Reproductive Life Planning and Contraceptive Action Planning for Privately Insured Women: The MyNewOptions Study.

CONTEXT: Although reproductive life planning (RLP) is recommended in federal and clinical guidelines and may help insured women make personalized contraceptive choices, it has not been systematically evaluated for effectiveness. METHODS: In 2014, some 984 privately insured women aged 18-40 who were not intending to become pregnant in the next year were randomly assigned to receive RLP, RLP with contraceptive action planning (RLP+) or information only (the control group). Women's contraceptive use, prescription contraceptive use, method adherence, switching to a more effective method, method s

Living Liver Donation Does not Significantly Affect Long-Term Life, Disability, or Medical Insurability.

INTRODUCTION: The growing practice of living liver donation requires comprehensive understanding of the financial implications for living liver donors. While obtaining and maintaining insurance is important to financial health, little is known about the impact of liver donation on future insurability. RESEARCH QUESTIONS: The purpose of this study was to evaluate the donors' experiences with insurance following donation and identify the insurance provider-driven factors that contribute to donor insurability. DESIGN: A two center cohort of living donors with donation between January 2000 and D

Quality insurance in head and neck cancer multidisciplinary team meetings: A watchful eye on real-life experience.

INTRODUCTION: Although Multidisciplinary Team Management (MDT) is integrated in most international head and neck cancer treatment guidelines, its applications and proceedings were rarely described. The present study explores a 6-year real-life experience in a French Comprehensive Cancer Care Center. METHODS: Patients, tumor and meeting characteristics of all consecutive cases discussed in head and neck MDT meetings between 2010 and 2015 were retrospectively reviewed. RESULTS: From 2010 to 2015, 1849 cases (accounting for 1786 patients) were discussed in 138 MDT meetings. Median age was 62 (r

Ophthalmic complications in retinopathy of prematurity in the first decade of life in Korea using the national health insurance database.

The aim of this study is to investigate the epidemiology of ophthalmic complications of retinopathy of prematurity (ROP) after preterm birth using population-based database in South Korea. Using the National Health Insurance database, ophthalmic complications among premature infants born in 2007-2008 during their 10-year follow-up period were identified. Annual cumulative incidence rate and period prevalence of complications at each age were analyzed among those with ROP and those who underwent treatment for ROP (tROP). The hazard ratios (HRs) according to the presence of ROP and treatment for

[Life prognosis of persons with dementia: Observation of persons registered for long-term care insurance in a community in Japan].

Objectives This study aimed to observe the life prognosis of persons with dementia during the first five years after registering for long-term care insurance, and to determine the factors affecting their prognosis. Methods A total of 556 persons, aged 65 years or older newly registered for long-term care insurance in a city in Japan, were observed. The life prognosis of the registered persons was determined using standardized mortality ratio (SMR) with the general population as a standard, and relevant factors were observed in terms of dependence indices of dementia and disability. Results The m

End-of-Life Care for Older Adults With Blood Cancers With Medicare Advantage Versus Medicare Fee-For-Service Insurance.

PURPOSE: Adults with hematologic malignancies (HMs) often experience suboptimal end-of-life (EOL) care, with patients from minoritized racial/ethnic groups at even greater risk. It is unclear whether these disparities are partly driven by modifiable factors such as insurance. METHODS: Using the Centers for Medicare and Medicaid Services database, we compared the quality of EOL care as defined by hospice use, high-intensity health care utilization, and advance care planning between Medicare advantage (MA) and Medicare fee-for service (FFS) insurance among patients with HM 66 years and older wh

Long-Term Care Insurance and Catastrophic Health Spending at the End of Life Among Older Adults: Evidence From China.

End-of-life (EOL) health spending imposes catastrophic financial burdens on elderly households, pushing them into poverty without corresponding improvements in health outcomes. Leveraging the quasi-experiment of China's public long-term care insurance (LTCI) pilot program, this study employs data from the China Longitudinal Healthy Longevity Survey to examine the impact of LTCI on catastrophic health spending (CHS) at the end of life. The findings indicate that over the 5-year EOL period, LTCI significantly decreases the probability of CHS by approximately 36.1-52.3 percentage points and reduc

BACKGROUND: People with mental illness have a reduced life expectancy, but the extent of the mortality gap and the contribution of natural and unnatural causes to excess mortality among people with mental illness in South Africa are unknown. METHODS: We analysed reimbursement claims from South African medical insurance scheme beneficiaries aged 15-85 years. Investigating genetic discrimination in the Australian life insurance sector: the use of genetic test results in underwriting, 1999-2003.

A major component of the Genetic Discrimination Project (GDP), an Australia-wide study to examine the advantages and disadvantages for individuals of having genetic information and cases of alleged genetic discrimination, is the analysis of insurers' use of genetic test results. The peak life insurance body, IFSA, had collected data through the Australian Institute of Actuaries (AIA) for the period June 1999-May 2003 from life insurance companies in Australia regarding their use of genetic test results in insurance underwriting. The GDP negotiated with IFSA and the AIA for access to this data

Comparison of Overall and Comorbidity-Free Life Expectancy Between Insured Adults With and Without HIV Infection, 2000-2016.

IMPORTANCE: Antiretroviral therapy (ART) has improved life expectancy for individuals with HIV infection, but recent data comparing life span and comorbidity-free years by HIV status are lacking. OBJECTIVE: To quantify the gap in life span and comorbidity-free years by HIV status among adults with access to care. DESIGN, SETTING, AND PARTICIPANTS: This matched cohort study used data from insured adults with and without HIV infection (aged ≥21 years) matched 1:10 at medical centers of Kaiser Permanente in northern and southern California and the mid-Atlantic states of Washington DC, Maryland,

Real-life practices for preventing venous thromboembolism in multiple myeloma patients: a cohort study from the French health insurance database.

PURPOSE: The risk of venous thromboembolic event (VTE) in multiple myeloma is particularly increased. Current guidelines recommend systematic VTE prophylaxis with vitamin K antagonists (VKA) or low weight molecular heparin (LWMH) or unfractionated heparin (UFH) in high-risk patients, based on treatment received [e.g. use of IMiDs (thalidomide, lenalidomide and pomalidomide), alkylating agents or erythropoietin] and individual risk factors (e.g. history of VTE). The aim of this study was to describe strategy of VTE prophylaxis and prescribing of other antithrombotic agents during the first 6 mo

Insurance status and health-related quality-of-life disparities after trauma: results from a nationally representative survey in the US.

PURPOSE: To determine whether insurance status modifies the association between injuries and health-related quality of life (HRQOL) in a nationally representative sample of US adults. METHODS: This is a longitudinal, observational study using the pooled 2000-2006 Medical Expenditure Panel Survey (MEPS). A total of 50,225 adults (age ≥ 18) with or without injuries were included in the study. HRQOL was evaluated using the EuroQoL Health Index (EQ-5D), visual analog scale (VAS), and the SF-12 physical component score (PCS) and mental component score (MCS).
