Tissue engineering strategies for treating intervertebral disc degeneration.

Low back pain (LBP) is becoming prevalent in clinics, owing primarily to intervertebral disc degeneration (IDD). The mechanism of IDD is complex, and its pathophysiology is primarily characterized by a decrease in intervertebral nucleus pulposus cells and an imbalance in the synthesis and degradation of the extracellular matrix of the intervertebral disc (IVD). Grounded in the concept of regenerative repair, IVD engineering has emerged as a significant research focus in spinal surgery. This review systematically examines the relationship between LBP and IDD, describes the physiological charact

The JNK signaling pathway in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) serves as the underlying pathology for various spinal degenerative conditions and is a primary contributor to low back pain (LBP). Recent studies have revealed a strong correlation between IDD and biological processes such as Programmed Cell Death (PCD), cellular senescence, inflammation, cell proliferation, extracellular matrix (ECM) degradation, and oxidative stress (OS). Of particular interest is the emerging evidence highlighting the significant involvement of the JNK signaling pathway in these fundamental biological processes of IDD. This paper explo

Association between intervertebral disc degeneration and Behçet's disease.

Behçet's disease (BD) is a chronic systemic vasculitis with a wide range of clinical findings. It has both autoinflammatory and autoimmune features and manifests with recurrent inflammatory attacks involving the innate immune system. Recently, autoinflammation has started to take place in the pathogenesis of intervertebral disc degeneration. The aim of this study is to evaluate the relationship between intervertebral disc degeneration and BD. We evaluated patients with BD who suffered neck or low back pain in the last 1 year. Eighty four patients underwent musculoskeletal system examination wi

The cell biology of intervertebral disc aging and degeneration.

Intervertebral disc degeneration, which mimics disc aging but occurs at an accelerated rate, is considered to be related to neck or low back pain and disc herniation. Degenerated discs show breakdown of the extracellular matrix and thus fail to bear the daily loadings exerted on the spine. Rather than a passive process of wear and tear, disc degeneration is an aberrant, cell-mediated response to progressive structural failure due to aging and other environmental factors such as abnormal mechanical stress. With aging and degeneration, disc cells undergo substantially biologic changes, including

Progress of melatonin in the treatment of intervertebral disc degeneration.

The most common degenerative condition affecting the musculoskeletal system, and the leading cause of persistent low back pain, is intervertebral disc degeneration (IDD). IDD is increasingly common with age and has a variety of etiologic factors including inflammation, oxidative stress, extracellular matrix (ECM) degradation, and apoptosis that interact with each other to cause IDD. Because it is difficult to determine the exact pathogenesis of IDD, there is a lack of effective therapeutic agents. Melatonin has been intensively studied for its strong anti-inflammatory, antioxidant, and anti-ap

Bioenergetic dysfunction in the pathogenesis of intervertebral disc degeneration.

Intervertebral disc (IVD) degeneration is a frequent cause of low back pain and is the most common cause of disability. Treatments for symptomatic IVD degeneration, including conservative treatments such as analgesics, physical therapy, anti-inflammatories and surgeries, are aimed at alleviating neurological symptoms. However, there are no effective treatments to prevent or delay IVD degeneration. Previous studies have identified risk factors for IVD degeneration such as aging, inflammation, genetic factors, mechanical overload, nutrient deprivation and smoking, but metabolic dysfunction has n

The effect of kyphoscoliosis on intervertebral disc degeneration in dogs.

In people, abnormalities in vertebral column conformation, such as kyphoscoliosis, induce degenerative changes in adjacent intervertebral disc (IVD) structure and composition. It was hypothesised that canine IVDs adjacent to a vertebral malformation undergo early degeneration. In a blinded retrospective study, thoracic IVD degeneration was evaluated in 14 dogs on magnetic resonance images using Pfirrmann's grade. IVDs adjacent to a vertebral malformation had higher grades of degeneration than non-adjacent IVDs (P < 0.0001). There was an age-dependency, with dogs between 1 and 4 years showing h

CSF1 regulates inflammation and apoptosis in intervertebral disc degeneration.

Intervertebral disc degeneration (IVDD) is a condition that severely impairs the quality of life for patients and is becoming increasingly prominent in aging societies. There is a pressing need to further investigate the mechanisms underlying the initiation and progression of IVDD. Current therapeutic approaches exhibit inconsistent efficacy, highlighting the urgency for the exploration of novel treatment strategies. In this study, we analyzed microarray datasets GSE147383 and GSE124272, downloaded from the Gene Expression Omnibus (GEO) database, which encompass 12 normal samples and 12 sample

Microstructure-informed biphasic-osmotic modeling of intervertebral disc degeneration.

The intervertebral disc (IVD) plays a fundamental role in load absorption and redistribution during daily activities. Its mechanical behavior arises from the intricate interplay between the structural anisotropy of the annulus fibrosus (AF) and osmotic swelling driven by fixed charge density in the nucleus pulposus (NP). This behavior is further governed by fluid redistribution through the porous matrix and across its boundaries, including interactions with the adjacent endplates and the surrounding physiological environment. Despite advances in IVD modeling, few computational tools accurately

Blood and lymphatic vascular remodeling in intervertebral disc degeneration.

A defining histological feature of the intervertebral disc (IVD) in healthy adults is its avascular and alymphatic status, which is closely related to its mode of nutrient supply and local immune homeostasis. Following the onset of intervertebral disc degeneration (IDD), blood vessels invade the interior of the degenerated IVD under the combined effects of anatomical barrier disruption, inflammatory responses, and aberrant activation of growth factor regulatory networks, thereby accelerating the degenerative process and being closely associated with discogenic pain. However, as another major c

Fundamentals of intervertebral disc degeneration and related discogenic pain.

Lumbar intervertebral disc degeneration is thought to be the main cause of low back pain, although the mechanisms by which it occurs and leads to pain remain unclear. In healthy adult discs, vessels and nerves are present only in the outer layer of the annulus fibrosus and in the bony endplate. Animal models, and histological and biomechanical studies have shown that annulus tear or endplate injury is the initiating factor for painful disc degeneration. Injury to the disc triggers a local inflammatory repair response that activates nociceptors and promotes the synthesis of neuropeptides such a

Wharton's jelly-derived nanovesicles for targeting intervertebral disc degeneration.

The present study was designed to characterize the matrix-bound nanovesicles (MBVs) in decellularized Wharton's jelly matrix (DWJ). We have previously used DWJ as a biomaterial demonstrating its efficacy in restoring the functional properties of human intervertebral disc (IVD) cells lost following degeneration (IDD), for which there is currently no cure. MBVs were isolated by ultracentrifugation from DWJ, characterized by transmission electron microscopy, nanoparticle tracking assay, and surface marker expression. MBVs uptake into cells was assessed by CalceinAM labeling. Nitric oxide producti

Targeting Ferroptosis With Ginsenoside Rg3 Alleviates Intervertebral Disc Degeneration.

OBJECTIVE: Intervertebral disc degeneration (IVDD) has been closely associated with ferroptosis in nucleus pulposus cells (NPCs), the underlying regulatory mechanisms and therapeutic strategies remain poorly defined. This study aims to delineate how ginsenoside Rg3 mitigates IVDD progression through ferroptosis suppression, providing a basis for clinical translation. METHOD: An erastin-induced nucleus pulposus cell ferroptosis model was established. Suitable Erastin concentrations (0-20 μM) were screened via CCK-8, qRT-PCR, and Western blotting based on viability, extracellular matrix (COL2A1

Single-cell sequencing: New insights for intervertebral disc degeneration.

Over the past decade, single-cell RNA sequencing (scRNA-seq) has revolutionized research on biological mechanisms of diseases. Moreover, this technique has been utilized to identify and characterize unique cell types and subpopulations, thereby illuminating cellular heterogeneity. The true value of scRNA-seq lies in its ability to detect transcriptional alterations or perturbed pathways within specific cell types under pathological conditions. In the context of intervertebral disc degeneration (IVDD), the pathophysiological foundation is largely rooted in inflammation. The primary target cells

Regulatory Effect of Inflammatory Mediators in Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IDD) is a major contributor to back, neck, and radicular pain. It is related to changes in tissue structure and function, including the breakdown of the extracellular matrix (ECM), aging, apoptosis of the nucleus pulposus, and biomechanical tissue impairment. Recently, an increasing number of studies have demonstrated that inflammatory mediators play a crucial role in IDD, and they are being explored as potential treatment targets for IDD and associated disorders. For example, interleukins (IL), tumour necrosis factor-α (TNF-α), chemokines, and inflammasomes h

Senolytics: Eliminating Senescent Cells and Alleviating Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IVDD) is the main cause of cervical and lumbar spondylosis. Over the past few years, the relevance between cellular senescence and IVDD has been widely studied, and the senescence-associated secretory phenotype (SASP) produced by senescent cells is found to remodel extracellular matrix (ECM) metabolism and destruct homeostasis. Elimination of senescent cells by senolytics and suppression of SASP production by senomorphics/senostatics are effective strategies to alleviate degenerative diseases including IVDD. Here, we review the involvement of senescence in the

Osteoarthritis and intervertebral disc degeneration: Quite different, quite similar.

Intervertebral disc degeneration describes the vicious cycle of the deterioration of intervertebral discs and can eventually result in degenerative disc disease (DDD), which is accompanied by low-back pain, the musculoskeletal disorder with the largest socioeconomic impact world-wide. In more severe stages, intervertebral disc degeneration is accompanied by loss of joint space, subchondral sclerosis, and osteophytes, similar to osteoarthritis (OA) in the articular joint. Inspired by this resemblance, we investigated the analogy between human intervertebral discs and articular joints. Although

Biologic treatment of mild and moderate intervertebral disc degeneration.

Disc degeneration is the most common cause of back pain in adults and has enormous socioeconomic implications. Conservative management is ineffective in most cases, and results of surgical treatment have not yet reached desirable standards. Biologic treatment options are an alternative to the above conventional management and have become very attractive in recent years. The present review highlights the currently available biologic treatment options in mild and moderate disc degeneration, where a potential for regeneration still exists. Biologic treatment options include protein-based and cell
