Challenging behaviour in patients with intellectual disability may be caused by physical, psychological and environmental factors. In 3 case histories, a multidisciplinary assessment in diagnosis and treatment is shown to be needed. A 48-year-old man with mild intellectual disability presented with cognitive deterioration, aggressive behaviour and physical symptoms. The diagnosis was mood disorder and dissociative symptoms. His functioning improved during and after treatment with psychotropic drugs and non-verbal psychotherapy. A 10-year-old girl with moderate intellectual disability who prese

X-linked intellectual disability (XLID) affects 1% to 3% of the population. XLID subsumes several heterogeneous conditions, all of which are marked by cognitive impairment and reduced adaptive skills. XLID arises from mutations on the X chromosome; to date, 102 XLID genes have been identified. The proteins encoded by XLID genes are involved in higher brain functions, such as cognition, learning and memory, and their molecular role is the subject of intense investigation. Here, we review recent findings concerning a representative group of XLID proteins: the fragile X mental retardation protein

BACKGROUND: Personal narratives are conditional for victims of sexual abuse to overcome their trauma. Counsellors can help victims with intellectual disability to take an active position in conversations about sexuality and to co-construct a personal narrative.
METHOD: Using discourse and conversational analysis, we studied 4 conversations between a counsellor and a woman with autism and mild intellectual disability.
RESULTS: In conversation with a counsellor the participant was able to express her inner emotions and understanding about sexual issues and other sensitive topics. She was able

BACKGROUND: There is a growing interest in the use of cognitive approaches for the treatment of psychological difficulties with people who have intellectual disability. The aim of the present study was to investigate the properties of an anger inventory, the relationship between self-reported and staff-reported levels of anger, and the relationship between different staff measures.
METHOD: Two studies are described that use self-report and staff-report measures. The first study employs a cross sectional design and the second, a longitudinal design.
RESULTS: No relationship was found between

Understanding positive, as well as negative, impact of a child with mental retardation will lead to a more balanced view of families and disability. In two studies we examined parents' perceived positive impact of a child with MR/DD. Study 1 involved 282 young adults with severe mental retardation; Study 2 involved 214 young children with, or without, developmental delays. In both studies, positive impact was inversely related to behavior problems. Moreover, positive impact moderated the relationship between behavior problems and parenting stress. Also, main and moderating effects of positive

CONTEXT: Comorbid severe mental health problems complicating intellectual disability are a common and costly public health problem. Although these problems are known to begin in early childhood, little is known of how they evolve over time or whether they continue into adulthood.
OBJECTIVE: To study the course of psychopathology in a representative population of children and adolescents with intellectual disability.
DESIGN, SETTING, AND PARTICIPANTS: The participants of the Australian Child to Adult Development Study, an epidemiological cohort of 578 children and adolescents recruited in 199

There is an increasing focus on the nutrition of people with intellectual disability (ID), but less interest in the range of eating disorders (EDs) that they may exhibit and the bio-psycho-social impact of these conditions. Despite diagnostic and methodological difficulties, psychopathology and ED research studies suggest that 3-42% of institutionalized adults with ID and 1-19% of adults with ID in the community have diagnosable EDs. Weight surveys indicate that 2-35% of adults with ID are obese and 5-43% are significantly underweight, but the contribution of diagnosable EDs is unknown. Such d

Determinants of (1) referral to psychiatric services and (2) the amount of mental health care consumed were analysed in a population of individuals with intellectual disability, using data from a cumulative mental health case register in a defined geographical area. Associations between level of disability, gender, age and social environment on the one hand, and psychiatric referral and service consumption on the other were expressed as odds ratios (ORs). Being older (OR = 1.9; 95% CI = 1.5-2.5), having milder intellectual disability (OR = 1.4; 95% CI = 0.9-2.3) and living alone (OR = 5.8; 95%

BACKGROUND: Crises in people with intellectual disabilities often occur at the intersection of mental health care (MHC) and intellectual disability care (IDC). Different actors and crisis regulations with their own definitions and agreements make appropriate care challenging. Effective collaboration between MHC and IDC is essential but often not structurally organized.
AIM: To distinguish between different types of crises in order to promote shared perceptions and a common language between MHC and IDC. This will improve the coordination of interventions and make cooperation between profession

It is time for people with intellectual/developmental disabilities (IDD), their families, the village that supports them, and the healthcare professionals that are responsible for their well-being to grab their pitchforks and storm the castle to end the purposeful exclusion by our government to designate them a medically underserved population (MUP). We (the authors) define IDD as people having a diagnosis of autism, cerebral palsy, Down syndrome, and hundreds of other genetic syndromes.

We examine the retirement of workers with intellectual and developmental disabilities (IDD) in developed countries, with research predominantly from North America and Australia. We propose that retirement involves withdrawal from paid work just as in the general community. Most research is on sheltered employment. A few recent studies consider retirement from mainstream work. We examine factors that affect retirement and decision making. We consider retirement pathways and decision-making processes, including education and planning. Workers with IDD have limited knowledge about retirement but

BACKGROUND: An estimated 1.5 million persons in Germany are intellectually disabled. Persons with intellectual disability (ID) are especially vulnerable to somatic and mental illnesses.
METHODS: This review is based on pertinent literature retrieved by selective searches in PubMed and the Cochrane Library.
RESULTS: Genetic abnormalities are a frequent cause of diseases that affect multiple organs and need interdisciplinary treatment. A number of somatic diseases are more common in persons with ID than in the general population, including epilepsy (30-50% in persons with severe or very severe

People with intellectual disabilities (PWIDs) are now living longer; thus, the incidence of cancer within this population is increasing. Available data indicate an excess of digestive tract cancers in PWIDs, but colorectal cancer has rarely been specifically studied and has not been extensively reviewed. This is despite risk factors such as being overweight, obesity, and lack of exercise being more frequent in PWIDs. In this article, we examine the literature on the frequency, screening, and treatment of colorectal cancer in PWIDs by as sessing 4 databases, Medline, EBSCO-CINHL, ASSIA, and Psy

BACKGROUND: People with intellectual disability have the same sexual needs as those without any disability, yet their sexuality is often restricted by reluctant attitudes and/or fears based on irrational beliefs. The aim of this study is to describe and analyse different areas of sexuality in adults with mild or moderate intellectual disability.
METHOD: The sample consisted of 180 men and 180 women attending occupational centres. All the participants were administered a questionnaire about sexuality adapted to their characteristics.
RESULTS: Results show that 84.2% have had sexual relationsh

The ingestion of one's own feces is referred to as coprophagia. In this report, the author discusses a case of coprophagia-associated death in a 36-year-old woman. The woman had a history of intellectual disability, bipolar disorder, and schizoaffective disorder and was living in a group home. At autopsy, examination of the upper airway revealed 4 distinct spherical boli of impacted fecal matter in the oral cavity and pharynx. Death was attributed to asphyxia due to aspiration of stool bolus during an episode of coprophagia. This is now the second reported case of coprophagic-related death, il

BACKGROUND: People with intellectual disabilities (IDs) die at younger ages than the general population, but nationally representative and internationally comparable mortality data about people with ID, quantifying the extent and pattern of the excess, have not previously been reported for England.
METHOD: We used data from the Clinical Practice Research Datalink database for April 2010 to March 2014 (CPRD GOLD September 2015). This source covered several hundred participating general practices comprising roughly 5% of the population of England in the period studied. General practitioner (GP)

BACKGROUND: This study aimed to evaluate the validity and reliability of the adult self-report and proxy version of the Trauma Screener-Intellectual Disability (TS-ID) in adults with mild intellectual disability or borderline intellectual functioning (MID-BIF). An optimal cut-off value was determined for the ratio of specificity to sensitivity for predicting the diagnosis of post-traumatic stress disorder (PTSD).
METHODS: The TS-ID was adapted from a Dutch Child and Adolescent Trauma Screener, for use with adults with MID-BIF. Outcomes based on the TS-ID were compared with the presence of PTS

Tourette syndrome (TS) is a neuropsychiatric and a neurodevelopmental disorder often presenting alongside other neurodevelopmental conditions such as attention deficit/hyperactivity disorder (ADHD), autism spectrum disorder (ASD), learning difficulties, and dyspraxia. TS shares genetic and neurobiologic features with ASD and ADHD, suggesting common underlying mechanisms. Recent diagnostic frameworks such as the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, APA, 2013) allow for multiple co-occurring diagnoses, reflecting the complex overlap among neurodevelopmenta
