exposure training followed by work with anxiety-laden dynamic conflicts. We present promising results of evaluation studies, and discuss the important role of cognitive reconstruction of anxiety and of passive and active avoidant attitudes in exposure training. Lastly, we make certain recommendations concerning the approach to anxiety problems in general practice.

In this study, we investigated the oral conditions of 77 handicapped children (57 boys and 20 girls) and backgrounds of living in an institution. The results were as follows. Among 73 children having permanent dentition, the percentage of subjects with one or more DMF teeth was 94.52%, while DMFT was 7.45. The percentage of teeth treated for dental caries was 57.35%. GI (Gingival Index) was 1.7, and OHI-S was 3.50. The other features observed were attrition of posterior teeth in 25 subjects and crowding in 39. Medications of elderly institutionalized incontinent females.

The purpose of the study was to explore drug use of elderly incontinent female nursing home residents, to identify potential drug--drug interactions, and to identify the number and categories of drugs taken having the potential to cause urinary incontinence. The medication administration records of 84 female residents were examined. The drugs were categorized by therapeutic classification as designated by the American Hospital Formulary Service. Potential drug--drug interactions were identified using The Hansten Drug Interaction Knowledge Base Program, a microcomputer version of Hansten's (198
