Estimated expenditures for hip fractures using merged healthcare insurance data for individuals aged ≥ 75 years and long-term care insurance claims data in Japan.

UNLABELLED: Little is known about hip fracture expenditure in Japan. Using claims data obtained from a core city near Tokyo, we estimated the mean healthcare expenditure and monthly long-term care expenditure post-hip fracture to be ¥2,600,000 (US$29,500) and ¥113,000 (US$1290), respectively. PURPOSE: We aimed to estimate healthcare and long-term care expenditures post-hip fracture in Japan. METHODS: Healthcare insurance claims data for adults aged ≥  75 years were merged with long-term care insurance claims data. We analyzed the data of hip fracture patients who were admitted to non-diagnos

Long-term care insurance pilots and a severe care-gap proxy among older adults in China: evidence from CHARLS, 2011-2020.

BACKGROUND: China's long-term care insurance (LTCI) pilots aim to support older adults, but evidence on whether policy rollout is followed by reduced care gaps among functionally limited older adults remains limited. METHODS: Using CHARLS 2011-2020, we included respondents aged 60 years or older at first observation and estimated a staggered difference-in-differences model comparing early-adopter pilot cities with never-pilot cities. The primary outcome was a severe care-gap proxy, defined as ADL/IADL difficulty with no help. This estimand describes care-gap trajectories among older adults wi

A scoring system and seven factors associated with certification for Japanese long-term care insurance in older people.

INTRODUCTION: The increase in the older population is a serious concern in developed countries, and how to maintain independence of these individuals is now an urgent issue. Various factors are known to put older people at risk for needing long-term care, but it is not clear to what extent each factor is associated with that need. MATERIALS AND METHODS: In a cohort of 1577 community-dwelling older persons, we excluded 40 persons whose long-term care insurance certification was unknown and then divided the remaining 1537 into two groups: dependent group (134 persons) certified as requiring ass

Assessing the role of long-term care insurance in shaping living arrangements of older adults: evidence from China.

The establishment of long-term care insurance (LTCI) has become one of the key measures adopted by countries worldwide to address growing care needs associated with population aging. While existing studies focus on LTCI's impact on medical expenditure and healthcare utilization, its effects on living arrangements of older adults, a core component of long-term care, remain underexplored in China. Living arrangements reflect both the well-being of older adults and their approaches to aging. This study examines the effects of China's LTCI pilots on living arrangements of older adults. Using three

Institutionalizing family care responsibility in China's multilevel medical security system: evidence from long-term care insurance policy texts through a Confucian care ethics lens.

BACKGROUND: China's long-term care insurance reform is a key component of multilevel medical security for addressing care needs among older adults with substantial functional decline, and it also raises an ethical governance question: how family-based Confucian care responsibilities are translated into modern rules of eligibility, payment, service provision and regulation. METHODS: This study analyzed core long-term care insurance policy texts from 48 pilot cities through directed content analysis, a 45-item coding framework, policy text indices, K-means clustering and a supplementary compari

Long-term care insurance and accessibility of home and community-based services for older adults: evidence from China.

With population aging, the establishment of universal long-term care insurance (LTCI) has emerged as a critical policy issue. This paper examines the effects of China's LTCI pilots on the physical accessibility of home and community-based services (HCBS) and specific services for older adults. Using three-wave panel data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS), we analyze the rollout of LTCI pilots across different cities from 2014 to 2021, employing a time-varying difference-in-differences (DID) approach. Our findings indicate that LTCI significantly improves access to

The effect of a subsidy policy on the utilization of community care services under a public long-term care insurance program in rural Japan.

This cross-sectional study investigated the effect of an interim subsidy policy for low-income beneficiaries to buy home help services under Japanese long-term care insurance (LTCI) for the service utilization of home help and other community care services. Claims records during the period from October to December 2001 were reviewed to compare the monthly amounts for each service and out-of-pocket copayment for subsidized (n = 137) and non-subsidized (n = 124) beneficiaries. A multivariable linear regression analysis was conducted to control for age, gender, household size, length of care, and

Does Social Support Alleviate the Caregiving Burden of Adult Children? Evidence from Chinese Long-Term Care Insurance Pilot Program.

In China, and many other developed nations, public long-term care insurance (LTCI) is a commonly adopted approach to meet long-term care needs, but its impact on the burden of family caregivers remains uncertain. This study investigated whether a parent having LTCI alleviates the caregiver burden for the adult child caregiver. Data derived from the 2011, 2013, and 2018 China Health and Retirement Longitudinal Study (N = 4595 adult child caregivers). Guided by the stress-appraisal model, Difference-in-Difference (DID) methods were used to investigate the spillover effects of LTCI on caregiver s

Older adults' adoption of shared service platforms outside long-term care insurance in Japan based on technology acceptance model.

The services that are not covered by the long-term care insurance play a critical role in supplementing the public care system in the aging society of Japan. Their combination with technologies of the shared economy is not only an extension of the shared economy into the specialized service market, but it is also a natural demand of the market in the long-term care service market that is the necessity to innovate and develop. This study constructed a technology acceptance extension model, using the Tokyo area as the survey scope. 486 valid samples were collected through a questionnaire survey,

Long-Term Care Insurance and Catastrophic Health Spending at the End of Life Among Older Adults: Evidence From China.

End-of-life (EOL) health spending imposes catastrophic financial burdens on elderly households, pushing them into poverty without corresponding improvements in health outcomes. Leveraging the quasi-experiment of China's public long-term care insurance (LTCI) pilot program, this study employs data from the China Longitudinal Healthy Longevity Survey to examine the impact of LTCI on catastrophic health spending (CHS) at the end of life. The findings indicate that over the 5-year EOL period, LTCI significantly decreases the probability of CHS by approximately 36.1-52.3 percentage points and reduc

Relationship Between Medical Therapy, Long-Term Care Insurance, and Comorbidity in Elderly Patients With Heart Failure With Systolic Dysfunction.

BACKGROUND: Although guideline-directed medical therapy (GDMT), including β-blockers, angiotensin-converting enzyme inhibitors (ACEi)/angiotensin receptor blockers (ARBs), and mineralocorticoid receptor antagonists (MRAs), improves survival and quality of life, most patients with heart failure with reduced (HFrEF) and mildly reduced (HFmrEF) ejection fraction are treated with inadequate medications. We investigated the prescription patterns of GDMT in elderly patients with HFrEF and HFmrEF and their characteristics, including the certification of long-term care insurance (LTCI), which represen

Decrease in the usual walking speed and body fat percentage associated with a deterioration in long-term care insurance certification levels.

BACKGROUND: In Japan, the number of older adults requiring long-term care insurance (LTCI) is increasing and the cost is becoming a social problem. In these fields, the role of geriatric rehabilitation includes maintaining the physical function and LTCI certification levels. The prevalence of sarcopenia is high among older adults requiring LTCI certification, and there are many opportunities to assess the handgrip strength, walking speed, and muscle mass. This study aimed to identify sarcopenia-related assessments sensitive to transitions in LTCI certification levels and determine cut-off valu

A scoping review of "nursing-technology collaboration" in home-based rehabilitation for the elderly under the long-term care insurance system.

BACKGROUND: Under the long-term care insurance system, home-based rehabilitation is a key service model for addressing population aging, but it faces the dual challenges of a shortage of care workers and increasingly complex service needs. The introduction of emerging tools, such as digital health technologies and social assistive robots, has opened up new possibilities for home-based rehabilitation. However, whether technology can truly improve care outcomes depends on the collaborative working model between nurses and technology-known as "nurse-technology collaboration." There is currently a
