Janus Kinase Inhibitors: A Review of Their Application in the Vitiligo.

The small-molecular inhibitors targeted JAks (JAK inhibitors) could modulate the cytokines- mediated signaling via the JAK-STAT pathway, which plays an important role in immune regulation and cell proliferation. The JAK inhibitors were previously designed and synthesized to treat diseases involving the hematologic and immune system. Increasing evidence shows that they are quite effective in Atopic Dermatitis (AD), Alopecia Areata (AA), psoriasis, vitiligo, and other autoimmuneinduced dermatologic conditions. Currently, many JAK inhibitors possessing anti-vitiligo activity are being investigate

Clinical significance of Janus Kinase inhibitor selectivity.

Cytokines are key drivers of inflammation in RA, and anti-cytokine therapy has improved the outcome of RA. Janus Kinases (JAK) are intracellular tyrosine kinases linked to intracellular domains of many cytokine receptors. There are four JAK isoforms: JAK1, JAK2, JAK3 and TYK2. Different cytokine receptor families utilize specific JAK isoforms for signal transduction. Phosphorylation of JAK when cytokine binds to its cognate receptor leads to phosphorylation of other intracellular molecules that eventually leads to gene transcription. Oral JAK inhibitors (JAKi) have been developed as anti-cytok

Janus Kinase Inhibitors for the Treatment of Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a disease where the immune system attacks the linings of the joints, resulting in joint pain, stiffness, swelling, and destruction. Although many products are available for the treatment of RA, limitations such as adverse reactions and tolerance greatly affect adherence. Many of the current biologic disease-modifying antirheumatic drugs on the market are injectables, leaving a void to be filled for a product that can be taken orally. The most advanced of these approaches, the Janus kinase (JAK) inhibitors, are oral drugs that have not only made a breakthrough in RA

Epigenomic landscapes define differential Janus kinases inhibitor sensitivity in IFN-γ-primed human macrophages.

Janus kinases (JAK) mediate signaling pathways of multiple cytokines, including interferon-γ (IFN-γ), which plays a pivotal role in rheumatoid arthritis (RA) pathogenesis. Although JAK inhibitors (JAKi) have demonstrated efficacy for RA, their molecular effects on macrophages remain incompletely understood. We investigate the impact of JAKi on IFN-γ-induced gene expression in human macrophages, uncovering that JAKi selectively modulates only a subset of IFN-γ-induced genes. Integrated transcriptomic and epigenomic analyses demonstrate that JAKi effectively inhibits IFN-γ signature genes associ

Pharmacophore-Based Virtual Screening and Experimental Validation of Pyrazolone-Derived Inhibitors toward Janus Kinases.

Janus kinases (JAKs) are nonreceptor protein tyrosine kinases that play a role in a broad range of cell signaling. JAK2 and JAK3 have been involved in the pathogenesis of common lymphoid-derived diseases and leukemia cancer. Thus, inhibition of both JAK2 and JAK3 can be a potent strategy to reduce the risk of these diseases. In the present study, the pharmacophore models built based on the commercial drug tofacitinib and the JAK2/3 proteins derived from molecular dynamics (MD) trajectories were employed to search for a dual potent JAK2/3 inhibitor by a pharmacophore-based virtual screening of

[Oclacitinib in dogs - Janus kinase inhibition in veterinary dermatology].

Oclacitinib (Apoquel, Zoetis) is currently the only Janus kinase inhibitor approved for veterinary medicine in the EU. The drug is approved for treatment of atopic dermatitis and associated pruritus in dogs. For this purpose, it has been used safely and successfully for over 10 years. Furthermore, a number of case reports have already been published which report successful off-label use of oclacitinib, particularly in T-cell mediated diseases, after approved drugs were ineffective. Oclacitinib is only approved for dogs. Its use in cats is not recommended as there is insufficient data from clin

Safety of Janus Kinase Inhibitors in Inflammatory Bowel Diseases.

In recent years, better knowledge of the pathophysiology of inflammatory bowel diseases (IBD) has led to a relevant expansion of the therapeutic arsenal for these conditions. Janus kinase (JAK) inhibitors are a family of small molecules that block one or more of the intracellular tyrosine kinases, including JAK-1, JAK-2, JAK-3 and TYK-2. Tofacitinib, a non-selective small molecule JAK inhibitor, and upadacitinib and filgotinib, which are selective JAK-1 inhibitors, have been approved by the US Food and Drug Administration (FDA) for moderate-to-severe active ulcerative colitis. The role of Janus kinase signaling in the pathology of atopic dermatitis.

Atopic dermatitis (AD) is a heterogeneous, chronic, relapsing, inflammatory skin disease associated with considerable physical, psychological, and economic burden. The pathology of AD includes complex interactions involving abnormalities in immune and skin barrier genes, skin barrier disruption, immune dysregulation, microbiome disturbance, and other environmental factors. Many of the cytokines involved in AD pathology, including IL-4, IL-13, IL-22, IL-31, thymic stromal lymphopoietin, and IFN-γ, signal through the Janus kinase (JAK)-signal transducer and activation of transcription (STAT) pat

Janus kinase inhibitors for alopecia areata: A narrative review.

The Janus kinase (JAK) and Signal Transducer and Activator of Transcription (STAT) pathway has been identified as a key player in the pathophysiology of alopecia areata and a potential target for therapy. Here, we give a narrative review of what is known about Janus kinase inhibitors in alopecia areata. Several clinical trials as well as smaller studies have demonstrated hair regrowth and remission with oral Janus kinase inhibitors therapy, even in patients who failed conventional treatment. Baricitinib is the only US FDA-approved treatment for alopecia areata but data for other oral Janus kin

Janus Kinase Inhibitors: Safety in Patients With Psoriatic Arthritis.

Janus kinase inhibitors (JAKi; or Jakinibs) have become widely prescribed around the world for a variety of immune-mediated inflammatory diseases, including psoriatic arthritis. A previous noninferiority surveillance study of patients aged > 50 years with rheumatoid arthritis and ≥ 1 additional cardiac risk factor raised a number of safety concerns. This review focuses on available safety data from peer-reviewed publications, as well as the most recent presentations from major conferences highlighting JAKi-associated adverse effects. The safety data for several types of JAKi are reviewed. The

The Use of Janus Kinase Inhibitors in Vitiligo: A Review of the Literature.

Vitiligo is a common acquired depigmenting disorder characterized by the development of white macules and patches due to the loss of melanocytes. Patients with vitiligo can be stigmatized by society, making the disease a source of psychological stress that can considerably affect quality of life. The goal of vitiligo treatment is to obtain skin repigmentation in the majority of cases, and less commonly to depigment the remaining normal skin. There is no consistent, long-term, durable therapy for vitiligo for all patients, highlighting the unmet need for new safe and effective therapies to cont

Janus kinase inhibitors: An innovative treatment for alopecia areata.

Alopecia areata (AA) is a relatively common disease, but no satisfactory treatment has yet been developed. Recently, research progress has been made in the pathogenesis of AA, revealing that autoreactive cytotoxic T cells are important and that the Janus kinase (JAK) pathway is involved. Therefore, the potential of JAK inhibitors as therapeutic agents for AA is attracting attention. Several single-arm clinical trials and retrospective studies demonstrated that oral JAK inhibitors are effective and tolerable treatments for moderate to severe AA. Although JAK inhibitors are emerging as an innova

Immunohistochemistry of Janus Kinase 1 (JAK1) Expression in Vitiligo.

BACKGROUND: Vitiligo is a chronic autoimmune disease in which the destruction of melanocytes causes white spots on the affected skin. Janus kinase (JAK) is a family of intracellular, non-receptor tyrosine kinases that transduce cytokine-mediated signals via the JAK-signal transducer and activator of transcription pathway. The aim of the present study is to explore the possible role of JAK1 in the pathogenesis of vitiligo using immunohistochemical methods. METHODS: The current study was conducted in a sample of 39 patients who presented with vitiligo and 22 healthy individuals who were age and

The promise of Janus kinase inhibitors in the treatment of hematological malignancies.

The Janus kinases (JAK) are a family of kinases that play an essential role in cytokine signaling and are implicated in the pathogenesis of autoimmune diseases and hematological malignancies. As a result, the JAKs have become attractive therapeutic targets. The discovery of a JAK2 point mutation (JAK2 V617F) as the main cause of polycythemia vera lead to the development and FDA approval of a JAK1/2 inhibitor, ruxolitinib, in 2011. This review focuses on the various JAK and associated components aberrations implicated in myeloproliferative neoplasms, leukemias, and lymphomas. In addition to rux

Janus Kinase Inhibitors and Stem Cell Transplantation in Myelofibrosis.

Myelofibrosis (MF) is characterized by splenomegaly, blood count abnormalities, particularly cytopenias, and a propensity for transformation to acute leukemia. The current treatment approach is to ameliorate symptoms due to these abnormalities. Treatment with Janus kinase 2 inhibitors reduces spleen size and improves symptoms in patients with MF, but most of the patients eventually have disease progression and stop responding. Allogeneic stem cell transplantation remains the only curative option. However, its efficacy must be balanced against the risk of treatment-related death and long-term s
