The results of percutaneous automated lumbar nucleotomy in the treatment of lumbar disc herniation were assessed in a series of 39 patients. The technique consists of mechanical decompression of the herniated intervertebral disc without total excision. Only one-space discs were treated by this method. Sciatica was the predominant clinical symptom in 30 cases, and lumbar pain in 9 cases. Thirty patients with persistent sciatica due to a single disc protrusion were injected with 0.5 ml (2.0 mg) or 2.0 ml (8.0 mg) of chymopapain. Serial 24-hour urine samples were collected 1 day preinjection and for 5 days postinjection for glycosaminoglycan analysis. We studied the clinical results of 100 patients operated after failure of chemonucleolytic treatment with chemopapain for lumbosciatica caused by disk herniation. Secondary surgical procedures were performed because of persistence of sciatic pain and disk herniation visible on CT scan. Post-operative follow-up was done at a minimum of one year. A follow-up of 1289 chemonucleolyses performed on 1141 patients and a questionnaire answered by 80% of these patients shows that some conditions hitherto held to be contraindications can be questioned: chemonucleolysis performed in cases of disk herniations with spinal stenosis can produce very good results. A second chemonucleolysis with chymopapain at a later date is possible. In patients who have previously undergone surgery on the same disk, chemonucleolysis offers good chances of improvement in the event of a reherniation.

One hundred and twenty-eight patients underwent discography in our department over a period of 3 years. Eighty-four of them were treated by chemonucleolysis. All the patients were punctured by a postero-lateral approach of the intervertebral disc under fluoroscopy. Although several contraindications to chemonucleolysis are known, in our study, only one case of spondylodiscitis was noted. Overall success rate was 72.6%. Twenty-three patients (27.4%) experienced similar or increased pain after treatment.

Results of chemonucleolysis were evaluated (on the basis of residual pain, occupational activities, physical activities, and use of analgesics) in 125 of 162 consecutive patients with lower back pain and sciatica associated with documented vertebral disc protrusion who were managed and followed up for at least five years. Three cases of acute disc herniation causing cauda equina compression syndrome after chemonucleolysis are described. All three patients had myelographic blocks and, despite emergency decompression procedures, were left with residual neurological deficits. Recommendations are made regarding evaluation and therapeutic intervention, and possible etiologies of this problem are reviewed.
