Patient-Ventilator Interaction During Noninvasive Ventilation.

Patient-ventilator discordance commonly occurs in patients receiving noninvasive ventilation (NIV). Unintentional leaks contribute to trigger and cycle asynchrony. Historically, ventilators designed for NIV with an intentional leak circuit compensated for unintentional leaks better than critical care ventilators. However, many critical care ventilators now have NIV settings for leak compensation. Per bench studies and some clinical studies, NIV settings for leak compensation variably improve synchrony related to leak. Some interfaces, such as the helmet, can contribute to asynchrony. Effects of Neurally Adjusted Ventilation Assist (NAVA) and conventional modes of mechanical ventilation on diaphragm functions: A randomized controlled trial.

BACKGROUND: Better patient-ventilator interactions, during neurally adjusted ventilator assist (NAVA), are expected to result in lower rate of diaphragm dysfunction. OBJECTIVE: This study was planned to compare the rate of diaphragm dysfunction between the conventional modes of mechanical ventilation and NAVA. METHODS: Patients who were initiated on mechanical ventilation for acute respiratory failure were randomized to continue either conventional mode of mechanical ventilation or switched to NAVA. The rates of diaphragm dysfunction were compared between two groups. RESULTS: This study inc

Patient-Ventilator Interaction During Noninvasive Ventilation in Subjects With Exacerbation of COPD: Effect of Support Level and Ventilator Mode.

BACKGROUND: Patient-ventilator synchrony in patients with COPD is at risk during noninvasive ventilation (NIV). NIV in neurally-adjusted ventilatory assist (NAVA) mode improves synchrony compared to pressure support ventilation (PSV). The current study investigated patient-ventilator interaction at 2 levels of NAVA and PSV mode in subjects with COPD exacerbation. METHODS: NIV was randomly applied at 2 levels (5 and 15 cm H2O) of PSV and NAVA. Patient-ventilator interaction was evaluated by comparing airway pressure and electrical activity of the diaphragm waveforms with automated computer alg

Neurally adjusted ventilatory assist compared to other forms of triggered ventilation for neonatal respiratory support.

BACKGROUND: Effective synchronisation of infant respiratory effort with mechanical ventilation may allow adequate gas exchange to occur at lower peak airway pressures, potentially reducing barotrauma and volutrauma and development of air leaks and bronchopulmonary dysplasia. During neurally adjusted ventilatory assist ventilation (NAVA), respiratory support is initiated upon detection of an electrical signal from the diaphragm muscle, and pressure is provided in proportion to and synchronous with electrical activity of the diaphragm (EADi). Compared to other modes of triggered ventilation, thi

Combined use of Neurally Adjusted Ventilatory Assist (NAVA) and Vertical Expandable Prostethic Titanium Rib (VEPTR) in a patient with Spondylocostal dysostosis and associated bronchomalacia.

Jarcho-Levin syndrome is a rare disorder characterised by defects in vertebral and costal segmentation of varying severity. Respiratory complications are the main cause of death or severe comorbidity due to a restrictive rib cage. A 3 months old infant with Spondylocostal dysostosis and associated bronchomalacia experiencing severe asynchrony during the weaning process is reported. The Neurally Adjusted Ventilatory Assist (NAVA) mode was used to improve adaptation to mechanical ventilation after Vertical Expandable Prosthetic Titanium Ribs (VEPTRs) were implanted. The synchrony achieved with t

Neurally adjusted ventilatory assist in weaning of neonates affected by congenital diaphragmatic hernia.

OBJECTIVE: The aim of the study is to evaluate the application of neurally adjusted ventilatory assist (NAVA) in the respiratory weaning of patients affected by congenital diaphragmatic hernia (CDH). METHODS: We analyzed the NAVA weaning in 12 neonates affected by CDH, relating the effectiveness of the electrical activation of the diaphragm (EAdi) signal to the type of CDH repair (with or without patch), the size of the patch, the stomach and His angle position, and the trend evaluation of some cardiorespiratory parameters with NAVA compared to pressure-support-ventilation (PSV). NAVA use in infants with established bronchopulmonary dysplasia, congenital diaphragmatic hernia, and those on ECMO: a narrative literature review.

Neurally Adjusted Ventilatory Assist (NAVA) is an innovative mode of ventilation that delivers support in synchrony with the infant's diaphragmatic electrical activity. This technology aims to improve patient-ventilator interaction, making its use in neonates with complex respiratory disease appealing. The use of NAVA in preterm infants is becoming more commonplace, but the specific role of NAVA in infants with established bronchopulmonary dysplasia (BPD), congenital diaphragmatic hernia (CDH), and those requiring extracorporeal membrane oxygenation (ECMO) support remain less clearly defined.

Non-invasive neurally adjusted ventilatory assist in preterm infants with RDS: effect of changing NAVA levels.

We aimed to examine the effect of changing levels of support (NAVA level) during non-invasive neurally adjusted ventilatory assist (NIV-NAVA) in preterm infants with respiratory distress syndrome (RDS) on electrical diaphragm activity. This is a prospective, single-centre, interventional, exploratory study in a convenience sample. Clinically stable preterm infants supported with NIV-NAVA for RDS were eligible. Patients were recruited in the first 24 h after the start of NIV-NAVA. Following a predefined titration protocol, NAVA levels were progressively increased starting from a level of 0.5 cm

Neurally-Adjusted Ventilatory Assist Can Facilitate Extubation in Neonates With Congenital Diaphragmatic Hernia.

BACKGROUND: Congenital diaphragmatic hernia is associated with a high risk of neonatal mortality and long-term morbidity due to lung hypoplasia, pulmonary hypertension, and prolonged exposure to positive-pressure ventilation. Ventilator-associated lung injury may be reduced by using approaches that facilitate the transition from invasive ventilation to noninvasive ventilation (NIV), such as with neurally-adjusted ventilatory assist (NAVA). We reported our use of NAVA in neonatal patients with congenital diaphragmatic hernia during the transition from invasive ventilation to NIV. METHODS: A re

Effective Neurally Adjusted Ventilatory Assist (NAVA) Ventilation in a Child With Jeune Syndrome.

Jeune syndrome (asphyxiating thoracic dystrophy) is a rare skeletal dysplasia mainly characterized by dystrophy of the thoracic cage. Neurally adjusted ventilatory assist (NAVA) is a respiratory support in which pressure assistance is provided in proportion to and synchronous with the electrical activity of the diaphragm. We present the case of a 4-month-old infant with asphyxiating thoracic dystrophy and respiratory failure successfully ventilated with NAVA. In this case, NAVA improved patient-ventilator synchrony, reducing endotracheal secretion and gastric overdistention. The reduction of b

Assessment of patient-ventilator breath contribution during neurally adjusted ventilatory assist in patients with acute respiratory failure.

INTRODUCTION: We previously showed in animals that the ratio of inspired tidal volume (Vtinsp) to inspiratory peak electrical activity of the diaphragm (EAdipk) can be used to quantify the respective patient and ventilator breath contributions (PVBCs) during neurally adjusted ventilatory assist (NAVA). The PVBC index has not been tested clinically. METHODS: We studied 12 intubated and mechanically ventilated patients with acute respiratory failure and measured EAdipk, airway (Paw) and inspiratory esophageal pressure (Pes) and Vtinsp. We applied 11 different NAVA levels, increasing them every

The Esophageal Pressure-Guided Ventilation 2 (EPVent2) trial protocol: a multicentre, randomised clinical trial of mechanical ventilation guided by transpulmonary pressure.

INTRODUCTION: Optimal ventilator management for patients with acute respiratory distress syndrome (ARDS) remains uncertain. Lower tidal volume ventilation appears to be beneficial, but optimal management of positive end-expiratory pressure (PEEP) remains unclear. The Esophageal Pressure-Guided Ventilation 2 Trial (EPVent2) aims to examine the impact of mechanical ventilation directed at maintaining a positive transpulmonary pressure (PTP) in patients with moderate-to-severe ARDS. Synchronized mechanical ventilation using electrical activity of the diaphragm in neonates.

The electrical activity of the diaphragm (Edi) is measured by a specialized nasogastric/orogastric tube positioned in the esophagus at the level of the crural diaphragm. Neurally adjusted ventilatory assist (NAVA) uses the Edi signal as a neural trigger and intrabreath controller to synchronize mechanical ventilatory breaths with the patient's respiratory drive and to proportionally support the patient's respiratory efforts on a breath-by-breath basis. NAVA improves patient-ventilator interaction and synchrony even in the presence of large air leaks, and might therefore be an optimal option fo

Evolution of diaphragmatic thickness and function in preterm infants on non-invasive neurally adjusted ventilatory assist.

UNLABELLED: Non-invasive neurally Adjusted Ventilatory Assist (NIV-NAVA) is respiratory support that is synchronized with the Electrical activity of the Diaphragm (Edi) and uses the Edi to proportionally assist breathing by unloading the diaphragm. The aim of the study was to evaluate the effect of this diaphragm-triggered ventilation on the diaphragmatic structure and function. This observational cohort study aimed to track changes in diaphragm thickness (DT), measured with ultrasound, and function in preterm infants receiving NIV-NAVA. Diaphragmatic ultrasound was performed to determine diap

Positioning of Diaphragm Electrical Activity Catheter in Premature Infants.

BACKGROUND: Neurally-adjusted ventilatory assist (NAVA) is a mode of ventilation that uses the electrical activity of the diaphragm (EAdi) to synchronize and to control delivered pressure. EAdi is measured with a naso-/orogastric feeding tube, containing an array of sensors. Proper placement of the EAdi catheter is critical for optimal synchronization and support. Positioning the catheter requires (1) predicting the insertion distance and (2) verification using a dedicated positioning window on the ventilator, based on electrocardiogram (ECG) and EAdi signals. In preterm infants, no study has
