Quantitative studies on menstrual blood loss in IUD users.

Despite the introduction of new intrauterine devices (IUDs), the most important complication involving their use continues to be excessive menstrual bleeding. IUD use in developing countries with women who are already depleted in body iron stores may prove to be deleterious to their health. Mean amounts of menstrual blood loss (MBL) for women not using contraception in the western world is about 32 ml. This mean is increased to 52-72 ml with use of the Lippes loop and other non-medicated devices up to 24 months after insertion. For the Copper-7 and Copper-T-200 devices this mean increase is to

The intrauterine device and the intrauterine system.

Intrauterine contraception is used by about 100 million women worldwide, making it the most popular form of fertility regulation. In UK community contraception clinics, however, long-acting reversible contraception has increased to 28% of users, and intrauterine contraception accounts for only 8% of methods used by women accessing these services. Potential exists to increase uptake of these more effective methods. In this chapter, we review the clinical advantages, disadvantages and cost-effectiveness of intrauterine contraception. We discuss the management of complications along with advice f

Intrauterine device use among women with ovarian cancer: a systematic review.

BACKGROUND: Fertility-sparing treatment may be an option for women with early stage ovarian cancer and certain tumor types. This systematic review evaluated the evidence on the safety of intrauterine device (IUD) use by women with ovarian cancer. STUDY DESIGN: We searched the PubMed database for peer-reviewed articles relevant to IUD (copper or levonorgestrel-releasing) use and ovarian cancer published in any language from database inception through August 2009. We sought studies that examined outcomes among women using an IUD at or after ovarian cancer diagnosis. RESULTS: Of the 250 article

Association between menorrhagia and risk of intrauterine device-related uterine perforation and device expulsion: results from the Association of Uterine Perforation and Expulsion of Intrauterine Device study.

BACKGROUND: Intrauterine devices are effective instruments for contraception, and 1 levonorgestrel-releasing device is also indicated for the treatment of heavy menstrual bleeding (menorrhagia). OBJECTIVE: To compare the incidence of intrauterine device expulsion and uterine perforation in women with and without a diagnosis of menorrhagia within the first 12 months before device insertion STUDY DESIGN: This was a retrospective cohort study conducted in 3 integrated healthcare systems (Kaiser Permanente Northern California, Southern California, and Washington) and a healthcare information exch

Elective surgical removal of migrated intrauterine contraceptive devices from within the peritoneal cavity: a comparison between open and laparoscopic removal.

BACKGROUND: Intrauterine contraceptive devices (IUCDs) comprise the most popular form of reversible contraception. Uterine perforation is a rare but potentially serious complication associated with their use. We examined all reported cases of elective surgical removal of peritoneally migrated IUCDs, to compare laparoscopic and open approaches, and to identify beneficial surgical techniques. DATABASE: MEDLINE and Embase were searched using the following medical subject heading terms: (IUCD or IUD or IUS or intrauterine device or intrauterine devices, copper or intrauterine devices, medicated)

Levonorgestrel intrauterine device with absence of threads: don't take expulsion for granted! A case series of possible errors in ultrasound evaluation, in case of "lost IUDs".

Levonorgestel intrauterine devices are safe contraceptive methods, with even medical indications. However, they may present rare, but severe complications such as perforation and abdominal dislocation. Ultrasound assessment may be limited if a medicate intrauterine device is lost, due to its particular echogenicity. We report cases complicated by loss of intrauterine devices. Based on this experience we believe that ultrasound should always be associated to an X-ray examination to prevent misdiagnosis, especially in case of loss of a levonorgestrel device. Although our conclusions are not inno

Management of the malpositioned levonorgestrel-releasing intrauterine system.

BACKGROUND: The management of a malpositioned levonorgestrel (LNG)-releasing intrauterine system (IUS) can present a clinical challenge, complicated by the high cost of replacing it with a new device. We tried to challenge the guidelines and common practice that usually suggest prompt removal of any malpositioned IUS and replacement with a new one. STUDY DESIGN: We present our experience with 18 patients in whom the LNG-releasing IUS was found during a routine sonographic evaluation to be displaced towards the cervical canal 2 to 36 months postinsertion. Using an alligator forceps, the device

Levonorgestrel-releasing and copper-releasing (Nova T) IUDs during five years of use: a randomized comparative trial.

A 20 micrograms/24 hours levonorgestrel-releasing intrauterine contraceptive device (LNG-IUD) has, in an open randomized multicenter study, been compared to the copper-releasing device Nova T during 5 years of use; 1821 women had the LNG-IUD and 937 women had the Nova T inserted. The 5-year cumulative gross pregnancy rate was 5.9% for Nova T and 0.5% for the LNG-IUD. The local effect of levonorgestrel in the uterine cavity causes reduction of menstrual blood loss and development of oligo-amenorrhea, and the termination rates because of heavy and/or prolonged menstrual flow were significantly (

Dose- and age-dependent ectopic pregnancy risks with intrauterine contraception.

Analyses of randomized trials of copper intrauterine devices (IUDs) substantiate three hypotheses: 1) Ratios of ectopic to total pregnancies conceived during copper IUD use did not differ among devices; 2) pregnancy rates per 1000 woman-years varied inversely with copper surface area; and 3) ectopic pregnancy rates consequently varied inversely by copper surface area. Randomized trials also demonstrated that progestin-only IUDs had a markedly higher ratio of ectopic to total pregnancies, 171 per 1000, than did copper IUDs, at 39 per 1000 (P less than .001). Ectopic pregnancy rates per 1000 wom

Local effects of intrauterine contraceptives placed within the peritoneal cavities of rabbits.

Two studies were conducted to evaluate local surface tissue responses to seven different intrauterine contraceptive units placed within the peritoneal cavities of rabbits, either near the uterus or in contact with the hepatic capsule. One or six weeks after insertion, the animals were killed and tissue responses evaluated. All local tissue responses were inflammatory although in some animals tissue responses were also associated with a mechanical component related to pressure of the contraceptive units on the hepatic capsule. Control animals revealed no response. One-week tissue responses amon

Intrauterine contraception.

Newer medicated IUDs, such as the copper or progesterone-releasing devices, are more effective, better tolerated and have fewer side effects than nonmedicated IUDs. Major complications are rare if adequate precautions are taken. The IUD is an ideal contraceptive for patients with contraindications to oral contraception and for those who have difficulty remembering to utilize other methods.

Pain and ease of insertion of three different intrauterine devices in Brazilian adolescents: A participant-blinded randomized trial.

OBJECTIVES: To compare pain and ease of insertion of the copper 380 mm2, levonorgestrel 52 mg, and levonorgestrel 19.5-mg intrauterine devices (IUDs) in Brazilian adolescents. STUDY DESIGN: We conducted a participant-blinded randomized trial at two clinics in Brazil. We enrolled 318 adolescents<19 years old in a 1:1:1 ratio from November 2021 to February 2022. We informed the adolescents about the IUD type inserted after they evaluated the pain associated with the IUD insertion using a Visual Analogue Scale and immediately after that the healthcare provider who placed the IUD evaluated the ea

[Summary of the practice guideline 'The intrauterine device' from the Dutch College of General Practitioners].

The practice guideline 'The intrauterine device' from the Dutch College of General Practitioners, first published in 2000, has been revised. Copper and hormonal IUDs have more or less the same level of reliability with respect to preventing pregnancy. During the use of a copper IUD, menstruation tends to be longer with a greater loss of blood; in 70% of women who use a hormonal IUD oligomenorrhea or even amenorrhoea develops. Women with a history of venous thromboembolism can use a hormonal IUD safely. In the first weeks after IUD insertion, there is an increased risk of pelvic inflammatory di
