One hundred and forty nine patients with carcinoma of the tongue or floor of mouth were treated with interstitial irradiation (+/- external beam therapy) using caesium needles or iridium wires between 1970 and 1986. Multivariate analysis showed the main predictors of outcome to be tumour stage, site and histology. Caesium and iridium techniques gave similarly good local control rates of 90% at 5 years for T1 and T2 tumours when used as the standard departmental method. BACKGROUND: For gynecological brachytherapy, multichannel vaginal cylinders (MCVC) offer improved control of the dose distribution as compared to single channel cylinders due to their peripheral channels. Evaluation of the source position in each channel is a crucial step when commissioning these applicators that can help to reduce systematic uncertainties, but is challenging with standard 2D techniques.

The success of radioactive implant therapy for head and neck carcinomas depends critically on careful planning and execution of the implant procedure. In this paper we discuss our experience with oral tongue and floor of mouth implants, and some innovations introduced to facilitate these procedures. Implants were carried out using standard angiocatheters modified with magnetic caps at the open end and terminated with Teflon spacers and lead shot at the closed end. The importance of accurate source placement and careful determination of the target dose rate is discussed with numerical examples.

A Lucite sandwich mold containing Iridium-192 wire for use in the treatment of soft palate carcinoma is described. This mold is very simple to use, produces a homogeneous (+/- 10%) radiation field across the midplane of the tumor, and requires minimal machinist time.

Between 1971 and 1985, 12 children and adolescents aged 0.7 to 19 years (median 4.7 years) with localized residual soft-tissue sarcomas (STS) underwent interstitial radiation therapy (IRT) at our institution. Eight received IRT as a component of initial therapy, and four were treated for recurrent or persistent disease. Nearly one-half of all patients with non-oat cell carcinoma of the lung are found to have mediastinal lymph node metastases at the time of initial presentation. High dose rate, remote-afterloading intracavitary irradiation (RALS) and low dose rate, interstitial irradiation were discussed. In Japan, RALS is now a standard treatment for uterine cervix carcinoma. A pilot study using transperineal interstitial implantation to the parametrium in patients with locally advanced carcinoma of the cervix or with distorted anatomy is presented. Twenty-one consecutive patients (2 cervical stump Stage IB, 3 Stage IIB, 15 Stage IIIB, 1 Stage IVA) received one (12) or two (9) implants following 40-45 Gy whole pelvis external irradiation. The results are presented of ten patients with stage II.B cervical carcinoma, treated with pre-operative after-loading intra-cavitary Iridium 192 followed by radical surgery. The benefits of this combined technique are the reduction of tumor volume, whether in the cervix or in the parametrium. The surgical approach became easier and very few complications were noted. The survival rate after two years follow-up is 100% and no local or distant recurrences have been noted. The following treatment in patients with negative and positive lymph-nodes are discussed.

A total of 70 patients with histologically proven diagnosis of carcinoma of the base of the tongue were treated with primary irradiation between May 1974 through April 1984. Fifty-eight (83%) of these patients had locally advanced tumors (Stage T3, T4, N2, N3). Fifty-one of the 70 (73%) patients had clinically palpable neck nodes at first presentation. 192Ir looping and crossing methods as a means of megavoltage tumor dose boosting have been applied for Stage II and III lateral and posterior oro-, hypopharyngeal tumors. A megavoltage and interstitial combined dose in excess of 9000 rad resulted in a soft tissue injury in two patients. Nevertheless, all five patients are alive without residual tumor at 8-38 months. We believe that a megavoltage dose of 5200 rad plus interstitial doses of 3000-3500 rad with a four week rest period intervening may improve cure rate at an acceptable injury rate.
