Diagnosis and Management of Idiopathic Persistent Iritis after Cataract Surgery (IPICS).

PURPOSE: To investigate the diagnosis and management of patients with idiopathic persistent iritis after cataract surgery (IPICS). DESIGN: Retrospective interventional case series. METHODS: Patients diagnosed with IPICS were evaluated for demographic and clinical characteristics and immune blood markers. Those with more than 6 months of follow-up were evaluated for treatment efficacy to achieve remission (ie, absence of inflammation for 3 months), with either exclusive slow tapering of topical steroids or the need for systemic immunosuppression. RESULTS: Forty-five patients presented with I

[Candida infiltrations in the iris and lens during iritis and situation after sepsis].

Intraocular Candida infections are overall rather rare; nevertheless they are often found as endogenous infections after Candida sepsis and can be sight-threatening. The most common manifestations are either a sole chorioretinitis or an endophthalmitis. Here we report the case of a 35-year-old man developing Candida infiltrations in the lens capsule and behind the iris after corticosteroid treatment of a presumed HLA-B27-positive iritis. The patient suffered from a life-threatening intensive care stay with positive Candida blood cultures earlier after intestinal perforation. With systemic intr

Herpetic keratitis with iritis after corneal crosslinking with riboflavin and ultraviolet A for keratoconus.

A 21-year-old woman had crosslinking for keratoconus in the right eye; the left eye was scheduled for penetrating keratoplasty. Five days postoperatively, she presented with geographic epithelial keratitis and iritis. Analysis of tear samples by polymerase chain reaction confirmed the diagnosis. The patient was treated with oral steroids and acyclovir, with significant improvement. Two months postoperatively, the visual acuity was improved and there was no evidence of herpetic disease recurrence. Crosslinking can induce herpetic keratitis with iritis even in patients with no history of herpeti

[Recurrent iritis caused by persistent hyperplastic primary vitreous body with pseudophakia lipomatosa].

In a 27-year-old patient a microphthalmic eye with congenital cataract was enucleated because of persistent recurrent iridocyclitis with a secondary glaucoma. Histopathological examination has shown a lens shrunken up to the capsule and a retrolental typical fatty tissue (pseudophakia lipomatosa). Secondary angle closure glaucoma and chronic uveitis seem to be due to an untreated hypermature cataract. These findings and the clinical course are suggesting that the opaque lens and the persistent hyperplastic vitreous body should have been operated in order to avoid complications.

Incidence, Mechanisms, and Clinical Characteristics of Traumatic Iritis: A Population-Based Analysis.

PURPOSE: To describe the incidence, mechanisms, and clinical characteristics of patients diagnosed with traumatic iritis in a U.S. Midwestern county population. METHODS: Retrospective population-based cohort of all residents of Olmsted County, Minnesota diagnosed with traumatic iritis from January 1, 2006, to December 31, 2015. The medical records of patients with traumatic iritis were identified using the Rochester Epidemiology Project database, which contains virtually all medical care in the county. Medical records were reviewed for demographics, presentation, and follow-up data. Incidence

Blau syndrome (familial granulomatous arthritis, iritis, and rash) in an african-american family.

Blau syndrome (familial granulomatous arthritis, iritis, and rash) was originally described in 1985, in 11 members of a family of Dutch ancestry. Inheritance is autosomal dominant. Several more Caucasian families have been described since. Skin and synovial biopsy specimens show noncaseating sarcoid like granulomas, but the lung is not involved as in classic sarcoidosis. This report describes 3 members of an African American family with Blau syndrome. It is important to differentiate this genetic disorder from other childhood arthritides, such as, juvenile rheumatoid arthritis, juvenile spondy

Low-dose methotrexate in the treatment of severe juvenile rheumatoid arthritis and sarcoid iritis.

OBJECTIVE: To assess the efficacy of low-dose oral methotrexate (MTX) therapy for children with severe iritis. METHODS: MTX in a weekly dose of 7.25 to 12.5 mg/m2 was administered orally to four patients (two with juvenile rheumatoid arthritis [JRA] and two with sarcoidosis) with severe iritis not adequately controlled by topical and systemic corticosteroid therapy. The treatment was initiated with half of the total dose and increased every 2 weeks until the final dose was reached. Iritis was graded from 0 to +4 according to the density of cells in the anterior chamber of the eye. RESULTS: T

Migration of acetylated hemicellulose from capillary hemodialyzer to blood, causing scleritis and/or iritis.

From November 1981 to early March 1982, an outbreak of scleritis and/or iritis occurred among patients treated with a Nipro brand NAC series cellulose acetate capillary dialyzer. The rate of incidence with dialyzers produced in 1982 was significantly higher than that with dialyzers produced in 1981. An extract obtained from the dialyzers caused iritis in rabbits after its infusion into an auricula vein. Glycerol, acetylated carbohydrate (AC) derivatives, urethane derivatives, and polypropyleneglycol were found in the extract. AC derivatives caused iritis in rabbits, whereas they caused hyperem

An unusual manifestation of Chlamydia pneumoniae infection: meningitis, hepatitis, iritis and atypical erythema nodosum.

A 37-year-old man was admitted to hospital with fever, muscle tenderness, headache and mild exanthema on the right thigh. During his hospital stay, the headache worsened and aseptic meningitis was diagnosed. A bilateral iritis developed, and the exanthema developed into an atypical erythema nodosum. In liver function tests, pathological results were recorded. Vasculitis was suspected but could not be confirmed. All serological tests proved negative except for a fourfold titre rise to Chlamydia pneumoniae. We concluded that the meningitis, hepatitis, iritis and atypical erythema nodosum were mo

[Iritis following cataract operations. Historical retrospective with critical comments on a so-called "toxic lens" syndrome].

Postoperative inflammation is one of the oldest complications of cataract surgery. It was described by the Indian Susruta as early as 500 BC. Following the introduction of cataract extraction by Daviel in 1745, these operations attracted increasing interest. In 1786 de Wenzel distinguished two types of postoperative inflammation. In the dangerous type there was involvement of the entire eye and severe pain; in the benign type, which began in the first few postoperative days, the conjunctiva and lids were not involved, and even if hypopyon developed there was no severe pain. During the 19th cen

Early-onset pauciarticular juvenile rheumatoid arthritis associated with human leukocyte antigen-DRw5, iritis, and antinuclear antibody.

Evidence has been sought for a genetically determined predisposition among children with juvenile rheumatoid arthritis (JRA) who are also at particular risk for the development of inflammatory eye disease.45 unrelated Caucasian patients (41 female) with early-onset pauciarticular JRA were human leukocyte antigen (HLA) types. 28 of the study group were found to be HLA-DRw5 compared with 16 of 84 controls (X(2), 24.3, P = <0.001). 9 patients were HLA-DRw8 compared with 4 of 84 controls (X(2), 7.51, P = <0.01). Iritis developed in 24 of the 45 children studied, 17 of whom were typed as HLA-DRw5 w
