Interleukin-18 Levels Are Associated With Disease Course in Patients With Still Disease Treated With Interleukin-1 Inhibitors.

OBJECTIVE: To evaluate the prognostic utility of circulating interleukin-18 (IL-18) levels in predicting disease activity, macrophage activation syndrome (MAS), and disease course in patients with Still disease (SD) receiving first-line IL-1 inhibitors (IL-1i). METHODS: We retrospectively analyzed 66 biologic-naive patients with SD who received first-line treatment with IL-1i. Plasma IL-18 levels were measured at baseline and at 3, 6, and 12 months after IL-1i initiation. Associations between IL-18 levels and clinical outcomes were assessed using mixed-effects models, receiver operating chara

Elevated Cutaneous Interleukin-21 Links Eczematous Eruption to Interleukin-17A Inhibitor Treatment in Psoriasis.

BACKGROUND: Eczematous eruption (EE) is an adverse effect observed in psoriasis patients undergoing interleukin (IL)-17A inhibitor therapy, with reported incidence rates ranging from 2.2% to 12.1%. In some cases, this reaction leads to discontinuation of treatment. However, the underlying mechanism of EE development remains unclear. Therefore, we aimed to elucidate the pathogenesis of EE associated with anti-IL-17A treatment and identify pathogenic molecules involved. METHODS: Skin samples were collected from psoriasis patients both before and after anti-IL-17A treatment, and the treated skin

The role of interleukin-17 and interleukin-23 inhibitors in the development, progression, and recurrence of cancer: A systematic review.

BACKGROUND: Biologicals targeting interleukin (IL)-17 and IL-23 improve quality of life in psoriasis and other chronic autoimmune disorders with a favorable safety profile. However, current guidelines do not recommend their use in patients with recent oncologic history due to limited evidence. OBJECTIVE: To understand the impact of IL-17 and IL-23 inhibitors on cancer development, progression, and recurrence by systematically reviewing available literature. METHODS: We conducted a systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.

The Treatment with Interleukin 17 Inhibitors and Immune-Mediated Inflammatory Diseases.

IL-17 inhibitors (IL-17i) are medicines used to treat dermatological and rheumatic diseases They belong to a class of medicines called biological disease-modifying anti-rheumatic drugs (bDMARDs). This class of drugs has had a major impact on the therapy of autoimmune diseases, being much safer and more effective than treatment with small molecules. At the same time, they have highly beneficial effects on skin and joint changes, and their efficacy has been extensively monitored and demonstrated in numerous clinical trials. More and more such drugs are still being discovered today to ensure the

Is It Time to Revisit the Role of Interleukin-1 Inhibitors in Osteoarthritis?

OBJECTIVE: There remains a huge unmet need for new osteoarthritis (OA) therapies. A putative reason for the failure of some therapies has been the absence of well-defined phenotypes, which might be more appropriate for specific targeted treatment. Interleukin-1 (IL-1) plays a key role in the development of OA, but the results of clinical trials targeting IL-1 in OA have to date been disappointing. METHODS: This narrative review is based on a literature search for publications describing interventions with direct Il-1 pathway inhibitors in patients with knee OA and substantiated by a descripti

Role of Interleukin 6 Inhibitors in the Management of Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a multisystem disease that affects the joints and various organs, resulting in compromised quality of life and increased mortality. A wide spectrum of treatment options is available for RA. Conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) are the first-line of treatment for RA, whereas tumor necrosis factor α inhibitors are commonly used as a second-line biological disease-modifying antirheumatic drug following inadequate response to csDMARDs. However, remission remains difficult to achieve. No single agent is effective for all patients. It i

The benefits and prospects of interleukin-6 inhibitor on systemic sclerosis.

Systemic sclerosis (SSc) is a connective tissue disease, the pathogenesis of which is thought to involve interleukin-6 (IL-6), an inflammatory cytokine. This is based on findings of its concentration in patient serum, the results of an IL-6 suppression experiment in an animal model, and the results of a pilot study using IL-6 receptor antibody. However, it appears that a number of factors are involved in the pathology of SSc depending on the state of disease progression. In addition, the degree of involvement of IL-6 differs depending on the difference of organs within particular severe sympto

Interleukins and tumor necrosis factor as inhibitors of food intake.

Cytokines, such as interleukins and tumor necrosis factor-alpha (TNFalpha), are produced in response to immune stimulation and have systemic effects, mediated by the central nervous system (CNS). Interleukins, in particular interleukin [IL]-1beta, and TNFalpha reduce food intake after peripheral and central administration, suggesting that they contribute to the anorexia during various infectious, neoplastic and autoimmune diseases. Because cytokines are mainly produced in the periphery during most of these diseases, IL-1beta and TNFalpha may inhibit feeding indirectly through neural and humora

Risk of candidiasis associated with interleukin-17 inhibitors: Implications and management.

The application of interleukin-17 (IL-17) inhibitors, including secukinumab, ixekizumab, brodalumab, and bimekizumab, are associated with elevated risk of candidiasis. These medications interfere with the IL-17 pathway, which is essential for maintaining mucosal barriers and coordinating the immune response against Candida species. The observational data and clinical trials demonstrate the increased incidence of candidiasis in individuals treated with IL-17 inhibitors. Brodalumab and bimekizumab pose a greater risk than secukinumab in eliciting candidiasis, whereas the data regarding ixekizuma

Beyond tumor necrosis factor and interleukin-12/23: The rise of interleukin-23 inhibitors in inflammatory bowel disease management.

Interleukin-23 (IL-23) has been identified as a key driver of chronic intestinal inflammation and treatment resistance, particularly in patients who have failed anti-tumor necrosis factor (TNF) therapy. Genetic studies and preclinical models have confirmed IL-23's pivotal role in promoting both innate and adaptive immune responses in the gut. This review examines the biology of IL-23 and the evidence supporting its targeting in inflammatory bowel disease (IBD). Clinical trials of IL-23p19 inhibitors-such as risankizumab, mirikizumab, and guselkumab-have shown promising results in patients with

A fusion protein of the gp130 and interleukin-6Ralpha ligand-binding domains acts as a potent interleukin-6 inhibitor.

Interleukin (IL)-6 is involved in the maintenance and progression of several diseases such as multiple myeloma, rheumatoid arthritis, or osteoporosis. The present work aims at the development of an IL-6 inhibitor for the use in anti-cytokine therapies. The IL-6 receptor is composed of two different subunits, an alpha-subunit (IL-6Ralpha) that binds IL-6 with low affinity and a beta-subunit (gp130) that binds the IL-6.IL-6Ralpha complex with high affinity and as a result triggers intracellular signaling. In its soluble form, gp130 is a natural antagonist that neutralizes IL-6.soluble IL-6Ralpha
