Effect of Telemedicine Support for Intraoperative Anaesthesia Care on Postoperative Outcomes: The TECTONICS Randomised Clinical Trial.

BACKGROUND: Novel applications of telemedicine can improve care quality and patient outcomes. Telemedicine for intraoperative decision support has not been rigorously studied. METHODS: This single centre randomised clinical trial (RCT, clinicaltrials.gov NCT03923699 ) of unselected adult surgical patients was conducted between 2019-07-01 and 2023-01-31. Patients received usual-care or decision support from a telemedicine service, the Anesthesiology Control Tower (ACT). The ACT provided real-time recommendations to intraoperative anaesthesia clinicians based on case reviews and physiologic ale

Operating Room Nurses' Understanding of Their Roles and Responsibilities for Patient Care and Safety Measures in Intraoperative Practice.

INTRODUCTION: Surgical care has been a vital part of healthcare services worldwide. Several patient safety measures have been adopted universally in the operating room (OR) before, during, and following surgical procedures. Despite this, errors or near misses still occur. Nurses in the OR have a pivotal role in the identification of factors that may impact patient safety and quality of care. Therefore, exploring the OR nurses' understanding of their roles and responsibilities for patient care and safety in the intraoperative practice, which could lead to optimal patient safety, is essential.

Rapid intraoperative immunoassay of parathyroid hormone and other hormones: a new paradigm for point-of-care testing.

BACKGROUND: The first description of the use of a rapid assay for the measurement of intact parathyroid hormone (PTH) in patients undergoing parathyroidectomy for hyperparathyroidism was reported in 1988. Subsequent improvements in the analytical performance of the rapid intraoperative PTH assay allowed the establishment of its clinical utility in the surgical management of hyperparathyroidism. These modifications also allowed the assay to be performed in or near the operating suite. METHODS: We searched MEDLINE, using the following key words: intraoperative, rapid, quick, parathyroid hormone

Local ampullary resection with careful intraoperative frozen section evaluation for presumed benign ampullary neoplasms.

BACKGROUND: Frozen section evaluation has been reported to be inaccurate in detecting foci of adenocarcinoma within adenomas of the ampulla of Vater, leading many authors to advocate pancreaticoduodenectomy as the method of treatment for these neoplasms. The authors hypothesized that (1) ampullary resection is less morbid than pancreaticoduodenectomy, and (2) frozen section evaluation following ampullary resection is accurate and allows for a selective application of pancreaticoduodenectomy to those with carcinoma or benign lesions too large to be locally resected. METHODS: A retrospective re

Perioperative Kidney Transplant Care: A Narrative Review of Preoperative Optimization, Intraoperative Management, and Early Postoperative Allograft Surveillance.

Kidney transplantation is the preferred kidney replacement therapy for eligible patients with kidney failure; however, early allograft function remains vulnerable to potentially modifiable events across the perioperative continuum. Delayed graft function (DGF), hemodynamic instability, electrolyte and acid-base disturbances, vascular compromise, and surgical complications can adversely affect early recovery, prolong hospitalization, and influence subsequent graft outcomes. This narrative review summarizes evidence-based perioperative strategies designed to reduce preventable contributors to DG

Timing of Intraoperative Dexmedetomidine Administration and Post-anaesthesia Care Unit Recovery: A Single-Centre Quality Improvement Audit.

Background Dexmedetomidine is increasingly used in multimodal perioperative analgesia due to its sedative, anxiolytic, and opioid-sparing properties. However, concerns remain regarding residual sedation, haemodynamic instability, and delayed post-anaesthesia care unit (PACU) discharge, particularly when administered close to the end of surgery. This audit aimed to characterise intraoperative dexmedetomidine use at a tertiary Australian hospital and evaluate the relationship between administration timing, PACU clinical recovery time, and postoperative complications. Methods A retrospective sing

Intraoperative Scoring System to Assess the Difficult Laparoscopic Cholecystectomy: A Prospective Study From a Tertiary Care Centre.

Background In comparison to a traditional cholecystectomy (open), the laparoscopic cholecystectomy approach provides a number of benefits and has been demonstrated in some studies to have a greater complication rate. The conversion rate from laparoscopic to open surgery ranged between 2% and 15%. A preoperative scoring or grading system (based on age, sex, history, clinical examination, laboratory, and sonographic results) was developed by Nassar et al., to anticipate the challenge of laparoscopic cholecystectomy. So, we conducted this study to assess the degree of difficulty in faced during l

Intraoperative electroencephalogram suppression at lower volatile anaesthetic concentrations predicts postoperative delirium occurring in the intensive care unit.

BACKGROUND: Postoperative delirium is a common complication associated with increased morbidity and mortality. A recently-reported association between intraoperative electroencephalogram suppression and postoperative delirium might be mediated in some patients by a heightened sensitivity to volatile anaesthetics. METHODS: This retrospective cohort study included 618 elective surgery patients with planned intensive care unit admission, who also received intraoperative electroencephalogram monitoring and had delirium assessments documented in the medical record. Sensitivity to volatile anaesthe

Total Intravenous Anesthesia With Remimazolam and Remifentanil for Intraoperative Care of a Child With Venezuelan Heritage, at Risk for a Mitochondrial Disorder.

Recent clinical communications have alerted healthcare providers to potential concerns related to a previously undiagnosed disorder of mitochondrial function in children with maternal Venezuelan heritage. These patients have developed severe neurological damage following the administration of an apparently uneventful anesthetic that included the volatile anesthetic agent, sevoflurane. Subsequent analysis of family members and patients has identified a point mutation of the NADH dehydrogenase 4 (ND4) gene (mtND4 m.11232T>C), a subunit of complex I of the mitochondrial electron transport chain.

The Association of Intraoperative Anesthesia Handovers of Anesthesia Care With Adverse Postoperative Outcomes: A Retrospective Multicentric Cohort Study.

BACKGROUND: Intraoperative anesthesia handovers are common in academic practice due to shift-based staffing, duty-hour limits, and the demands of prolonged procedures, and occur among both attending anesthesiologists and supervised providers. Although handovers may serve as important safety checkpoints, they also represent vulnerable periods during which loss of critical information can occur. This study evaluated whether intraoperative handovers among attending anesthesiologists are associated with prolonged hospital length of stay (HLOS) and increased risk of unplanned 30-day readmission. Reduction in intraoperative bacterial contamination of peripheral intravenous tubing through the use of a passive catheter care system.

BACKGROUND: Bacterial contamination of intravascular devices has been associated with increased morbidity and mortality in various hospital settings, including the perioperative environment. Catheter hub disinfection has been shown in an ex vivo model to attenuate intraoperative injection of bacterial organisms originating from the anesthesia provider's hands, providing the impetus for improvement in intraoperative disinfection techniques and compliance. In the current study, we investigated the clinical effectiveness of a new, passive catheter care station in reducing the incidence of bacteri
