A standardised hot debrief process was implemented following predefined critical events, with senior staff trained as facilitators. Staff perceptions were assessed using anonymous pre-, post- and final-implementation surveys, with quantita

An evaluation of the contribution of critical care outreach to the clinical management of the critically ill ward patient in two acute NHS trusts.

This paper reports on an evaluation of the role and contribution of outreach in the management of the critically ill ward patient using Stake's Responsive Model (Stake, 1975) and case study methodology (Simons, 1980). Twenty cases were examined, purposefully sampling all staff involved in the case identified by an initial interview with the outreach nurse. In total, 80 interviews were carried out, 20 with the outreach nurses and 54 with other members of health care teams involved in the cases, and six further targeted in-depth interviews with senior anaesthetic and nursing staff. Peripheral nerve-blocks and associations with length of stay and readmissions in fast-track total hip and knee arthroplasty.

BACKGROUND: Peripheral nerve blocks (PNB) have recently been recommended in total hip (THA) and knee (TKA) arthroplasty as they may reduce pain, morphine consumption, length of stay (LOS) and complications. However, whether PNBs are associated with early discharge within an enhanced recovery protocol including multimodal analgesia is uncertain. METHODS: An observational multicenter study from January to August 2017 in six Danish Arthroplasty Centers with established fast-track protocols. Prospective recording of preoperative characteristics and information on PNB, LOS and readmissions through

Ambulatory Parathyroidectomy for Secondary Hyperparathyroidism at a Large Dialysis Program in Toronto: A Program Report.

PURPOSE OF PROGRAM: Operative wait times for non-oncology-related procedures continue to rise in Canada, and this was further exacerbated by the COVID-19 pandemic. These challenges will remain prevalent beyond the pandemic given the limited number of acute care beds and resources required to care for patients. As a result, the need for innovative approaches to optimize the utilization of health care resources while maintaining equitable and timely access is needed. In this report, we describe the development of a collaborative ambulatory parathyroidectomy program between two centers in Toronto

Factors Associated With Prolonged Emergency Department (ED) Length of Stay (LOS) at a Tertiary Care Hospital in Nepal: A Cross-Sectional Study.

BACKGROUND: Emergency department (ED) length of stay (LOS) is an important indicator of healthcare efficiency and patient flow. The duration of ED stay is influenced by a complex interaction of patient-related, clinical and system-level factors. Prolonged LOS (> 4 h) contributes to ED overcrowding and adverse patient outcomes. However, limited data exist regarding factors influencing ED LOS in Nepal. METHOD: A descriptive cross-sectional study was conducted in the ED of KIST Medical College and Teaching Hospital (MCTH), a tertiary care hospital in Lalitpur, Nepal, from May 15, 2025, to May 30

Perceptions, challenges, and improvement strategies for reducing intravenous infusion use in hospitalised patients within Chinese medical institutions: a qualitative interview study.

BACKGROUND: Intravenous infusions are widely used in inpatient care, yet their overuse may increase safety risks, waste healthcare resources, and impose unnecessary burdens on patients. In China, where infusion use remains high, understanding the factors influencing efforts to reduce unnecessary intravenous infusion use is of clear clinical relevance. This study explored stakeholder perceptions, perceived barriers, and potential strategies for reducing unnecessary intravenous infusion use within the broader framework of standardised and rational infusion use. METHODS: A qualitative study usin

Implementation of a Virtual Hospital in the Home Service for Patients With COVID-19 in Queensland, Australia: Mixed Methods Evaluation Using the RE-AIM Framework.

BACKGROUND: Hospital in the home (HITH) provides home-based care as an alternative to traditional hospitalization. In response to the COVID-19 Omicron wave, a public hospital in the rural Western portion of Southeast Queensland implemented a virtual HITH service to support adults, maternity patients, and children with moderate COVID-19 symptoms and additional health concerns. Although the pandemic accelerated the uptake of virtual care within HITH models, existing literature has focused on clinical outcomes, with limited evidence on key implementation outcomes. OBJECTIVE: Using the RE-AIM (re

Survival outcomes and healthcare utilization between immigrant patients and Danish-born patients with hematological cancers: a Danish population-based study.

Overall survival (OS) for patients with a hematological cancer may differ between immigrant and Danish-born patients due to disparities in socioeconomic status, health literacy, and language proficiency. This cohort study aimed to investigate survival and hospitalization according to immigrant status while controlling for confounders. Patients with newly diagnosed hematological cancer in 2000-2020 were identified in the Danish nationwide hematological registers and stratified into Danish-born, Western, and non-Western patients. Patients were followed from diagnosis until death, 31st December 2
