Comparison of Analogue insulin with other insulins in patients with type 1 diabetes in Ahmedabad, Western India: A Rretrospective study.

BACKGROUND & AIM: Type of insulin is prescribed according to the glycaemic status of the patient, affordability, and preference of the patient. Analogues are considered to be the good therapeutic treatment for patients with type 1 diabetes, as they closely mimic physiological insulin kinetics and minimize the risk of hypoglycemia as compared to other insulin formulations. In this study, we aimed to assess the effectiveness of different insulin regimen (analogue insulins, regular insulins, and NPH insulins) in patients with type 1 diabetes in Ahmedabad, western India. METHOD: A retrospective s

Structure-function studies of four disulfide-containing insulins from Conus textile and Conus memiae and a snail-guided human insulin analogue.

Cone snail insulins represent a unique class of insulin-like polypeptides that can regulate the human insulin receptor, offering valuable lead compounds for the treatment of diabetes and its complications. Among these, signalling insulins constitute the predominant class of snail insulins and are characterized by the presence of an additional fourth disulfide bond in the sequence. However, studies examining their affinity for the human insulin receptor remain limited. Database analysis revealed signalling insulins from Conus textile (Scon-ins Tx1) and Conus memiae (Scon-ins Me1) with chain len

Insulin, insulin sensitivity and hypertension.

The sensitivity of tissue to insulin is of physiological, pathophysiological and therapeutic relevance. The quantity of insulin and the response to insulin are paramount complementary factors in the regulation of glucose metabolism, and may, at least under certain pathophysiological conditions, also affect cardiovascular function. Hypertension has a high prevalence among subjects with decreased insulin sensitivity and/or hyperinsulinaemia due to obesity, impaired glucose tolerance, non-insulin-dependent diabetes mellitus, and certain other conditions. There is evidence that, even in the absenc

Cost-Effectiveness of Once-Weekly Insulin Icodec versus Daily Basal Insulins in Chinese Adults with T2DM: A Treatment-Background Stratified Analysis.

OBJECTIVE: This study evaluated the long-term cost-effectiveness of once-weekly insulin icodec versus daily basal insulins in Chinese adults with type 2 diabetes mellitus (T2DM) across treatment backgrounds (insulin-naïve to basal-bolus users). METHODS: Using the UKPDS-OM2.1 model calibrated to ONWARDS trial data (1-5), we simulated lifetime outcomes over a 40-year horizon. Cost-utility analyses incorporated direct healthcare costs, complication utilities. Uncertainties were addressed using one-way and probabilistic sensitivity analyses. Scenario analyses explored pricing thresholds and adher

Injection Site Reaction to Various Insulins as Type III Allergy With Urticarial Vasculitis in a Patient With Type I Diabetes Mellitus.

Injection site reactions are defined as skin reactions at the injection site to drugs administered subcutaneously. Pathophysiologically, these reactions are based on different immunological mechanisms. We report the case of a 49-year-old patient with type 1 diabetes mellitus (first diagnosis in 1994 at the age of 23 years). Continuous subcutaneous insulin infusion using an insulin pump has been used for many years. The patient presented to the department of dermatology with progressive symptoms in the area of the insulin injection sites on the lower abdomen, accompanied by pain, burning, eryth

Insulin secretion, adipocyte insulin binding and insulin sensitivity in thyrotoxicosis.

The pattern of insulin secretion following an oral glucose load and the insulin receptor status and insulin sensitivity of adipocytes have been studied in patients with thyrotoxicosis and in matched controls. Thyrotoxic subjects showed normal basal and peak levels of serum immunoreactive insulin (peak, 69.0 +/- 6.8 vs 54.3 +/- 8.8 mU/l) and serum C-peptide (peak, 1.95 +/- 0.13 vs 1.71 +/- 0.12 nmol/l for thyrotoxic and control subjects, respectively). Peak serum proinsulin was higher in the thyrotoxic group (64.8 +/- 7.3 vs 39.0 +/- 3.7 pmol/l; P less than 0.01). Maximum specific insulin bindi

Major adverse cardiovascular events with basal insulin peglispro versus comparator insulins in patients with type 1 or type 2 diabetes: a meta-analysis.

BACKGROUND: To identify possible differences in cardiovascular (CV) risk among different insulin therapies, we performed pre-specified meta-analyses across the clinical program for basal insulin peglispro (BIL), in patients randomized to treatment with BIL or comparator insulin [glargine (IG) or NPH]. METHODS: One phase 2 (12-week) and 6 phase 3 (26 to 78-week) randomized studies of BIL compared to IG or NPH, in patients with type 1 or type 2 diabetes, were included. The participants were diverse with respect to demographics, baseline glycemic control, and concomitant disease or medications,

Diabetic control in patients treated with once or twice-daily insulin injections, including a comparison of conventional beef and highly purified pork insulins.

Twenty patients were changed from a single daily injection of beef insulin (a mixture of soluble and protamine zinc insulin) to two daily injections (mixtures of soluble and isophane insulins). This was associated with a reduction, one month later, in the concentration of glycosylated haemoglobin (HbA1) and in the degree of late evening glycosuria. A reduction was shown 6 months later in antibody binding of beef and pork insulin by serum. Subsequent conversion to a twice daily regime of highly purified pork insulin was not associated with further improvement in diabetic control, but was associ

Estradiol Binds to Insulin and Insulin Receptor Decreasing Insulin Binding in vitro.

RATIONALE: Insulin (INS) resistance associated with hyperestrogenemias occurs in gestational diabetes mellitus, polycystic ovary syndrome, ovarian hyperstimulation syndrome, estrogen therapies, metabolic syndrome, and obesity. The mechanism by which INS and estrogen interact is unknown. We hypothesize that estrogen binds directly to INS and the insulin receptor (IR) producing INS resistance. OBJECTIVES: To determine the binding constants of steroid hormones to INS, the IR, and INS-like peptides derived from the IR; and to investigate the effect of estrogens on the binding of INS to its recept

Emulating a Randomized Controlled Trial of Long-Acting Insulins and Cardiovascular Events Using Real-World Data for Patients With Type 2 Diabetes.

AIMS: Randomized controlled trials (RCTs) have high internal validity but often have limited generalizability. To our knowledge, there have been no previous studies emulating RCTs using real-world data to evaluate the risk of major adverse cardiovascular events (MACE) among patients with type 2 diabetes mellitus (T2DM) treated with long-acting insulin analogues. METHODS: We emulated the DEVOTE trial of insulin degludec vs. glargine among patients with T2DM using data from the United Kingdom's Clinical Practice Research Datalink. DEVOTE eligible and ineligible subpopulations were created. Cox

Safety and Effectiveness of Glargine 300 U/ml After Switching from Basal Insulins in Patients with Type 1 Diabetes: COMET-T Study.

INTRODUCTION: Appropriate glycemic control is paramount for people with type 1 diabetes (PwT1D) by the effective delivery of exogenous insulin. However, glycemic variability and the risk of severe hypoglycemia must be reliably controlled. METHODS: COMET-T is a prospective, multicenter, observational study conducted in Germany, Austria, and Switzerland during 2021-2022 to assess the effectiveness and safety of insulin glargine 300 U/ml (Gla-300) after switching from other basal insulins. Out of 135 PwT1D, data of 94 patients were analyzed. The primary endpoint was the change in time in range (

The Comparative Dosing and Glycemic Control of Intermediate and Long-Acting Insulins in Adult Patients With Type 1 and 2 Diabetes Mellitus.

OBJECTIVE: The various basal insulin products possess differences in pharmacokinetics that can significantly impact glycemic control and total daily basal insulin dosing. In addition, there will be instances where transitions between the different long-acting insulins will need to be made. Because every basal insulin product is not interchangeable on a 1:1 unit-to-unit basis, it is important for health care providers to understand the expected dose adjustments necessary to maintain a similar level of glycemic control. DATA SOURCES: A Medline and Web of Science search was conducted in Septembe

Insulin receptor antibodies and insulin resistance.

The presence of insulin receptor antibodies is a rare cause of insulin resistance. Patients usually have a combination of hyperglycemia, insulin resistance, acanthosis nigricans, and autoimmune features. We report a patient with systemic lupus erythematosus and severe insulin resistance due to insulin receptor antibodies. The most striking aspect of the clinical presentation is the resistance to insulin therapy, with our patient unresponsive to doses of up to 154,075 units in a day. While on a low-dose glucocorticoid therapy, the patient had clinical improvement, and glucose levels subsequentl

Insulin receptor autoimmunity and insulin resistance.

The frequency of insulin receptor autoantibodies (IR-ab) was determined among adolescents and young adults with documented insulin resistance syndrome (IRS) with and without concomitant autoimmunity. The study population was comprised of 61 patients with obesity, acanthosis nigricans and insulin resistance (simple IRS); 12 with IRS and other autoimmune problems (lupus erythematosus, rheumatoid arthritis, dermatomyositis) (type B insulin resistance); six with autoimmune polyglandular syndrome type 2; and 40 healthy controls. Using our newly developed radiobinding assay system, we found no contr
