Immune reconstitution inflammatory syndrome (IRIS)--another new disease entity following treatment initiation of HIV infection.

After the introduction of highly active antiretroviral therapy (HAART) in 1995 it took 3 years to recognize the new syndrome of lipodystrophy as a long-term complication of HAART [1]. It was found to be a direct toxic consequence of the various drugs used in combination regimens. Another 2 years later, an additional syndrome was described in patients who recently initiated HAART-the immune reconstitution inflammatory syndrome (IRIS) [2-4]. Although already recognized in the pre-HIV era in patients with treatment for tuberculosis [5, 6] and leprosy [7], it became substantially more frequent in

Quantifying iris pigmentation irregularities following an avian influenza outbreak: implications for population-level disease surveillance in a colonial seabird.

Emerging infectious diseases can have catastrophic impacts on wildlife populations, yet identifying individuals that survived exposure, especially when external symptoms are absent, remains challenging. Since 2021, a virulent strain of highly pathogenic avian influenza virus (HPAIV; H5N1 clade 2.3.4.4b) has caused unprecedented mortality in wild birds. Northern gannets (Morus bassanus) suffered dramatic mortality and reproductive failure at North America's largest colony in 2022. Following the outbreak, Lane et al. reported that a subset of gannets at a UK colony displayed darkened irises asso

Identifying the mechanism of missingness for unspecified diabetic retinopathy disease severity in the electronic health record: an IRIS® Registry analysis.

Observational studies of diabetic retinopathy (DR) using electronic health record data often determine disease severity using International Classification of Disease (ICD) codes. We investigated the mechanism of missingness for DR severity based on ICD coding using the American Academy of Ophthalmology IRIS® Registry. We included all patient encounters in the registry with a DR ICD-9 or ICD-10 code between January 1, 2014 and June 30, 2021. Demographic, clinical, and practice-level characteristics were compared between encounters with specified and unspecified disease severity. Practices were

A longitudinal study on the acceptance and effects of a therapeutic renal food in pet dogs with IRIS-Stage 1 chronic kidney disease.

Currently, nutritional management is recommended when serum creatinine (Cr) exceeds 1.4 mg/dl in dogs with IRIS-Stage 2 chronic kidney disease (CKD) to slow progressive loss of kidney function, reduce clinical and biochemical consequences of CKD, and maintain adequate nutrition. It is unknown if dietary interventions benefit non-azotemic dogs at earlier stages. A prospective 12-month feeding trial was performed in client-owned dogs with IRIS-Stage 1 CKD (n = 36; 20 had persistently dilute urine with urine specific gravity (USG) <1.020 without identifiable non-renal cause; six had persistent pr

[Intraoperative brain surface blood flow monitoring using IRIS V thermographic imaging system in patients with Moyamoya disease].

Surgical revascularization for moyamoya disease prevents cerebral ischemic attacks by improving CBF. But little is known about the changes of intraoperative cerebral hemodynamics and its effect on postoperative neurological status including symptomatic cerebral hyperperfusion. To address this issue, we applied a novel infrared camera system (IRIS-V thermographic system) for real-time, visual monitoring of surface CBF during surgery in patients with moyamoya disease. Seven patients (8 sides, male:female= 3:4, 7-62 years old) with moyamoya disease were included in the study. After STA-MCA anasto

Kaposi Sarcoma-Associated Immune Reconstitution Inflammatory Syndrome (KS-IRIS) in Advanced HIV: Challenges in Managing Visceral Disease With Pericardial Involvement.

Immune reconstitution inflammatory syndrome (IRIS) is a well-described complication following initiation of antiretroviral therapy (ART) in people with HIV. In this case report, we describe the complex clinical presentation of a 27-year-old ART-naïve male with advanced HIV who developed Kaposi sarcoma-associated IRIS (KS-IRIS) with visceral involvement and pericardial effusion shortly after starting ART. We review known risk factors for KS-IRIS, its clinical manifestations including rare cardiac involvement and paradoxical worsening with steroid use, as well as current evidence for chemotherap

Prediction of unfavorable outcomes in cryptococcal meningitis: results of the multicenter Infectious Diseases International Research Initiative (ID-IRI) cryptococcal meningitis study.

Cryptococcal meningitis (CM) is mostly seen in immunocompromised patients, particularly human immunodeficiency virus (HIV)-positive patients, but CM may also occur in apparently immunocompetent individuals. Outcome analyses have been performed in such patients but, due to the high prevalence of HIV infection worldwide, CM patients today may be admitted to hospitals with unknown HIV status, particularly in underdeveloped countries. The objective of this multicenter study was to analyze all types of CM cases in an aggregate cohort to disclose unfavorable outcomes. We retrospectively reviewed the
