OBJECTIVE: Limited evidence exists characterizing the incidence, risk factors, and clinical associations of cerebral vasospasm following traumatic intracranial hemorrhage (tICH) on a large scale. Therefore, the authors sought to use data from a national inpatient registry to investigate these aspects of posttraumatic vasospasm (PTV) to further elucidate potential causes of neurological morbidity and mortality subsequent to the initial insult.
METHODS: Weighted discharge data from the National (Nationwide) Inpatient Sample from 2015 to 2018 were queried to identify patients with tICH who under

Our objective was to apply the technique of measuring diameters of optic nerve sheath (ONSD) for the intracranial pressure assessment for the cases with traumatic head injury without hemorrhage. In a retrospective study, CT data of 720 adult patients were collected and analyzed. ONSDs were measured at the point where the ophthalmic artery crosses the optic nerve (anatomical landmark) together with the eyeball transverse diameter (ETD). The ONSD/ETD index was calculated. The correlation analysis was performed with gender, age, the Glasgow Coma Scale score, and the Glasgow Outcome Score. ONSD wa

Background Researchers have shown that extracranial (EC) injuries can influence outcomes in patients with intracranial hemorrhage (ICH). Large data sets have noted that the presence of EC injury can have detrimental effects on ICH. However, the effects are not well quantified. EC injury has been historically treated as a categorical variable. Only one study besides the current one has attempted to calculate the degree of EC injury in affecting outcomes in ICH. We calculated a novel systemic variable for EC injury as the EC Injury Severity Score (EC-ISS) and aimed to assess the impact of EC inj

Introduction Telemedicine protocols for minimal traumatic intracranial hemorrhage (ICH) enable peripheral management without transfer to trauma centers, but post-discharge utilization patterns among these patients remain poorly characterized. This study evaluated factors associated with 30-day readmission and unplanned re-encounter among adult patients managed after implementation of a telemedicine minimal head bleed protocol. Methods A retrospective cohort study was conducted of adult patients with minimal traumatic ICH managed without transfer under a telemedicine protocol at a regional trau

OBJECTIVE: Although oral anticoagulant use has been implicated in worse outcomes for patients with a traumatic brain injury (TBI), prior studies have mostly examined the use of vitamin K antagonists (VKAs). In an era of increasing use of direct oral anticoagulants (DOACs) in lieu of VKAs, the authors compared the survival outcomes of TBI patients on different types of premorbid anticoagulation medications with those of patients not on anticoagulation.
METHODS: The authors retrospectively reviewed the records of 1186 adult patients who presented at a level I trauma center with an intracranial

Introduction: Patients with traumatic intracranial hemorrhage (ICH) or skull fracture are typically admitted to the Department of Neurosurgery for fear of delayed neurological deterioration. Neurosurgeons, therefore, must be careful not to overlook a spinal fracture in these patients. In this study, we investigated the occurrence and risk factor of spinal fracture in patients with traumatic ICH or skull fracture. Patients and methods: We retrospectively analyzed the hospital records of 134 patients admitted to the Department of Neurosurgery at Kagawa Rosai Hospital for traumatic ICH or skull f

Our purpose was to improve the technique of measuring optic nerve sheath diameter (ONSD) for intracranial pressure (ICP) monitoring in cases of traumatic head injury with hemorrhage. In a retrospective study, computed tomography (CT) data of 312 adult patients were collected and analyzed. ONSDs were measured at 3 mm and 8-10 mm distance from the globe together with the eyeball transverse diameter (ETD). The ONSD/ETD ratio was calculated. The correlation analysis was performed with gender, age, Glasgow Coma Scale score, and Glasgow Outcome Score. ONSD was enlarged in all cases when CT scans ind

Indicators of intracranial pressure (ICP) and cerebral compliance in 48 patients with primary non-traumatic intracerebral hemorrhages who underwent surgery using precise aspiration and local fibrinolysis were studied. Based on clinical and neuroimaging data, ICP indicators and cerebral compliance, we can evaluate the compressive influence of the cerebral hematoma, perform the dynamic control of fibrinolysis efficacy and intracerebral hematoma aspiration as well as of dislocation syndrome's regress and predict outcomes of disease. The restoration of ICP indicators, cerebral compliance and elimi

BACKGROUND AND OBJECTIVES: Traumatic cerebral venous sinus injury (tCVSI) is a rare but serious complication of traumatic brain injury. Although similar radiographically to spontaneous cerebral venous sinus thrombosis (CVST), tCVSI treatment approaches have not been fully investigated. We characterized complications, clinical outcomes, and recanalization in tCVSI and evaluated treatment approaches.
METHODS: In this retrospective review of patients with radiographically confirmed tCVSI across 3 Level 1 trauma centers from 2018 to 2024, we collected patient characteristics, injury patterns, tre

IMPORTANCE: The increase in the risk for bleeding associated with antithrombotic therapy causes a dilemma in patients with atrial fibrillation (AF) who sustain an intracranial hemorrhage (ICH). A thrombotic risk is present; however, a risk for serious harm associated with resumption of anticoagulation therapy also exists.
OBJECTIVE: To investigate the prognosis associated with resuming warfarin treatment stratified by the type of ICH (hemorrhagic stroke or traumatic ICH).
DESIGN, SETTING, AND PARTICIPANTS: This nationwide observational cohort study included patients with AF who sustained an

PURPOSE: To summarize current literature evaluating tranexamic acid in the management of intracranial bleeding associated with traumatic and nontraumatic brain injuries and implications for clinical practice.
SUMMARY: Intracranial hemorrhage, regardless of etiology, is associated with high morbidity and mortality. Tranexamic acid is an antifibrinolytic with anti-inflammatory properties shown to reduce mortality in trauma patients with extracranial injuries. In traumatic brain injury, a large randomized trial found no difference in outcomes when tranexamic acid was compared to placebo; however

BACKGROUND: Hemorrhage is the leading cause of preventable death world-wide. Traumatic brain injury (TBI) is the primary driver of mortality and morbidity among individuals aged 1-44 years worldwide, with intracranial hemorrhage (ICH) being the major contributor to acute mortality post-TBI. Based on prior studies, we hypothesized that lyophilized platelet-derived extracellular vesicles (LPEVs) would reduce ICH and preserve blood-brain barrier (BBB) integrity after TBI.
METHODS: LPEVs were characterized by flow cytometry, scanning electron microscopy, and Nanosight. LPEVs were evaluated in vit

BACKGROUND: This case report is the first documented and illustrated case of the identification and treatment of intracranial vasospasm as a sequalae of traumatic lumbar puncture (LP). LP is a routine procedure performed for both diagnostic and therapeutic purposes. Although rare, this procedure has risks and complications that should be considered before performing.
CASE DESCRIPTION: A 58-year-old male was found to have intracranial subarachnoid hemorrhage (SAH) 2 days after a traumatic LP which occurred in the setting of subtherapeutic international normalized ratio. During his hospitalizat

BACKGROUND: Traumatic brain injury (TBI) remains one of the most challenging health and socioeconomic problems of our times. Clinical courses may be complicated by hemostatic abnormalities either pre-existing or developing with TBI.
OBJECTIVE: To review frequencies, patterns, mechanisms, novel approaches to diagnostics, treatment, and outcomes of hemorrhagic progression and coagulopathy after TBI.
METHODS: Selective review of the literature in the databases Medline (PubMed) and Cochrane Reviews using different combinations of the relevant search terms was conducted.
RESULTS: Of the patients

PURPOSE: To synthesize evidence of the use of near-infrared spectroscopy (NIRS) to detect intracranial hemorrhage in traumatic brain injury (TBI) patients.
METHODS: The literature search was conducted in PubMed and Google Scholar (from inception to July 2021).
RESULTS: 216 original articles were found, 197 of which were omitted, and the final review contained 19 original articles covering 2291 patients.
CONCLUSION: For patients with TBI, a NIRS test may be useful as a screening tool for intracranial hemorrhage, especially at the prehospital level. Negative results may help rule out intracra

BACKGROUND: Mild traumatic brain injury (mTBI) comprises the majority of pediatric traumatic brain injury. Children with mTBI even with traumatic intracranial hemorrhage (tICH) rarely experience a clinically significant neurologic decline (CSND). The utility of routine surveillance imaging in the pediatric population also remains controversial, especially owing to concerns about the risks of radiation exposure at a young age. This study aims to identify demographic or injury-related characteristics that may facilitate recognition of children at risk of progression with mTBI.
METHODS: We perfo

OBJECTIVE: To outline differences in neurosurgical intervention (NI) rates between intracranial hemorrhage (ICH) types in mild traumatic brain injuries and help identify which ICH types are most likely to benefit from creation of predictive models for NI.
METHODS: A multicenter retrospective study of adult patients spanning 3 years at 4 U.S. trauma centers was performed. Patients were included if they presented with mild traumatic brain injury (Glasgow Coma Scale score 13-15) with head CT scan positive for ICH. Patients were excluded for skull fractures, "unspecified hemorrhage," or coagulopa
