Benign intracranial hypertension.

Although antecedent factors are occasionally implication, most cases of benign intracranial hypertension are idiopathic and occur in young adult women who otherwise appear healthy. The natural course is usually one of spontaneous recovery, although medical intervention may accelerate improvement. Computerized tomographic scanning is the procedure of choice for excluding more ominous conditions, such as brain tumors. The most serious complication of this syndrome is visual loss and in these patients the disease is not "benign."

Benign intracranial hypertension.

Ninety percent of these 120 patients with BIH were women and were obese. Headache was present in almost all patients and visual symptoms occurred in 35%. There was no neurologic abnormality other than visual loss in these patients followed up to 10 years. Twenty-five percent spontaneously remitted after the initial LP and the others required repeated LP, steroids, or surgical procedures. Thirteen of 15 patients who did not remit spontaneously or respond to repeated LP improved dramatically in 3 to 4 days on prednisone or dexamethasone. Complications of BIH include recurrence of the syndrome, v

Intracranial hypertension. Advances in intracranial pressure monitoring.

Research is continuing to find improved methods for monitoring intracranial pressure as well as the effect of intracranial hypertension on the patient's recovery. Each of the methods of monitoring has its advantages and disadvantages and the use of one method over others is usually the preference of the neurosurgeon. It is the responsibility of the nurse to be aware of the strengths and weaknesses of each device in order to strengthen patient outcome. Again, the major benefits of the bolt are a low infection rate and easy insertion. Problems associated with its use of which the nurse should be

Spontaneous Intracranial Hypotension Followed by Intracranial Hypertension.

INTRODUCTION: Spontaneous intracranial hypotension is a secondary cause of headache caused by suspected cerebrospinal fluid leaks. It is associated with vascular changes that may predispose to superficial siderosis. When treated with an epidural blood patch, rebound intracranial hypertension may ensue. CASE REPORT: A 55-year-old man presented with orthostatic headaches responsive to rest and hydration. Brain magnetic resonance revealed subdural collections, consistent with intracranial hypotension. Three weeks later, the patient experienced sudden severe holocranial headache and spontaneous s

[CME: Idiopathic Intracranial Hypertension].

CME: Idiopathic Intracranial Hypertension Abstract. Idiopathic intracranial hypertension is a pressure-induced secondary headache disorder and optic neuropathy. It primarily affects obese women of childbearing age and poses an interdisciplinary challenge both diagnostically and therapeutically. The most common symptom of this disorder are headaches frequently accompanied by photo- and/or phonophobia, whose semiology often resembles that of migraine, followed by transient visual obscurations and pulsatile tinnitus. While protection of visual acuity and visual fields are of first therapeutical p

Intracranial Hypertension Is Painless!

INTRODUCTION: Headache is usually considered a key symptom of intracranial hypertension (ICHT). However, there are no published experimental data to support the concept that increased intracranial pressure (ICP) is painful in humans. MATERIALS AND METHODS: This prospective study was performed in 16 patients with suspected normal-pressure hydrocephalus, necessitating a lumbar infusion test with measurement of cerebrospinal fluid (CSF) hydrodynamics. During the test, ICP was increased from baseline to a plateau. Headache was scored on a visual analog scale (VAS) (0 = no pain, 10 = very severe p

[Hypothermia for intracranial hypertension].

There is a large body of experimental evidence showing benefits of deliberate mild hypothermia (33-35 degrees C) on the injured brain as well as an improvement of neurological outcome after cardiac arrest in humans. However, the clinical evidence of any benefit of hypothermia following stroke, brain trauma and neonatal asphyxia is still lacking. Controversial results have been published in patients with brain trauma or neonatal asphyxia. Hypothermia can reduce the elevation of intracranial pressure, through mechanisms not completely understood. Hypothermia-induced hypocapnia should have a role

Management of intracranial hypertension.

Raised intracranial pressure (ICP) is a life threatening condition that is common to many neurological and non-neurological illnesses. Unless recognized and treated early it may cause secondary brain injury due to reduced cerebral perfusion pressure (CPP), and progress to brain herniation and death. Management of raised ICP includes care of airway, ventilation and oxygenation, adequate sedation and analgesia, neutral neck position, head end elevation by 20 degrees-30 degrees, and short-term hyperventilation (to achieve PCO(2) 32-35 mm Hg) and hyperosmolar therapy (mannitol or hypertonic saline

Management of intracranial hypertension.

Effective management of intracranial hypertension involves meticulous avoidance of factors that precipitate or aggravate increased intracranial pressure. When intracranial pressure becomes elevated, it is important to rule out new mass lesions that should be surgically evacuated. Medical management of increased intracranial pressure should include sedation, drainage of cerebrospinal fluid, and osmotherapy with either mannitol or hypertonic saline. For intracranial hypertension refractory to initial medical management, barbiturate coma, hypothermia, or decompressive craniectomy should be consid

Idiopathic Intracranial Hypertension (IIH).

The first report of Idiopathic Intracranial Hypertension (IIH) was by Quinche in 1893. Throughout history it has had many names. When I was a child growing up, before we had antibiotic therapy, middle ear infections occasionally would trigger intracranial inflammation and the disease was called Otitic Hydrocephalus. Then it was called Benign Intracranial Hypertension which we now know is far from benign. In 1931 Symonds was the first to suggest that it was caused by dural sinus obstruction. Loss of vision or field is not uncommon in untreated cases. We now use the term Idiopathic Intracranial

Intracranial hypertension in neuroborreliosis.

Neuroborreliosis is an infection of the nervous system caused by the spirochete Borrelia burgdorferi, from which patients most commonly develop lymphocytic meningitis, radiculoneuritis, or cranial neuropathy. In this report a 9-year-old male with an unusual neurological complication of neuroborreliosis--benign intracranial hypertension (BIH)--is described. Clinical symptoms of BIH, which consist of increased CSF pressure in the absence of an intracranial mass or obstruction to the circulation of CSF, resolved completely after antibiotic therapy with ceftriaxone.

[Benign childhood intracranial hypertension].

INTRODUCTION: Infrequent in children, benign intracranial hypertension (or pseudotumor cerebri) is most often observed in adults. Careful diagnosis requires eliminating all the other etiologies of intracranial hypertension. Most often medical, its treatment must be rapid to avoid permanent visual loss. However, a surgical procedure is necessary when vision is threatened. We present our experience with this pathology and discuss its clinical aspects, its etiologies, and the physiopathological mechanisms. PATIENTS AND METHODS: We conducted a retrospective study on children who presented benign

Intracranial hypertension and migraine.

We describe the case report of a migraineur who developed daily postural headache due to benign intracranial hypertension. The CSF pressure was elevated despite the lack of papilledema on clinical examination. This case emphasizes that intracranial hypertension without papilledema must be in the differential diagnosis of the so-called "migraine transformation" headache. Accordingly, CSF analysis, including CSF pressure measurement, should be part of the diagnostic evaluation of "migraine transformation" headache.

[Benign intracranial hypertension syndrome].

Altogether 45 patients (41 women and 4 men) with benign intracranial hypertension (BIH) were observed. BIH developed in 22 women in the gestation period, in 4 during a menopause, in 2 in the presence of hypothyroidism, in 2 due to respiratory infection, in 2 due to a slight cerebrocranial injury, and in one patient with associated galactorrhea-amenorrhea. The cause of BIH in 12 patients remained unclear. Most of the women were obese (33 of 41). The clinical picture of BIH consisted of headaches, congested optic discs, and elevated pressure of cerebrospinal fluid. In most cases prognosis turned

Familial benign intracranial hypertension.

Three sisters with benign intracranial hypertension are reported. This is the first documentation of benign intracranial hypertension in three family members. Obesity is a striking feature in these patients as well as five of the six previously reported patients with familial benign intracranial hypertension. Pregnancy and chronic dysfunctional uterine bleeding, well known predisposing factors in this syndrome when it occurs sporadically, were present in two of the sisters. A familial metabolic defect may be responsible for the intracranial hypertension in these patients.
